Just got back from new RE and I'm depressed...2 new observations...IVF in future but DH unsure

Well I just got back from a consult with a new RE in my area. I'm CD10 so I was expecting to see a dominant follicle. Well turns out the second his assistant (woman doctor in her residency) shoved the wand up my vagina he made a note of a very small fibroid and a small section of possible adenomyosis. I have NEVER in my life heard of adenomyosis, but apparently it cannot be accurately diagnosed without pathology and there is no hard evidence that it effects fertility so pretty much I just hope if I do have it, it doesn't get any worse. I also have NEVER had my old RE mention any fibroid issues. So pretty much I'm going to spend the whole day Dr. Googling adenomyosis and fibroids. So after these discoveries he goes on to say he sees two collapsed follicles, but doesn't know if I just ovulated or if these collapsed follicles were from my last cycle. I'm hoping to the SUN that I just ovulated because DH and I had sex last night and Sunday night, with the assumption I would ovulate CD12, which would be Thursday. So it is very possible this natural cycle is a bust already.

The new RE mentioned he wanted to get my AMH tested. Apparently it is a better test for ovarian reserve than the FSH and he suspects since my periods last on average 24-26 days I might have a low ovarian reserve! My FSH came back at 6 last October so I thought my egg quantity was in good shape, but maybe not. So with the observation of a fibroid, possible adenomyosis and no dominant follicle for this cycle and the "well since you have a shorter cycle at your age of 33 you may have a low ovarian reserve" I might as well start drinking and crying into a pillow now.

But I can't start drinking right now because it gets better. He explained that "unexplained" fertility usually means either the sperm can't penetrate the egg because the sperm runs out of energy or the egg has a tough outer shell and the sperm can't muscle through so if 3-6 cycles go by with no luck we will have to move on to IVF. That is fine, DH and I already talked about moving to IVF if I am not pregnant by July 2015, except when I called him and told him all this he was very hesitant to put $25,000 (estimate of course) towards having a kid if success wasn't guaranteed. I then screamed that being childless was not an option for me, so if it came to it we would have to pay for IVF or pay for adoption, both of which are expensive. So I'm SUPER pissed at him right now. He is a positive thinker and doesn't think I'm thinking positively enough about getting pregnant and that I'm jumping a head to the worst case scenario....HELLO I suffer from fucking SEVERE ANXIETY I like to assume the worse case scenario and make a plan of action. Planning for the worse soothes my anxiety.

AHHHHHH I'm so upset, annoyed and wished it was later in the day so I could start drinking

Ugh sorry you're having a bad day. My RE tested for both FSH and AMH. I have to double check but I'm pretty sure they tested both. Did you say your periods last 24-26 days? Or did you mean to say your cycle? Who says it's too early to start drinking

My cycle usually last 24-26 days. I think I would die if my period was that long lol. TYPO BRAIN!

I'm so sorry, that's too much stressful/uncertain news for one day!

I hope the fibroid and the adeno....thing turn out to have no impact on anything.

And if your cycles are 23-26 days, it makes sense that you may have Oed before CD10. It's not unheard of at all. I never heard that it was related to lower ovarian reserve, but I guess that makes sense if there aren't as many follicles producing estrogen and dampening the follicle stimulating hormone. Hopefully it's not an issue and you had a nice healthy ovulation!

MH was the same way when we first got our official diagnosis. He went so far as to blame my "negative" (read - not idiotically optimistic) attitude for harming our fertility. It didn't take long for him to start to respect my feelings as simply different from his, though. I was surprised, but he told me he still believe 100% that we will get KU every cycle we try, but he doesn't mind if I give up totally and we just wait for AF/beta together.

And thanks for that explanation of unexplained fertility - it's a nice clear picture of the tired sperm or walled-off egg and I think it will come in handy if I need to convince DH to move on to IVF.

Sorry for the unpleasant news. Hope it ends soon. Did they mention an action plan and how long until you're back on track?

The new RE wants to view my records from my old RE and do the AMH to review my ovarian reserve. Next cycle he wants to use letrozole instead of clomid. He stated there is a higher pregnancy rate using letrozole, though I haven't read this in any research, but I'm down for trying a new drug that won't make me emotional with bleeding gums. He was OK with TI and not an IUI. At this point my DH is leaving the house at 4:30 AM for his lovely 2 hour commute to Los Angeles every morning so logistical there is just no way to get a sample from him in time for an IUI. So he wants to do 3-6 cycles and if no luck he wants to explore IVF which is music to my DH's ears (sarcasm).

The RE had nothing to say about the fibroid since it was no where near anything so that is good, I just have to educate myself on fibroids because I know nothing about them. Yay for a night of Dr. Googling all my issues!

So sorry! It's NEVER to early to have a drink, or two, or three. I'm the same way...I always analyze everything and think the worst. My DH sounds like yours. Can be a huge pain in the ***. Ugh. Hugs!!

Sorry for the tough day! Before venturing into IVF, have you looked into any shared risk programs with a money back guarantee? The money aspect is super scary! A failed IVF may disqualify you, so if you think you might be interested, look into it now.

It is officially past 5 and now the drinking will begin.

For some weird reason I've really been into Nirvana lately (ever since watching their induction into the Rock & Roll Hall of Fame) so I started blasting their music while I chug back wine, try to play my electric guitar along with the music (it's been 17 yrs since I've played the guitar so I suck) and sing. What a great night I have ahead. Thank the SUN my DH is away on business so he doesn't have to hear me LOL.
