Pregnant after a Loss

Others with MTHFR here?

If so, which mutation do you have and is your RE/OB have you on anything special? Ie, which prenatal are you on, do you take aspirin or something else, and are you on lovenox?

I'm compound hetero (one of 677C and 1298C) and my OB has me on baby aspirin, a natural pre-natal and 2mg extra folate. (L5-mthf), plus I'm about to start progesterone gel.  My OB has offered heparin injections, although she said it's just as a precaution, if I want it. I still need to talk to her about that next week. I'm only 4.5 weeks.

I just wasn't sure if I'm going overboard for compound hetero, since I know I still have one healthy gene... or if a lovenox injection is standard protocol for MTHFR? I've had two healthy pregnancies with nothing but regular prenatals, but I've also had two losses. So far my betas look good, I'm going in for my 3rd betas tomorrow.

  • I'm not much help. I'm hetero A1298C. My RE advised me to take an 81mg aspirin for the rest of my life.

    On my own (RE approved) I started taking an additional 1mg of folic acid + what was in my prenatal. I also started taking a super b complex. For a while I was doing additional b6 & b12, but I've stopped those.

  • **lurking**  I had my loss back in April and I just started TTC this month. After my loss I did the testing and was diagnosed with compound homozygous MTHFR and my RE has me on baby aspirin, folgard, and Vitamin Code Raw Prenatal Vitamins. We will test progesterone when we have conceived. I work for a neurologist who has advised me to insist on going on lovenox. In my case when we had our ultrasound that we learned of the loss there were various clots. I am absolutely planning on requesting to do lovenox!

  • I am also compound hetero with one of the A and one of the C. I take Thorne Basic Prenatal that has folate instead of folic (I take a separate DHA because it doesn't have it in it), Thorne Basic B Complex with folate in it too, a baby aspirin until 37 weeks and 1600 mcg's extra of folate. I am not taking lovenox and any other blood thinner except the aspirin. I am also on extra vitamin d but I don't think that's anything to do with MTHFR. I am getting an extra ultrasound next week at 29 weeks to check on growth and clotting bc of MTHFR and my loss history. Congrats to you.

    There is a helpful site called MTHFR.net I believe that is filled with lots of good info.

    @jess123456 - glad to know you are using the Thorne prenatal. That's the brand I'm using, plus the Thorne folate too. I'll look into their B complex.

    Thorne is a great quality prenatal based on the research I did for one with folate. It's not the cheapest in price for sure. Three big pills a day are a bit gaggy but luckily the B Complex only requires one a day. I use the Nordic Naturals Prenatal DHA because Thorne's doesn't have DHA.
  • I'm compound hetero and neither my RE or MFM feel that it causes an issue. I had a heathy pregnancy and my loss was not due to this. The MFM is having my homocysteine levels checked every trimester. She feels there is a link to clotting when these levels are high.
  • I'm hetero A1298C and I take baby aspirin and 4 mg of folic acid.

    @denamarie: if you're compound homozygous you should DEFINITELY insist on blood thinners.  Compound homozygous is super rare.

    Some recent research suggests that MTHFR mutations aren't really associated with losses, but I would (and do) err on the side of caution.  In any case, the mutations definitely reduce the body's ability to break down folic acid...by a lot.  
  • I only have one copy of the A mutation. That being said my RE decided that he would treat me with everything possible in hopes to get this pregnancy to be successful, he put me on progesterone suppositories, baby aspirin, L-methylfolate, and also Heparin. He told me the Heparin was probably overkill but I didn't care. I took the progesterone until 10 weeks and Heparin until 12. I am still on baby aspirin and will be for life and I'm still taking the broken down folate my body needs. I'm not sure if the Heparin did work but I am currently 37w3d, and never thought I would make it to this point.

     From my understanding every doctor is different in their approach to treat MTHFR. Some don't even consider it needing to be treated. The main thing is what you are comfortable with, if you don't like your current plan then you could always get a second opinion.

  • I have two copies of the C mutation, my homeocystine levels were normal, so I don't think my RE feels my loss history is due to MTHFR but since he didn't find anything else suspicious, decided to be pro-active. My RE had me on daily low dose aspirin snd extra folic acid before I got pregnant. Once we confirmed intrauterine pregnancy with heartbeat, I was referred to a hematologist, who started me on methyl-CpG which contains folate, lovenox, iron and calcium. I believe the plan is to be on lovenox and aspirin until 30 weeks, not sure what happens after that.
