Trouble TTC

New to 3T (Introduction - Loss Mentioned)

edited October 2014 in Trouble TTC
Hi Everyone! I've been lurking a bit so I thought perhaps it was time to make an introduction.

Here's my story: my name is Meredith and I am fairly active on TTCAL and TTC35+. In January I got a BFP after going off BCP in the fall of 2013. At 9 weeks I started spotting and cramping and a missed miscarriage was diagnosed; the embryo was between 6 and 7 weeks. I was benched for one cycle and then started NTNP for a bit as I warmed back up to the idea of TTC. I have gotten more serious the past four/five cycles. I had been using OPKs all along, but now I am also temping. It has been 6 months of trying without any result and I have an appointment with an RE the first week of November.

The only known issue at this point is submucosal fibroids. I have had them for a few years at least. I recently went back and looked at the radiology reports. In 2011 the largest one was about 5cm. I was told by 2 OB/gyns and a perinatologist at the time that at 5cms they weren't particularly concerned. If I recall correctly, the perinatologist told me then that she would worry if it was over 7 or 8 cm. Well, as of my ultrasound in March the largest is 8.5 so I am concerned. I really like my OB, but this is not her area of expertise so hopefully the RE can help me out.

I am somewhat familiar with the tests that REs do just from reading others' posts, but now I am the one who will be facing this and I know I will be looking for all the insight I can get. I am looking forward to getting to know those of you I don't already know from the other boards. I am sorry, however, that any of us are in need of this board at all. Thanks for taking the time to read my story.

ETA: fixed grammatical error.

  • welcome!  If you don't already know what to expect at your first RE appointment, I would suggest doing some quick reading up.  I didn't even know the ultrasound was internal, and I therefore found it all a little overwhelming.  Good luck!
  • @rainbowbridge14 - Thanks for the shout out!  ;)  I'm not a fibroid expert by any means but I'm happy to share what I know ...having dealt with them quite a bit recently.  Sorry for the late response here ...I just returned from a 9 day trip to the East coast/Bahamas late last night.

@meredithcarole - Welcome but I'm sorry that you have to be here.  Big hugs for your loss!  Dealing with IF is hard enough but I can't imagine the emotional rollercoaster of a loss since I haven't been there.  My heart goes out to you and your DH.  I know that pregnancy causes fibroids to grow in size so it's not surprising that yours grew to 8.5 at the most recent check.  I actually believe (although not even my RE will confirm) that the fertility meds (at least Clomid) causes them to grow too.  I have 2 subserosal (on the outside wall of the uterus) fibroids that were measured at 4-5cm back in 2011 and after 3 rounds of Clomid they are now 8x9 and 11x13 (Crazy, right?).  I'm very frustrated because my RE told me right before starting the Clomid that I did not need surgery for them.  I wish I'd followed my gut and insisted on surgery.  I was so upset to discover how much they'd grown and that I now needed surgery for sure.  I have been waiting on surgery since July and finally have it scheduled next month (long story short - My 1st Dr said it could not be done laparoscopically and would require an invasive laparotomy.  I saw a 2nd Dr who prescribed me Lupron, a med to shrink the fibroids and said she'd do the surgery laparosopically Nov 5th ...she got injured and so I had to cancel my surgery with her and find a THIRD surgeon ...now scheduled for laparoscopic myomectomy on Nov 21st).  In your shoes, I would definitely opt to just get surgery and remove the fibroid(s).  Yours are submucosal which are inside the uterine cavity - these definitely can contribute to miscarriage and infertility.  Mine (subserosal) are usually considered to not be problematic ....but at the size they are now, they definitely need to come out.  I hope your RE will explain your surgical options to you and find a plan you're comfortable with.  I'm speaking from experience ....I wish I'd listened to my own gut instead of 3 different Dr's (including my own RE) who told me I didn't need surgery.  At 8.5cm, I'm pretty sure you could get it done laparoscopically with no problem.  Recover from surgery and resume TTC 3 months later.  This is all just my opinion of course.  But I just hate to see someone else put off surgery and have the fibroids grow even more (which is what happened to me).

    Me 34   Him 33

Me - Left salpingo-oophorectomy at 19 due to large cyst/torsion  and 2 large uterine fibroids found at age 30
Him - Borderline low sperm count (correcting through Fertility Blend supplement)

    Married 8/20/2011
    TTC #1 since Jan 2013
    First appointment with RE 10/2013.
    April 2014 - Clomid 100mg CD3-7.  HCG shot CD10 IUI #1 done on 4/12/14 (CD12) ....BFN
May 2014 - Break cycle to repeat saline sonogram and re-group.  Travel to Kauai 5/7-5/12 (Yay!!)
    June 2014 - Clomid 150mg CD4-8.  HCG shot CD14.  IUI#2 done on 6/9 and 6/10 (CD15-16) w/ Prometrium suppositories during TWW .... BFN
    July 2014 - Clomid 150mg CD3-7.  Ultrasound CD12 - 4 mature follies! HCG trigger shot CD12 AM.  IUI#3 on CD13 w/ Prometrium suppositories during TWW ....BFN
July 2014 - Repeat ultrasound done to measure size of fibroid - 8x9cm (the size of a grapefruit!) Surgery referral made.
August 2014 - MRI done which revealed 2 fibroids: a 13x15cm fibroid as well as an 8x7cm one.  Super freak out mode over surgery plan.  OB-GYN/surgeon said surgery would most likely have to be a laparotomy (open abdominal surgery)
August 2014 - After a 2nd opinion, plan to take Lupron x 2 months to shrink fibroids then laparoscopic/robotic myomectomy.  Surgery date is set for Nov 21st!
November 2014 - Robotic myomectomy done (2 large fibroids removed, mild endo found).  Benched x 3 months

    PAIF/SAIF welcome
  • @eastbaybride508  Thank you so much for the information.  I really hope one of the first things the RE says next week is "those need to come out!"  I will be sure to advocate for myself, though.
