1. In the past week DS has been waking up 30-60 min after we put him down for the night. Sometimes I just need to rock him for a couple min and sometimes I nurse back to sleep. Could this be s growth spurt thing or should we change his bedtime? He is 6 weeks old.
2. Do you change LO's diaper AF every MOTN feed? Especially those that swaddle, what do you do? I've read that it's not necessary to change every time and I have noticed that the past two nights DS only has wet diapers, not poopy.

I change for poop, or if its been a long time since the wet was changed- like if shes up for a 5/6 am feed I usually change her then, because she's largely stopped pooping overnight, that diaper's been on since maybe 8-9 pm. We'll go back to every time changing of she gets rashy, but so far she's OK
We don't change unless its really wet or poopy. We swaddle with a woombie (actually the cheaper Summer Infant version) and it has a two way zipper so I can change his diaper and not unwrap his arms only his legs. They didn't have this feature five years ago when I had our daughter so it was a nice surprise.
Edited to answer first question.
I need to check out the woombie! DS sleeps better swaddled but fights it for a little bit in the beginning so getting him in it again after a change in the middle of the night isn't always fun
Yes I do feed before bed. Our routine is bath, pjs, feed, swaddle, rock/finish nursing to sleep. If he keeps waking up maybe we will shift his bedtime to be a little later and see is that works.

I have found that a little later does make a difference. For us anytime before 830 results in waking up every three hours all night but after 830 he sleeps for a long stretch. Sounds strange but that is what we have noticed.
Also the woombie I great because they can move their arms (for those babies that like to) much more but they can't get out. He rarely fights it and we don't have to take his arms out to change him.
