Pregnant after IF

XP from IF: Thoughts on protocol, dx as RIF

Hello Ladies! I am looking for some feedback about the plan my RE has for us. We are now considered/diagnosed as Repeat Implantation Failure (RIF) we have transferred 5 high grade blasts with a CP to show (and that was with a endometrial scrape that cycle). Our plan currently is an antagonist protocol with hysteroscopy and repeat endo scrape prior while on BCP. RE also wants to try trental and levaquin (antibiotic) for 6 weeks prior to starting treatment to help with blood flow and any potential inflammation. Also vitamin E for 6 weeks prior to stims. We will do a higher dose of stims to get more eggs, do assisted hatching of the embryos and he wants me to take benadryl 50mg three times a day from ET until a heartbeat is seen. We had discussed intralipids as well, and he has been conversing with the RMA NJ and apparently now isn't sold on the intralipids.  My gut is telling me the intralipids will help. I'm not so sure about the antibiotics and benadryl.
I have been around IF boards for a long time and don't recall hearing about any similar protocols and am curious if you were in a similar boat as me what led to your successful cycle!?

Feb-April 2013: Femara + TI: BFN
May - September 2013:  Follistim + Ovidrel + IUI#1-4 = BFN
IVF # 1 November 2013: transferred 1 perfect blast = BFN
IVF # 2 April 2014: Endo scrape, transferred 2 blasts = BFP!! (first ever!), CP
FET #1 June 2014: transferred 2 blasts = BFFN

  • Have you had any immunological testing done? I'm not well versed in your diagnosis, but it would seem a reproductive immunologist may be a good person for you to see. Some REs are very opposed to RI, and I'm not quite sure why.

    I know there are some ladies floating around here who have been to RIs; hopefully they can chime in too!

  • I don't know a lot about RIs or ways to treat RIF.  But I know for my FET the embryologist did assisted hatching for both embryos.  She had said it was standard protocol for them on all FETs, and they had a high success rate with it.  I was initially worried about them messing with the embryos, but so far it seems to have helped.  I sometimes wonder if that's why my beta came in high (with an early implantation), since FETs normally implant late.  But that's just my random opinion.

  • I know just enough about implantation failure to be dangerous, so I'm going to keep my mouth shut. You might try asking this question on the IF vets page, if you haven't already, as a fair number of them have walked this path.

    I hope this cycle does the trick for you!!

  • We have had three pregnancies and three losses. Before our last loss, we saw an RI, we added intralipids, benadryl, and lovenox.
    We had our last loss of twins and found out they were genetically normal. 

    At that point it was very clear to me that I could not carry our children. We moved on to gestational surrogacy and are now expecting in November.

    It is not an easy decision to make, but it just became very clear to me that this was our only option.
