mastitis?

hi ladies,

this morning my breasts were engorged but I had no time to breast feed my baby or pump before getting to work due to me getting up late (we had a 9 days Eid holiday here and today is the first day of getting back to work) I normally pump at night before going to bed and my baby takes BM in her midnight feeding in a bottle by my DH or me, it's easier this way and has been this way for a while (my baby is 2 months 3 weeks) I normally however wake up with full breasts of milk and my baby feeds a few times between 5-7 am and she empties both sides or at least half empties them, today we didn't though.

I pumped at 10pm last night and didn't pump again until almost 9 am this morning and the flow of milk was slower of normal and i didn't get as much milk as I would have with such engorgement. I double pumped (electric) for 22 minutes and still didn't feel empty and I feel that milk is mostly gathered in a wedge shaped lump in my left side, it hurts to touch it and my breast is warm. I tried massaging it while pumping but it didn't go away and it hurt. I pumped again 2.5 hours ago and again the lump is still there and it hurts, when I massage it I feel the milk flow faster into the pump but it didn't go away. I will be pumping again in 30-40 minutes but I'm fairly uncomfortable. Not engorged anymore but the lump area hurts when touched a lot...can't do compressions while at work and the cold doesn't help.

Do I need to go to the doctor's or would pumping/nursing a lot resolve this issue? I don't feel feverish/flu symptombs

  • Is it getting better when you pump? When I'm really engorged sometime let down takes longer and I have to pump longer-even 30 minutes if you can spare it. Try also leaning forward when you pump if you can and massage around the spot or on the spot while pumping.
    I would think that pumping and nursing should resolve it (nursing is better) unless you develop a fever. One tip a lc gave me was when you develop knots like that, point your baby's nose toward the spot and it should help a little more.

  • I pumped and then had my baby nurse and it really improved when she nursed as oppose to when I pumped but I can still feel it, it doesn't hurt anymore but it's tender and it's definitely still there but smaller

  • I nursed my baby all of her home feedings last night, mid of night, and this morning as oppose to pumping and giving her a bottle and I feel so so much better, the pain is gone. I'm just surprised that this happened to me now and not earlier

  • It sounds like a blocked duct. I had a couple spots where my ducts blocked frequently, so I would massage them while I was pumping any time I felt like the kids maybe hadn't nursed as much or I hadn't gotten as much pumping. Over time you will figure out where your "hot spots" are and can work to prevent a real blockage as soon as it starts. Mastitis you will know - I have never been that sick in my entire life. Roughly, spot on breast, no fever = duct issue that might lead to mastitis; fever/chills/headache starting, head into the dr.

  • Good that it went away. Sounds like a blocked duct. It may still be sore for a while - don't worry about it. If you keep getting persistent plugged ducts get Lecithin granules supplement. Works great at preventing them.

  • thank you ladies...so what do we do once our baby wean off the night feed and start STTN because my baby is definitely heading that way soon. she gets a single MOTN feeding and a couple of times recently she would skip that feeding all together.

  • Your body will adjust once she starts STTN. You don't have to still wake up and pump or anything!

  • If you are still engorged after the morning nursing session, massage. Pumping would tell your body to make MORE milk. If you want more milk, by all means pump in the morning, too. But except to provide an ounce of relief if you think you have a blocked duct, I would not pump to offset the skipped feeding unless you are trying to augment production.  After a few milky mornings it levels out.
