I posted a few days ago about how anxious I've been about my upcoming surgery. I met with the OB-gyn/surgeon on Friday and she thinks she probably won't be able to complete my fibroid surgery laparoscopically. She's willing to try to complete it laparoscopically but thinks she'll probably need to convert it to a laparotomy. And she thinks it would probably have to be a vertical incision laparotomy (from my navel down to the pubic bone). If there was no other alternative, I would accept that it was just what I had to do to get my BFP eventually. But I wasn't ready to accept that.
There's a Dr that I used to work with at my previous job ....I've always known her to be pretty well connected. I shot her an email and expressed my anxiety over my upcoming surgery and what my current Dr has planned for me. She suggested I contact a friend of hers, a Dr in Oakland who routinely does laparoscopic myomectomy on large fibroids. Oakland (a city 30 miles away from me) is not within my service area but I could still see her with a referral. I contacted this Dr by email. Got an auto email response - "I will be out of the office until Jan 2nd 2015. Please contact my colleague _____ who will be taking over all my patients." F*ck! I felt pretty hopeless and was on the verge of a panic attack thinking of my (sure to be) laparotomy on 8/29. Anyways, the Oakland Dr responded to me and told me she was on Maternity leave but to call her on her cell phone. After work, I called her. She was so sweet. I said "Thanks for being willing to talk to me about this when you're on maternity leave, Dr. Z!" She said "Please, call me Ella." She had been going over my electronic chart. After asking me a bunch of questions, she said that she felt fairly confident that she could complete my surgery by laparoscopic or robotic means. Say what?! I was in shock. BUT ...she's on maternity leave. My heart sank. She offered an alternative. Her colleague, Dr. Y has trained with her for years and also does myomectomy on very large fibroids. "She's great. Honestly, she's better than me," Dr Z humbly said. She sent a text to Dr Y with all my info. Told me to text her if I didn't hear from Dr Y by the next afternoon. Dr Z confided in me that it was awkward to be "stealing" a patient case away from a colleague that she respected (my Dr who is scheduled to do the myomectomy). But, she said "Not every Dr is comfortable removing a 15cm fibroid laparoscopically. I've done quite a few of them and so has Dr Y. I am fairly sure she can do it."
Within an HOUR, I got an email from Dr Y. She said "I was referred your case by Dr Z. I am fellowship trained in advanced minimally invasive gynecologic surgery as Dr Z is, and my focus areas are fertility sparing myomectomy as well as endometriosis. I also agree that your surgery is very feasible to complete either laparoscopic or robotic." She went on to say that she would like to prescribe me a medication called Lupron to shrink the fibroids and ideally do the myomectomy in 2 months (which she said would lessen the total surgery time as well as improve my recovery).
You guys ....I think I want to jump ship and go with Dr Y!! My current Dr has been great in terms of responding to my emails promptly and showing that she really cares about me having a good outcome. But she made it very clear she probably can't complete my surgery laparoscopically. Dr Y. seems pretty sure she CAN. This would mean delaying my surgery by 2 months. And taking Lupron which is supposed to cause horrible menopause like symptoms. But it seems worth it to avoid laparotomy! This will be a bit complicated/messy because I'll have to cancel my application for medical leave. Ask my boss to put me back on the schedule for work (I hope she won't be difficult about it). And I'll have to let my current Dr scheduled to do my surgery know that I've decided to go with a different Dr I got a 2nd opinion from.
I am so happy. I know it's not a done deal as I haven't cancelled my surgery and rescheduled with Dr Y just yet. But I feel really good about it. It looks like I have a good chance of sparing myself a laparotomy here. Earlier I was literally jumping up and down in the kitchen and squealing. And thinking that I was probably just as happy as I would be someday if I got a BFP (Is that crazy?).

June 2014 - Clomid 150mg CD4-8. HCG shot CD14. IUI#2 done on 6/9 and 6/10 (CD15-16) w/ Prometrium suppositories during TWW .... BFN
July 2014 - Clomid 150mg CD3-7. Ultrasound CD12 - 4 mature follies! HCG trigger shot CD12 AM. IUI#3 on CD13 w/ Prometrium suppositories during TWW ....BFN
July 2014 - Repeat ultrasound done to measure size of fibroid - 8x9cm (the size of a grapefruit!) Surgery referral made.
August 2014 - MRI done which revealed 2 fibroids: a 13x15cm fibroid as well as an 8x7cm one. Super freak out mode over surgery plan. OB-GYN/surgeon said surgery would most likely have to be a laparotomy (open abdominal surgery)
August 2014 - After a 2nd opinion, plan to take Lupron x 2 months to shrink fibroids then laparoscopic/robotic myomectomy. Surgery date is set for Nov 21st!
November 2014 - Robotic myomectomy done (2 large fibroids removed, mild endo found). Benched x 3 months

As far as your current doc, she'll probably understand why you'd switch docs to someone who is comfortable doing it laparoscopically. This is about your body; it's nothing personal.

Don't feel bad for seeking out another Dr. Not all Dr are the best at every case, you have to do what is best for your particular case. I saw 8 ENT's for my Ménière's and it wasn't until I saw my 8th one did he come up with a plan that would actually help. You have to be your own advocate for you health.

July 2014 - 3rd round of Femara. CD10 Follicular U/S. No follies in left ovary. 7 follies in right. 1 20mm, 1 12mm, 1 10mm, 4 <5mm. Scheduled for IUI. Canceled due to low estrogen level of 145. TI this month. Prepping for Injectables next month. BFN.
August 2014 - CD3 BW Normal. Injectables not happening because of stupid miscommunication about "required injectables class." Taking class this month. No medication. TI for the month. SIS scheduled for 8/7/14. SIS results - "I have a beautiful uterus." Huzzah!
September 2014 - Cycle cancelled due to stupid AF coming early and making my IUI run in to DH's business trip. Try again next month.
October 2014 - Injects with IUI cycle. 75U 5 days. Estrogen at 36. Bumped up to 125 for 4 days. Estrogen 105. Bumped up to 225. Ganirelix for 2 days. 4 mature follies. Triggered 10/9. IUI 10/10.
BFP 10/24/2014. Beta #1 - 178 Beta #2 - 398. U/S 11/7/14- TWINS!!!!
I have been feeling extremely anxious about the high probability of laparotomy with the surgery scheduled for 8/29. To cover all my bases, I contacted Dr Y at San Leandro for a 2nd opinion. She has some experience in dealing with extremely large fibroids like mine. Upon reviewing my MRI results, she feels confident that she can remove my fibroids by laparoscopic or robotic means. Her plan for me would involve 2 months treatment with Lupron to shrink the fibroids, then scheduling the surgery for late Oct/Nov. I've decided to cancel my surgery on 8/29 and reschedule with Dr Y. I will shred the triplicate you gave me for Percocet as I won't be needing it for now.
I really appreciate you responding so promptly to all of my questions and keeping me well informed every step of the way. I'm especially grateful that you were so honest with me about how you saw the surgery going on 8/29. I know my case is quite challenging. Thank you so much for everything!"
