Has anyone gotten her OB to fill out the FMLA papers yet?

(I apologize in advance if this was asked before but I'm on mobile today and couldn't figure out how to search the board.) I just printed the forms out from the Dept. of Labor website since my HR person told me to find it there and I'm bringing it to my OB on Wed. I really hope the office will fill it out now even though I'm not due until 12/9, because I am pretty sure I will forget about it in November when getting ready for baby. Also I figure they would want to fill it out ASAP since you never know for sure when labor will happen, but nobody seems to be in a hurry to ask me for it (OB and job included). Just wondering if anyone else did this already or if you were told it was too soon. I know there are some pretty specific questions on there. Thanks!

  • Hi there. Yes, my OB filled mine out 3 weeks ago & I'm due 12/22. You shouldn't have any issues. Hope things go smoothly for you!

    April 2012=IVF#1- EPP Antag+ICSI, 2R,0F (BFN), Now dx’d with DOR.

    June-July 2012=IVF#2- MDL+ICSI&AH, 8R,4M,3F (BFP 9dp3dt) Beta#1 at 11dp3dt=36, Beta#2 at 15dp3dt=156, Beta#3 at 19dp3dt=671, dx'd with SCH, no growth-m/c at 7wks/Lap Dec 2012 to remove small fibroid.

    Feb 2013=IVF#3-MDL, 2/1=baseline, started 10 units mdl, AFC-7, 2/3 start gonal f, self cxld cycle b/c of low e2.

    April 2013=IVF#3.5-(with new RE)AG/ANT Conv + ICSI. 4/10-4/23 bcp's, 4/20-4/27 lupron, 4/28-ganirelix until end of stimming, 5/2-600 gonal f, 5/4-add 1/2 vial menopur, 5/13-ER (9R,1M,1F), 5/17-ET, 1 beautiful 8 cell (please be my sticky baby!!!!) 5/28-BFFN.

    Oct 2013=IVF#4-LLP+ICSI &AH, 10/14 (6R, 2M, 2F), 10/17-ET, 1 seven cell & a six cell, BFP at 9dp3dt, 1st beta=56, 2nd beta=52, CP.

    Jan 2014=IVF #5-LLP, Cxld after 6 days of stims due to fast response and lead follicle. 

    March 2014=IVF 5.5-LLP, Lupron 3/10, BL 3/18, 11 days of stims, Trigger 3/29, ER 3/31. 7R, 2M, 2F. ET 4/3. Txfd one 5 cell & one 9 cell. BFP on hpt from 7dp3dt & on. Praise be to God. Beta #1 at 11dp3dt=106, #2 at 13dp3dt=239. First u/s 4/28, measuring on track & heartbeat seen. 5/5-2nd u/s, measuring on track with strong heartbeat. 5/12- 3rd u/s & released from RE. Grow baby grow, we love you! Baby G&T is a BOY! Born 12/2014 via c/s! 8lbs, 8oz & 21 inches.

  • I got mine filled out in august. I asked my hr dept for it early since I'm having twins and they had no problem with it

  • Yes. Btw, all, is it normal for your doc to have a form fee?? Have to pay $15 to have it faxed to my work.

  • My company's short term disability provider, Liberty Mutual, does all that. I just had to fill out an authorization to give to my OB to keep on file.

  • Hmmm I guess my work doesn't do that? The only thing that I think needs to happen is disability papers need to be signed. Which I find weird.

  • I did mine in August. I've had weekly ultrasounds requiring me to take off 1 hr. early every Thursday from 16 weeks on. I've been able to use FMLA time. For me it's easier to have a few less hours of pay now and save the PTO hours for when I'm off unpaid for 12 weeks!
  • FYI for those who don't know what it is u may not qualify. Only companies of 50+ employees qualify for FMLA

  • I was told by a coworker at another location that it took her 6 weeks to get it all settled so I am going to start this week. Our hr for this stuff is offsite so it is going to be fun figuring it all out!
  • I did mine about three weeks ago. I filled out some stuff online, called the STD and company that handles my company's FMLA stuff, fielded one question from my OB nurse, and got the official judgement letter last week. I only have to contact them now if I go sooner than anticipated and my leave needs to start sooner.

