  I think sometimes if you wait too long to introduce a bottle, then LO may refuse.  We introduced the bottle at 1 week, because I had to go back to work really soon.  There's a great book called "On balancing breast and bottle" that I found really helpful.  It's basically how/when to introduce a bottle while still breastfeeding.

  Because our baby was feeding well from the start, the lactation consultant I saw recommended introducing the bottle once a week starting when he was two weeks. She said she personally doesn't like the advice given to many moms to wait a month or two (unless the baby has special circumstances, of course) because she thinks it makes it more likely for babies to refuse the bottle if that much time has passed. I'm sure a lactation consultant could observe you with your baby once he/she arrives and give you good advice based on your specific situation, but introducing the bottle at that point really worked well for us. 

  
  

  LO is now almost seven weeks and has no trouble with nipple confusion; he can easily take both the breast and the bottle. I feed from the breast most of the time and have been giving him the bottle 1-2 times per week.

  I had BFing issues so LO got the bottle at 2 days old. She loves the boob and the bottle, at least for right now she is not nipple confused at all.
