As we were driving to the office for our transfer the Nurse called and said they are pushing us back to a 5 day transfer. Three of the four embryos are growing, the fourth is a bit slower (4 cell). She said 5 days could mean blastocysts which have higher success rates and better selection.  ?
I'm nervous that this is not good, but really do not have enough knowledge (I'm resisting asking Dr google for now). Do any of you ladies have knowledge or experience with this? Positive or negative experiences are welcome, I want to be realistic.  My previous two IVF cycles were three day transfers (one successful, one failed). 
Update: we made it to 5 day and transferred two blasts! I'm so excited and relieved. As we drove to the clinic I kept hoping my phone would not ring. The Dr. was optimistic, said the embryos looked good :) we might even have a third to freeze, will find out later in week, that would be amazing.

  • I transferred and froze 5 day blasts. My fresh transfer was successful with two embryos. The first frozen transfer was not but was a single embryo transfer. Our second single embryo transfer was successful. :\">
    TTC since June 2011
    DH diagnosed with Testicular Cancer 8/2010
    Low sperm count. Only option IVF with ICSI
    Starting First cycle Jan/Feb 2012- Canceled due to not responding to BCP and/or Lupron
    IVF 1.2- stims March 14, ER March 24, 16 R 10F 6 made it to blast
    ET March 29- transferred two blasts, 4A and 4BA, froze 4
    Positive HPT 9dp5dt Beta 1- 385 11dp5dt
    u/s April 25- one healthy heartbeat
    EDD 12/12/12>

  • We ran into a similar situation prior to transfer for IVF #2. All 4 of our remaining embryos were still growing well and developing normally on day 3, so we were pushed to a day 5 transfer (our previous transfer that resulted in DS1 was a 3dt). DH and I were pretty shocked because we never thought we would be pushed to a 5dt with only 4 embryos to work with. But our RE was confident that we would be able to better choose the best 1 or 2 candidates by day 5.

    I was so terrified that none of the embryos would be left by day 5, but to my surprise all 4 of them had developed into blastocysts by then. We chose to transfer the best 2, under the advice of our RE. That transfer resulted in the birth of our twins :-) 1 of the remaining 2 blasts also made it to freeze.

  • My RE's goal with us was to make it to a 5dt, which we did for my fresh cycle and the result was DS :)  From what I know, transferring a blast is the best possible situation.

  • I have gotten pregnant from d3 and d5 ETs. It is scary getting pushed to day 5, but when the embryologist can look at blasts, they really can tell which are the strongest embryos and therefore, what will give you a better chance at a pregnancy. Good luck!

  • I did 5 embryo transfers. 2 of them were with 3 day embryos and neither of those worked. I then switched RE's and he said that he prefers a 5 day transfer (when there are a available about of emrbyos) b/c he said at day 3 it's hard to tell which ones are going to continue to grow and which ones aren't. Apparently by day 5 they start to weed themselves out! I think the fact that they think your embies look strong enough to push to day  is a good sign!

  • I think either is fine.  Although you may fear not having anything to transfer if you wait until day 5, logic would almost dictate that that same embryo inside you wouldn't have implanted and produced a pregnancy. 

    I disagree a bit with the idea that by day 5 they can better pick a "good" embryo.  Yes, it gives them more time to see if any stop developing and any that are developing in a "bad" way (like, one of mine was shaped like a football instead of being round), but most studies have shown that embryo grading, for the most part, has very little to no bearing on pregnancy results.  In fact, most REs will admit that they have transferred tons of "perfect" embryos and not had a pregnancy result while transferring lower grade embryos in other women and they end up pregnant with twins.  The mystery of how or why or which embryo continues to develop is something that is still a mystery.

  • I had a 5d transfer with my first cycle that resulted in H. It is preferred in my clinic as well although with DH's sperm issues, 5 of 6 never made it to the blast stage and the four we didn't transfer arrested over night on day 5.

  • We did a freeze all for PGS and my RE only freezes day 5/6 blasts. I am pretty sure these days my RE only does or very, very strongly prefers 5 day transfers. Things definitely changed quite a bit from day 3 to 5 with my embryos. Good luck!!

    We transferred 2 on day 3 and I took home my sweet baby J. I think either way a genetically normal embryo is going to implant, provided the internal conditions are right on (lining, no fluid in the ute, right med combos, no immune issues, etc). 

    My clinic prefers 5dt, but when we only had 4 at day 3, they pulled the trigger. We were obviously pleased with the results ;)
