Trouble TTC

X-posted to IF: my WTF appt today (after IVF #1)

Basically, my RE's biggest concern was egg quality which he said should have been better for my age. (We had 22R, 20M, and 8F... 2 transferred and 6 didn't make it it to cryo). So he wants to do te same protocol but add menopur and also told me to start taking pregnitude.

I'm the one who brought up the issue of the tube being partially blocked with just a small opening for te dye to drain through and he agreed I should do an HSG again (my 4th). If it's fully blocked he would recommend removing the tube first. Otherwise we can move forward if it is still draining.

He actually recommended not doing ICSI this time if sperm looks good because the rate of fertilization with ICSI and regular were the same so he says we might as well save the money. (Although not doing ICSI still makes me slightly nervous!)

The weird thing though is that he had me doing a ton of blood panels that I have not done before. They are common for PCOS, but unsure why I have never done them before (at least the fasting ones). So next Wednesday I have the HSG and Thursday the fasting labs including 2 hour glucose test. I'm worried about the possibility of diabetes, as it runs in my family and I am overweight. And always tired. :/ (Do you know how this could potentially impact moving forward with another IVF?)

For the first time in the last four years of seeing him, the RE mentioned my weight and if I was doing anything like exercise, etc. he didn't seem to think it was significant enough to put more treatment on hold. I'm assuming it was brought up because of the fact that IVF #1 did not work and we are looking at reasons why. My BMI is at the high end of 31.

Assuming my HSG is okay still (not looking forward to it because it hurts more each time) I will start the long Lupron protocol again right after that. Oh and also he said he would plan on transferring 2 if good quality and 3 if not as good. That surprised me. We did 2 this last time because quality was eh. I am totally okay with this though because I highly doubt all would take.

Thanks for letting me get all that out! Any thoughts?

February 2013: second HSG, fibroids pushed on tubes which blocked them somewhat
March 2013: MRI to determine what type of surgery may be necessary

July 29, 2013: fibroids (5) removed via robotic laparascopy
August 2013 - Nov 2013 : benched due to recent surgery

IUI #1, Dec. 24, 2013, BFN 
IUI #2, Jan. 25, 2014, BFN
IUI #3, Feb. 25, 2014  BFN
IUI #4 canceled due to lack of response to letrozole
IUI #4.1 April 28, 2014, BFN

May 16, 2014: wtf consult, start prepping for IVF in June and add injects for one last IUI in the meantime
IUI #5 started letrozole and bravelle but canceled after HSG led to new diagnosis

May 21, 2014: third HSG, tubes blocked, one at the beginning, one hydrosalpinx??
June 11, 2014: consult, approved to move on to IVF because the hydro is not completely blocked therefore allowing fluid to move through slowly rather than backwards
IVF #1 August 8, 2014 - 3dt of 2 embryos, BFN
September 17, 2014 - 4th HSG, the right tube is very patent (open!!) dye went straight through this time. Weird!
October 2, 2014 - started metformin treatment
November 14, 2014 - blood work, brought A1C down from 5.8 to 5.5

November 26, 2014 - RE finally back from vaca and reviewed my chart, no more IVFs for rest of calendar year
December 1, 2014 - Right after Thanksgiving, I called a new clinic and got in right away! Plan for IVF
December 17, 2014 - ER! 29 retrieved (!!), 16 mature, all 16 fertilized (ICSI)

IVF #2 December 20, 2014 - 3dt of 3 embryos, BFN

We are done with treatment unsuccessfully. :(

    Rooting for you! No suggestions, just that it sounds like your doctor is doing everything possible for a better result this time. I see a ton of IVF #2 successes on PAIF so there's something to be said for refining the protocol and addressing any specific issues (and one more embryo can't hurt your chances!)

  • Well, gee that's lot you've likely got on your mind! Even though I wouldn't wish an HSG on my worst enemy, it is likely a good idea to do a recheck. How long has it been since your last? Regarding ICSI, I am a proponent of it, seeing as I would have like 0% chance of success without it due to DHs MFI. However, if your partners count and morphology is good, ICSI isn't really necessary. I would be cautious though. DH has really good count and motility, it's just that his morph is so bad that his post wash viable count is pretty bad. If you really want it, and do not mind the extra cost, ask for it. I have no insight regarding the PCOS testing or BMI concerns. But I hope your RE is willing to answer all your questions. Don't feel bad about calling them back as often as you need to bring up anything that comes to mind or concerns you. Making you an informed patient is also their job.

  • I'm sorry you have to go through the HSG again, but I hope it gives you some peace knowing that the possible hydrosalpinx will be checked on. :(

    Hope the blood tests come back normal, but again it's got to be good to know your RE is being thorough!!

    If ICSI had same fert rate as non-ICSI (is that called natural? no idea), it makes sense that your RE doesn't see a reason to repeat it. But, of course, good luck with whatever you choose!

  • I think your new plan sounds good! I think it's really great that your doctor is taking the time to explore a lot of other things to try to get a clearer picture of what's going on. I think that's really promising.

    I'll have my fingers crossed for you! Do you have any idea when you can cycle again? Or are you just waiting for all of the information before even thinking about it?

    @rainbowbridge14‌ I plan on taking a large amount of Advil... But it occurred to me today that maybe I should take a leftover Vicodin lol... I may run it by them. But they might tell me I would need a ride home. As for the testing, I'm not sure what all it is but the lady who does the blood work pulled it up and was surprised at everything there and said that was the PCOS labs. I am wondering if the RE didn't think it was serious before, like maybe I only had a couple of the symptoms and now he thinks there may be more to it? Not sure.

    @Buttmonkey34‌ I miss cycling with you too! But glad for you that you have been able to start your cycle already. :)

    @Sha259‌ Yeah I may bring up the ICSI issue again.. My DH has slight motility issues. But last time they did nine ICSI and eleven regular and if I remember correctly had four of each fertilize...

    @BunnyBerry‌ I'm not sure what that's called... Just regular IVF I guess, lol. Yeah I reallly don't want to do the HSG but it definitely brings peace of mind already knowing I'll have an answer one way or another.

    @lebradford‌ we are cycling as soon as possible... So if all is good next week I will start Lupron then and start stims 1-2 weeks later. If I need surgery... Then I don't know. Hopefully this year still!
