Pregnant after a Loss

progesterone plummeted... any experience? **UPDATE**

I'm on PIO. Progesterone dropped from 65 to 16 over the weekend. Feels like this pregnancy is doomed. Has anyone has this happen?

    I hope the PIO helps from further decreases.

  • Hugs to you. Progesterone can vary depending on the time of day I believe. Plus if you are on suppositories it won't necessarily show in your blood because it goes directly to the source. If you had a vanishing twin that could also explain some of it. Thinking positive thoughts for you.
  • These blood draws were pretty much at the same time of day, although the low one was the only one that I did not do the shot right before. (It's not the suppositories, it's the painful ass shots.) Nurse said that shouldn't matter. They were concerned and want me to increase my dose from 1 ml to 1.5 ml every day. I'm not sure if it's related to the possibly-a-vanishing-twin thing, either. My clinic says they want the progesterone over 30, but it's good to hear that other clinics are okay with lower levels.

    Ever just start to feel like something is not gonna happen? I have miscarriage traditions at this point (wine and a 2000 piece puzzle), while I just sob my eyes out. Feel like I should just go out and buy the damn puzzle now. Won't buy the wine bc I'd be too tempted.

  • ((HUGS)) I only know my progesterone levels from when I was with the RE. It varied between 13-27, and every draw was around the same time, before I even ate breakfast (I was told that can affect the number). The NP that always gave me my results would say they like to see it above 12. I'm not sure how far along you are, but at 7 and 8 weeks, my progesterone was 17. At my last RE draw, 9 weeks, it was 24.

  • Mine was 18.5 at 5 weeks and my dr said that was perfect. I keep reading about how other doctors want their patients over 20 and I kept getting anxious over it. Hopefully it goes back up. ((Hugs)) I know how nerve wracking this all is.

  • Thank you all. (((hugs))) Reading about other people's levels has helped me calm down. I have another blood draw in the morning. I also realized that no one has tested my TSH (mild hypothyroidism) which seems like a pretty serious oversight to me, so that will be tested too.

    Hoping it was just a fluke draw that caught your level on the downside. My p4 was pretty consistently hovering around 20 and they weren't worried (on both PIO and Endometrin suppositories) and then randomly it just shot up over 40. My e2 does wonky things like that too....my new "mantra" is if they're not worried I won't worry. If they're increasing the dose just see how that works before getting the puzzle out.

  • No advice but good luck! I know when I was miscarrying my progesterone was only .4 so you are way higher then that! And my doctor was very happy with my progesterone of 14.6 at 4 weeks.

  • My progesterone at 4 weeks was 19.5. It was 20 with my son, and fluctuated a bit, and he was a normal, healthy pregnancy. As long as it's above 10, I've heard that it's viable.

  • So the clinic just called, and I'm kind of flabbergasted. Here are the last few progesterone results:
    Friday, 8/15             65.4
    Monday, 8/17          16.5
    Wednesday, 8/20     82.5

    I can't help but think something is off there, but I guess I can stop freaking out now about that.

    New thing to freak out about: the nurse wasn't thrilled with my HCG levels. They went from 4659 to 7763 between Monday and Wednesday, which is a little below what the minimal good doubling time should be at this stage. I guess it's possible that I might have m/ced a twin, so the hcg wouldn't look quite right, but otherwise there may be problems.

    Also, TSH went up to 3.7, so that's not good either. Sigh. Can't freaking believe my clinic didn't test before this!!

    Anyone got insight on either of those?

  • I am glad you progesterone is looking better! And I know doubling slows down a lot once you get in the 1000's. As far as the TSH I would ask to be put on synthroid (synthetic thyroid hormone) immediately. I have hashimoto's disease which is an autoimmune thyroid disorder. My thyroid does crazy things especially while pregnant and postpartum and my thyroid levels are followed closely by my endocrinologist and my ob. I know my endo likes to keep my TSH below 2 or 2.5 while I am pregnant. Good luck! Prayers things continue to get better for you!

  • Once you hit a certain level you don't double every 48 hrs. It is every 72 hrs. Nurse should know this. Go onto betabase.com and compare and look at other ranges Hun. I think it will make you feel better:) progesterone fluctuates a lot I would say the one in the 60's and one in the 80's are accurate ...throw the other one out of your pgal mind!;)

  • Oh my god. You ladies are right. I was thinking that these tests were 72 hours apart, which would have made this a 97.75 doubling time, which would NOT have been good. BUT you're right--they were done 48 hours apart, which is a great doubling time. Gah. Nurse said they wanted it to increase by 67%, which would have meant that my HCG should have been 7780 or above, but that seems a pretty high bar! Good grief.

    So now I just have to panic about my TSH results. I should have said I'm already on 8 pills a week of 25 mg of Synthroid. Normally my levels are subclinically elevated (~1.8-2.4), but they do shoot up when pregnant, apparently. Call is in to my endocrinologist for instructions.

    PgAL is hard enough without all this garbage!!!!!
