C Section question...

When do you find out if you will have a csection vs vaginal birth.

I was asking the nurse at my ob's about the child birth classes today and she said....

"Your having twins right? Aren't you having a c-section?"

I know things can change, but right now both babies are head down and doing well. (knock on wood). I guess I was surprised, but what she said. My OB said it is all based on their position at my first appt.
TTC since May/June 2012

SA #1 Low motility (17%) 
SA #2 Lower sperm count, but much higher motility
CD 3 Bloodwork and ultrasound - Normal HSG - Clear tubes 
Oct/Nov 2013 Cycle 1 with RE 50 mg Clomid + Ovidrel + IUI BFN  
Nov/Dec 2013 Cycle 2 with RE 50 mg Clomid + Ovidrel + IUI BFN 
Jan/Feb 2014 Cycle 3 with RE 50 mg Clomid + Ovidrel + IUI BFN 
2/20/14 Follow up consult RE recommends IVF with ICSI 
3/10/14 New patient consult with RMA NJ 
3/11/14 AMH and other Bloodwork to prep for IVF with ICSI at RMA NJ

  • Wait and see. We have a c/s scheduled if we make it that far as I had one with dd and am choosing not to be induced (baby b is breech and there are enough unknowns with a twin vbac that I don't want that on top of it). If I hadn't made that choice, we'd probably wait and see if I was favourable for induction at 38 weeks and if not at all, go for a c/s. If I go into labour on my own before the scheduled c/s we might give that a try, depending on how things are looking.

  • We didn't schedule my csection until I was 36 weeks because at that point if they hadn't flipped positions, they probably weren't going to. My baby A was head down and baby B transverse. They said I could request and ultrasound when I was there before the csection to be sure they hadn't flipped, but by that point i was already in the mindset of having a csection that I just went with that. It all depends on the positions of the babies and if other complications arise.

  • My water broke at 35.3 wks and they did an ultrasound in the hospital and realized they needed to do a csection. Until then we were waiting to see if would flip.
  • We had planned on vaginal birth all along because my girls were head down. My doctors agreed that I was a really good candidate for vaginal birth. They induced at 38 weeks but it didn't work and after 4 days of failed induction, they did an emergency C-section because twin A was getting a little stressed in there.  So, you just never know.  I would tell your doctor early on if you would like a vaginal birth.  If they are both in position, it should be ok and then at least you two will be on the same page and can re-evaluate the situation as you go.

  • Since around 22-24 weeks baby A was breech and baby B was transverse. They never moved from those positions. We still waited until 36 weeks to schedule a c-section. And had a u/s that morning to see if they flipped.

  • Thank you everyone for the feedback. I am not opposed to a c section if it is the safest, but I would like to try a vaginal birth if I am able to. The one thing I know I don't want is the double whammy! ( not sure how common that is).

  • It's been wait and see my entire pregnancy. Unfortunately A only spent about two weeks with her head down -- the rest of the time she has been breech. I think we all gave up hope of her flipping around 35-36 weeks and started making plans for a c section. That also might have something to do with the fact our baby b is so much larger. Even if she flipped last minute, I am not wanting to try a vaginal.

  • I just responded to someone who asked about vaginal birth. In addition to other things I mention, in my humble opinion, I think if you're interested in vaginal birth it may be helpful to avoid induction unless medically necessary (e.g. in my case I had pre-eclampsia) and let the babies dictate when they arrive as far as possible. Hopefully your healthcare provider(s) will be supportive.
  • I'm scheduled to be induced at 38 was if they havent come out on their own first. A is head down and B us breech-- but my MFM does breech extractions. A lot of doctors won't do them and automatically do a c/s if breech. So it's a wait and see thing usually.