    On this same vein, make sure you know what you'll need to provide and when for your insurance to pick up baby on the policy. With my policy, we have 30 days to contact the underwriter and fax them a copy of the baby's crib card from the hospital. Just make sure you have any paperwork you need before you leave the hospital.

    Yup, my OB charged $20. I had them do it back in August since they had to do one for my gallbladder surgery anyways. They charged me for both so $40... that sucked.
  • Mine was weird, they asked me what date I wanted to be off from. I said well my workplace offers from 38 weeks....and they were like so if that's ok with you, then what's the date? So I got the papers back and it has that I'm unable to work from 38 weeks. I felt weird because I thought you'd have to wait until that date to determine disability to work? Anyway, we just did it last week (due dec 4), so I'll see what the company has to say. They said it can take up to 2 weeks. I'm glad I filled it out early because I've heard of issues with salary and so on.

  • @shinycat that is the form I have downloaded too. I am pretty sure it is the right one because it seems to be the only form on the site requiring doctor approval.

  • Mine filled it out no problem... they were suppose to charge a fee but told me not to worry about it. Maybe it comes on the final bill.
  • I'm in awe that some of you were charged! That's ludicrous. Why are they charging you?

    I just need to get the form from my HR again. I think last time they said 6 weeks. My dr filled it out with an anticipated leave date which was my due date and either i or she faxed it in. I don't remember who did it.

  • My OB just filled out the same paperwork 2 weeks ago at my visit. No problem whatsoever (and no fee, that seems really weird to charge an established patient!). He just put my EDD as the date it starts, but my HR department understands it is subject to change based on when I actually deliver.

  • @Halsing‌ I know how you feel about HR not providing answers. I have gotten ALL of my info. regarding maternity leave from my coworkers and basically I feel like every question I ask HR is bothering them. They never told me to fill anything out but my coworkers filled out the FMLA form so I'm following their lead. That being said, my leave has already been approved but I want it in writing that I fall under FMLA and this is the only way I know to ensure that.

  • I need to remember to bring mine to my appt this week. I didn't have to do it last time (I probably should have but my work didnt have an HR dept at the time and no one was very good at knowing what needed to be filled out.) Not sure if they charge a fee but it's good to know some places do so I'm not blindsided when they ask for it.

  • I keep forgetting to bring in the paperwork to my appointments! My hr is out of a corporate location and have been pretty useless when it comes to FMLA. They wouldn't tell me what time off I have to use but told another coworker at a different location that she had to use all her vacation and sick time. The only information I can get out of them is the 1800 number for the short term disability company.
  • I brought my FMLA papers into my last OB check up (my HR rep sent the papers to me and requested I get my OB to fill everything out) and my OB refused. She said she won't fill anything out until the baby is born :-w

  • I had my dr fill out my paper work a few weeks. Took no time at all. I don't think my HR did anything with it yet but I felt better knowing it was done.

  • I'm in CA where disability can begin at 36w0d. So I brought the forms to my 34W appt, the OB had her nurse fill them out and fax them back to my HR contact. Then HR contacted me a few days later saying we got your stuff, good luck with baby and enjoy your leave. Pretty painless.

  • Just called my doctor because this thread made me nervous! They will do it for free, they just told me to fax them rather than bring them in at my next appointment because it takes 14 days to turn around. And I just have to send a note with all the details since my leave starts before my due date.

  • So apparently I didn't even need to fill the forms out because I am only taking Federal Leave and not FMLA. Now to figure out whether I have to send any forms to the state for Federal Leave...they certainly don't make these leaves easy to figure out!

  • FYI for those who don't know what it is u may not qualify. Only companies of 50+ employees qualify for FMLA

    You also need to have worked at your place of employment for one year, as well as a certain amount of hours.

    My OB does the first form free and charges $15 for any additional forms.
