Trouble TTC

Back again, this time with test results

Hey Ladies! It's me again.
So after finding out that I have a "pelvic mass" and freaking out for about a day, I finally met with my doctor. Apparently it is not a fibroid like I originally thought (they run in my family). Turns out I have a 5.4cm x 3.0cm x 4.4cm cyst on my left ovary that, according to the test write-up, is probably an endometrioma. I have yet to Google what that is. That was the only abnormality that showed up on the ultrasound and all of my blood work came back perfect. My doctor also game me the results from the 'third day of period' blood work. She did not go into much detail and said that the OBGyn would know more about the levels and how they correspond to the phases of the cycle and if the cyst in effecting them. Anyway, the levels were:
Progesterone: 0.70 (Follicular Phase: 0.2-1.5 NG/ML)
Estradiol: 69.7 (Follicular Phase: 12.5-166 PG/ML)
FSH: 5.54 (Follicular Phase: 3.5-12.5 mIU/mL)
LH: 4.8 (Follicular Phase: 2.4-12.6 MIU/mL)

So, my next step is to get an MRI and go to the OBGyn.
Does anyone have any experience with endometriomas or ovarian cysts?

Re: Back again, this time with test results

  • An endometrioma is when some of the uterine lining cells, or endometrial cells, make their way into the pelvis through the tubes and land on the ovaries. They end up thickening each cycle just like the lining in the uterus. On an ovary, it can build up into a cyst just like any other follicle, but made of these endometrial cells. Like other forms of endometriosis, they contribute to an inflammatory environment in the pelvis which can be detrimental to fertilization and implantation, so if they are large enough (I don't remember the cutoff but I'm almost certain that yours are larger than it), they warrant removal through a laparoscopy. Also, their presence is the only form of endo that can be seen on imaging without a laparoscopy, and indicate that there may be more endo to be removed.

I'm not a doctor - so I could be totally off here - but this is what I've learned. Your RE may choose to leave it but my guess is that if it ends up being the only cause of IF they discover for you two, they may want to remove it.

Good news is that endo often causes IF, and treating it often fixes IF! :)

As far as the bloodwork - I'm guessing that the hormone panel could be a clue as to whether the growth is an endometrioma or something else, as other types of cysts may produce extra estrogen but not an endometrioma, if I remember correctly. BTW, your FSH is a great number - being on the lower side means you have plenty of ovarian (egg) reserve, since it doesn't take your body a ton of stimulating hormone to help your follicles develop!
