Trouble TTC

any reason to repeat HSG?

So I had an HSG in January of 2013 and everything was normal. I'm not dying to repeat the experience or anything... but now that it's been over a year and a half, I started to worry that something could have blocked up my tubes and I wouldn't know it.  Does that ever happen?  Is there any reason to repeat the HSG just to make sure? It's the only test my RE didn't make me repeat when I started working with him this year, so now I'm paranoid about it.

My husband was diagnosed with state IV colon cancer in 2009. Surgery and six rounds of chemo left him in remission (yay!) but with low testosterone. He took Androgel for two years starting in Jan 2011.
-SA in Jan 2013: big fat zero; stopped Androgel
-varicocelectomy in June 2013
-SA in Dec 2013: 23mil/ml, really low morph
-SA in March 2014: Count doubled! Morphology apparently no longer an issue.

April 2014: hysteroscopy
June 2014: TI (clomid/bravelle/ovidrel: 2 folliclces) = BFN
July 2014: IUI#1 (clomid/bravelle/ovidrel; 1 follicle, 23mil post-wash) = BFN
August 2014: IUI#2 (clomid/bravelle/ovidrel; 1 follicle, 43mil post-wash) = BFN
September 2014: IUI#3 (clomid/bravelle/ovidrel; 2 follicles, 15mil post-wash) = BFN
October 2014: IUI#4 (clomid/bravelle/ovidrel; 1-2 follicles, 12mil post-wash) = BFN

    I requested a second one, over a year after my first. The RE said it wasn't a bad idea. Plus we were getting closer towards ivf which we needed to have it redone for anyway.

  • I've done 3 and am thinking I'll have to do another soon. The second one was done because I took so long to go back after my first consult and the fibroids I had were way worse so the RE wanted to see what it would look like this time. The next one was a year after my surgery after I had four unsuccessful IUIs to see if scar tissue or adhesions had caused any blockage (which they did). He said if we waited a few months to proceed with IVF (which we did right away instead) that he would want to repeat it again. So there are reasons to repeat but I don't think it's always necessarily part of the plan.

    Is there anything in particular you think may have happened to block your tubes? It might not be a bad idea if you have reason to believe you should have been successful by now.

    2008: HSG (normal), couple rounds of clomid through gyno
    2008 - 2010: dragging my feet out of fear and procrastination
    October 2010: first consultation with RE, dx PCOS and fibroids (DH slightly low count/motility)
    Oct. 2010 - Dec. 2012:  In DENIAL! avoided the issue because I was scared of surgery
    January 2013: returned to RE, fibroids grew significantly
    February 2013: second HSG, fibroids pushed on tubes which blocked them somewhat
    March 2013: MRI to determine what type of surgery may be necessary

    July 29, 2013: fibroids (5) removed via robotic laparascopy
    August 2013 - Nov 2013 : benched due to recent surgery

    IUI #1, Dec. 24, 2013, BFN 
    IUI #2, Jan. 25, 2014, BFN
    IUI #3, Feb. 25, 2014  BFN
    IUI #4 canceled due to lack of response to letrozole
    IUI #4.1 April 28, 2014, BFN

    May 16, 2014: wtf consult, start prepping for IVF in June and add injects for one last IUI in the meantime
    IUI #5 started letrozole and bravelle but canceled after HSG led to new diagnosis

    May 21, 2014: third HSG, tubes blocked, one at the beginning, one hydrosalpinx??
    June 11, 2014: consult, approved to move on to IVF because the hydro is not completely blocked therefore allowing fluid to move through slowly rather than backwards
    IVF #1 August 8, 2014 - 3dt of 2 embryos, BFN
    September 17, 2014 - 4th HSG, the right tube is very patent (open!!) dye went straight through this time. Weird!
    October 2, 2014 - started metformin treatment
    November 14, 2014 - blood work, brought A1C down from 5.8 to 5.5

    November 26, 2014 - RE finally back from vaca and reviewed my chart, no more IVFs for rest of calendar year
    December 1, 2014 - Right after Thanksgiving, I called a new clinic and got in right away! Plan for IVF
    December 17, 2014 - ER! 29 retrieved (!!), 16 mature, all 16 fertilized (ICSI)

    IVF #2 December 20, 2014 - 3dt of 3 embryos, BFN

    We are done with treatment unsuccessfully. :(

  • @jencity9 - no, no reason to think I have a problem there other than I'm not pregnant yet.  And it's only been two failed treatment cycles so far, so hopefully that's not indicative of some larger issue.  I just got nervous about it since it's been well over a year, and I'd hate to be wasting these cycles if my tubes aren't clear.

    Thanks, everyone, for the advice!  I think I'll request it just so I can stop worrying about it.  And maybe it will help to "clear out the cobwebs!" I like that analogy.  Do you think I would have to skip a treatment cycle to get an HSG?  I know they do it before ovulation but I don't know if it affects follicle growth.

  • I'm meeting with my new RE next Tuesday and this question has been on my mind. My tubes are clear, but when the radiologist was injecting the dye, he told me to move over to my left side because of my tilted uterus. Now I'm thinking maybe my left ovary is twisted. I find it very odd that during my 3 cycles of clomid my left ovary has been a huge producer, producing 3 mature follicles during each cycle, while my right ovary only produces 1 mature ovary. I suspect my left ovary is twisted and therefore preventing the eggs from meeting the sperm. So back to your question I think it is a GREAT idea to redue an HSG after a year and also the CD3 blood work. Good luck!

    Tilted uterus
    3 day ultrasound 17 follicles
    HSG: 11/13/13- tubes open
    DH SA: SUPER sperm (145 million, 84% motility, 22% morphology)
    All infection disease and immunity blood tests NORMAL

    FIRST IUI May 2014: 100 mg Clomid days 5-9, third ultrasound CD 13 revealed four follicles 27, 24 and 20 and 13mm. Trigger shot May 28 with IUI May 29 and 30 (fingers crossed) - BFN started spotting 11DPO :(

    IUI #2 B2B 6/23 and 6/24, three mature eggs and 130 million sperm! FX this is the month!  BFN 7/8

    3rd cycle benched due to cysts - TI with OPK tests -  BFN

    3rd mediated cycle: Clomid+Trigger+TI  (three follies left side, one on right as usual....what the hell right ovary get it together and produce some damn follicles!) 7DPO progesterone level 43 with NO suppositories YAY for a natural strong ovulation. Beta canceled started spotting 13DPO - Third time is not a CHARM! 14 day cycle WTF! Everything looks normal - RE wants to start injectables next cycle so this cycle I'm benched

    Officially benched until March....but still plan on trying the baking soda douche due to my excessive and thick CM
