Trouble TTC

New to 3T

Hello,

I just met with an OB who also does IUI. My husband is 6mo post-op vasectomy reversal with struggles in ejaculating. With that being said I will be going in for the ultrasound to check my ovaries during ovulation to make sure I am ovulating as he suspects. If that does good then next month might be our first month with iui. They did do a blood draw today for TSH levels only.

I was wondering about what experiences people have had with iui since this is a new world to me.

  • Unfortunately you won't find many success IUI stories on this board as the majority of us are still waiting for our BFP's. In my experience the actual IUI procedure isn't that bad. Slightly uncomfortable getting the catheter in and some cramping throughout the day. It really varies though. Some people say it's a breeze and some have to stay in bed for the rest of the day. I wouldn't be too worried though, most seem to not have many issues.

    I strongly recommend seeing a RE instead of your OBGYN. They are properly trained and specialize in IF and will give you proper treatment. I'm not sure exactly how much an OB would know about male fertility issues which is your main concern right now. I also recommend getting more testing done on yourself before starting IUIs. Hopefully all is clear on your end, but if there are issues you will want to know before wasting time and money on treatment that might not work. For example, you may have blocked fallopian tubes, but you can't know that from a blood draw. If that were the case an IUI doesn't have a chance of working since sperm has no way to meet the egg.

  • Thank you! The doc I am using has 15 years experience with OB/iui/Ivf and has me coming in for an ultrasound at a peak LH. We will/could work with an urologist at 1year post vasectomy reversal but they won't until then.

  • As long as you're comfortable with your doc and keep yourself educated you're doing the best you can. The ladies on this board are incredibly knowledgeable and helpful :)

  • Good luck with the IUI. I am in the same boat as you and will hopefully start our first IUI in the next few weeks. These ladies are a wealth of knowledge about IUI's and everything they entail. Welcome!

  • I've had one IUI and it was a breeze. The nurse doing it said I had "the cleanest cervix of the day" (I guess that's a compliment?) so it went super quick and was painless. I did have cramping the rest of the day, but it wasn't anything that I couldn't handle. Like @10v3 said, I (and probably the rest of the ppl that will comment) would see an RE vs an OB since they specialize in getting you PG, while OB's keep you PG. Also, make sure you have had all the proper testing and get proper monitoring while going though the process of fertility drugs/procedures.

    who know I'd ovulate on my own after a cancelled cycle and end up with a mega cyst :(

  • Thanks ladies. Yes they are checking my ovaries for follicles are growing correct. We know we have a male factor already due to a VR along with him struggling to ejaculate enough. The dr I am seeing has 15 years doing iui and is on call 24 hours a day. The only doc in this practice. If he finds my TSH is not where he wants it and my ovaries are not what he likes we move to next testing. Hopefully that makes sense

  • What @murrt said. The original post sounded like you did not have thorough testing or monitoring with this doctor? 

    IUI for me is briefly painful and then uncomfortable due to cramping, soreness, cold/hard table for 30 minutes. Totally worth it though!!

  • Welcome! The IUI process is not scary at all, although I am sorry that you have to go through it. I wish you the best of luck.

    I am currently seeing an OB who works with IF (we have issues with the donor sperm we are using, so even though I would prefer an RE, right now it is the best option for us). Like everyone has mentioned, it is better to see an RE unless there is some outside consideration. That being said, one thing that I have learned from lurking on this board is that you have to be very proactive about what treatments you get and why, especially if you are seeing an OB. 

    @murtt is right on about getting properly testing before IUI and properly monitored during the process. It doesn't mean that your OB won't take care of that, but I feel like you have to be a little more informed. My OB probably thinks I am the most annoying person in the world because I am constantly up in her business about my treatment. I definitely think getting an HSG and SA are important before doing an IUI. Good luck with your ultrasound! Hope your stay here is short :)

  • Welcome. Murrt gave great advice about all the testing you should make sure to have before IUI. If your OB is getting you all the tests and monitors IUIs then you are one lucky person. I was wondering though since you said your OB has experience with iui/ivf, do they work about of a fertility clinic? I have never heard of a regular OB being involved in IVF other than possibly local monitoring if people are cycling out of state. 

    Welcome to 3T, I am curious if your OB is affiliated with a fertility clinic or RE also? I've never heard of an OB doing IVF either, so I'm just curious also. I have only had one IUI but it is a fairly easy procedure, I had a little cramping and it was difficult for the nurse to get the catheter in but all in all it was simple and painless. Good luck.

  • He is not affiliated with a fertility clinic and I am not sure about the RE part of it.  I know he has been in the field for many years with many years experience.  He is has a very good and high reputation in the area as an OB and also in the IUI area.  

    We have done the SA and doing a repeat do to the VR.  He is checking my TSH and then will be checking my Ovaries for follicles, and then I do believe the Endometrial thickness. Depending on how those look he will do other testing.  We are in the prelim and can't wait to see what is going on with my body.  

    Please remember we have a FOR SURE MALE factor for issues at this point.  We are only doing the testing on me to make sure I have a chance for IUI, and this is a choice we are choosing to do not required to do.

  • Got BAD news today..... at this point no IUI happening.  Got DH's 6 mo SA from post op Vasectomy Reversal.  Well good news is count went up but the bad is motility is at ZERO%.  Waiting to hear back from the Urologist who performed the VR which won't be until end of next week!  :(

  • First of all welcome.. Second sorry for your bad news.  Hopefully the urologist will see the importance and get you in quickly and start some more changes.  There are a lot of ladies whose DH have MFI. I believe there is even a check in every week for this.  Maybe they can give you some advice... As for the questions about your monitoring.  The ladies are not trying to be rude but are trying to ensure that you are being monitored and tested correctly as it can hurt you to not do so...

  • @larsonbussman‌ My husband had a VR 2.5 years ago. He has between 9 and 21% motility depending on which one of the many SAs you look at. Since its something that was never explained to me before the VR, the potential for antibodies following a vasectomy is not unusual. The surgeon that did his surgery did not tell us that, and the SA they did even as a fertility clinic could not verify the presence of antibodies, but said it was likely and referred us to a University that happens to have lab to test for that as well as a couple options for treating it. For the first year of TTC we knew DH was a hurdle. My OB assured me that since I was ovulating and having regular cycles it wasn't necessary to do fertility testing because of my age even though insurance covers testing. 6 months ago we're referred to an RE by MHs urologist and we find out that I have a blocked tube. So it's important to get as much testing as you're able regardless of the route you choose.

  • Welcome and I'm sorry you have to find yourself here. I've had 3 IUI's, my first was done by Nurses and it was painful since they had trouble with the catheter, the last 2 were done by the RE and it was a breeze. Good luck.

  • Thank you all ladies for you advise.  I started to research this morning about what Sertraline and motility.  I did find that it can cause motility to decrease, and with an already low motility from the Reversal I am hoping that is the cause and not an antibody issue.  I guess only time will tell over the next couple months on what we find out.
