Full of Follies

Hello Ladies!  Today I went to my CD3 u/s appt. Ute looks all clear. The RE checked both ovaries and count the mini follies. Last cycle I had 8 follies on one ovary and 10 on the other. At time of trigger, only 1 follie had grown to 18mm with a couple smaller ones. Today I had 17 follies on one ovary and 21 on the other!  I asked the RE if the significant number of follies (total = 38) was cause for concern. RE said well we don't need that many for sure and reminded me that not all of them will mature. If logic prevails, then I can expect 2 mature follies this cycle based on my response last cycle, however, I know its not that simple.

I am curious if other ladies start out with a large number of follies at the beginning of their cycle. How many develop to maturity?  Have you had a cancelled cycle as a result?

Here's hoping for a couple good follies to grow healthy and strong.

Re: Full of Follies

  • That's a lot of follies - usually associated with polycystic ovaries. But as long as the doctor keeps good track of their development with monitoring and checking on your doses for injects, their plan is to help a couple come to maturity. The developing follies themselves produce hormones to signal your body to halt the maturity of the other follicles, so whoever gets into the larger range first will just kind of take over.

    Even women without polycystic ovaries still have many follies in various early stages of development hanging around at the start of each cycle. Basically, one that grows to maturity in a given cycle would have started its life in a previous cycle as a small one, then development was paused when another mature follie took over in the previous cycle, and now that one has a head start and becomes the mature follie this time around.

    Our office doesn't bother to count each immature follie at baseline, but simply records there there are "many" or "over 10" on an ovary...but in the end I've never had more than 1 mature! Having more antral follies can put you at a higher risk of overresponding, but the RE should have good experience with keeping the right balance.

  • I have never had my follies checked before, but I wish you luck on your next US for some good growth.
    Me-33 DH-36.
    Married 11/10 TTC 11/10
    06/11- diagnosed with PCOS 
    7/11- D&C and benched for a year 
    9/12-4/13-6 rounds of 50 mg Clomid- no sucess 
    6/13- new doc
    7/13-4/14- 6 more rounds of Clomid. 50 mg and then stepped up to 100mg- no success 
    5/14-7/14- took break waiting for new insurance 
    7/14-new insurance= new doc- actually a RE!!!! 
    7/14-HSG and blood test good-PCOS confirmed by new doc 
    10/14 looking forward to 1st IUI w/ Menopur. Blood Test= Negative
    11/14- IUI #2
    w/ Menopur- Blood Test= Negative
    1/15- IUI # 3 w/
    Menopur- Blood Test= Negative
    2/15- Self Benched this cycle

  • @BunnyBerry Thank you for the feedback. What you wrote makes a lot of sense. Although, none of my doctors have ever officially diagnosed me with PCOS because I do not show all the signs. I have had doctors tell me I have PCOS tendencies. Also, I had no idea that mature follies halt the growth of the immature follies - good to know!

    My RE has me come in on CD11 and CD13 for monitoring. Last cycle I had only 1 low dose inject of FSH before the trigger. I am confident in my RE offices monitoring.

  • I don't know the exact number that I have each month but I know that each ovary has 15+
    With injects, I've had between 2 and 6 mature follies. Usually, we triggered sooner rather than later to prevent more mature ones.
    This last cycle with letrozole, I had one 24mm and one 12mm

    4 Losses (2003, 2008, Apr 2012, & Oct 2012)
    All RPL and IF testing with multiple REs = normal

    5 IUIs = BFN

    All AL are welcome

  • I had over 30 follies during one IUI cycle and 1 mature by the end.  Another cycle, I had more than 20 on each ovary, to the point where they didn't count them all and 1, maybe 2 matured.  I don't have PCOS, but my AMH is slightly higher than normal.

  • Hey love! My situation is kind of unique due to my "arthritic" ovary. My right ovary is the one that a dermoid was removed and it's never been the same since. It's misshapen and only produces about 6 to 7 follies. My left ovary is a trooper and this CD3 had 23 follies growing. Everyone seems to be in agreement that I only ovulate from my left side. Lefty and the left ovary are my only hope of conception. So far, I have had only one mature follie every cycle, with some others that are very close seconds.
