Student doctors

Normally I am very open to student doctors but yesterday I had an appointment with the 1 OB in my practice that I really can't stand. And insteadnof him it was one of his residents so I let her do my check to avoid him but of course he came in at the end and was his normal asshole self. Back to the resident. She was so rough when she checked.my cervix I thought she was bruising me. Well since my appointment yesterday where she said I was fine no effacing no dilation. I have had untlerable pain in my pelvic region. It feels like babies head is buried down on my cervix. I keep getting sharp pains in my lower abdominal region and pelvic region. If I move around it hurts worse if I sit its more consistent. I keep going to the bathroom thinking something has to relieve it. But nothing. It started slowly after that appointment and has just got more consistent and more painful. :( needless to say I'm in pain and I can't take it much longer.

  • @jalara48‌ Oh yeah. I told her that it shouldn't cause that much pain when she said I could feel "a little discomfort" I know the feeling of the small amount of discomfort as my docs check you cervix every visit from 12 weeks on. I couldn't I was wincing and definitely expressed pain. There is no way I am seeing her again.
    I am not a fan of being a learning tool but I am in the medical field so I understand everyone has to learn on someone. But I'm not going to bebher learning tool anymore.

  • Every visit. That seems strange in itself. I had the pap done first visit and they don't check again unless you have a reason like early labor and will check again at 38 weeks .
    See I have always been going to this practice so I didn't know anything differently, but yet it is undress from the waist down and checked at every visit and a PAP at the first visit. I didn't realize this was odd until someone posted the cervical check thread.

    @Jalara48 It is just very consistant. I think the worst part is I am at work so i feel trapped in the pain. I can't take anything and I can't go to bed. If we weren't short staffed tonight I would have asked someone to cover my hall (Assisted Living) which is super easy on midnights and I would run over to L&D (literally across the street) and see if they could get the stabbing pains to subside at the very least.

  • Yikes. Perhaps she had trouble finding it? I had a couple residents check me out with my last pregnancy and they always had problems actually finding my cervix because apparently its very prosterior/kind of in a weird position. My doctor sometimes had/has trouble too.

    But yeah, if you're in that much pain I would definitely go get checked out, make sure nothing was disturbed. Even though thats probably the last thing you want to do.

    Isn't that part of the CN's duties? To cover in an emergency?

    Isn't that the truth. I don't want anything checking out that region right now, but at the same time the pain is ridiculous. @PinkShades05 I agree I am really starting to wonder what their logic is behind always having to check. I never asked because I always thought it was normal and it was never painful so I didn't think too much of it, but after the incident with the resident I am really beginning to question why they have to do that all the time.

    Unless you are a high risk patient or have some need for it, it's a very unnecessary procedure. In fact, it tells very little about when you will go into labor. If I were you, I would ask your OB about this at your next appointment and see if you can opt out until you are at least 36+ weeks.

    Oh hehe- makes sense. Yes, but we don't have a charge nurse or director of Nursing at night. We only have just enough to make census on the the weekends right now unfortunately.

As graphic as this sounds: I don't understand how someones fingers can cause more pain than a penis and cause so much discomfort after an exam versus after intercourse.

  • Cervix checks are no walk in the park. I'm sorry you had an especially rough one, and I hope you find some relief. Definitely let the OB office know that resident caused you so much continuing pain!

  • I'm sorry. You were already given good advice. I had cervical checks at the end of my last pregnancy but will be opting out this time. They are not necessary and my doctors always indicated it was up to me to decide.

  • I'm at an academic hospital so in addition to my MFM, there's usually a team of MFMs and fellows who come into the room.  I don't mind as I chose this hospital because my MFM is one of the top in the country, and I don't mind the fellows as long as I get to speak to my MFM or one of his colleagues. 

    I had a cervical check performed in L&D a few weeks ago because a contraction showed on the NST.  The check ended up being performed by a MFM fellow, with 2 other MFMs in the room.  I ended up screaming hysterically, and it took forever, and apparently I screamed, "why are you taking so long!?!?", while squeezing a MFM's hand (yeah, I'm going to be a peach during labor).  Apparently I could have made it worse because I was moving away from her :-S (wonder why).  I felt fine afterwards and ended up just trying to make her feel better about my freakout.  The fact that she has small hands and I kept moving just made it worse for me.

    If you are concerned I would definitely all L&D and report your symptoms.

  • That sounds terrible. I hope you get some relief from the pain soon and that nothing else is going on.
  • Update: since I worked until 7:00am this morning I just woke up. I am still having pain but not quite as severe after I have been awake for a couple hours I will update again with what I decided to do.

  • Update: I feel like I have been sleeping all day. When I called my doc on call he didn't seem concerned so I have just been trying to sleep off the pain.

  • My checks are the same as @Justabean3‌. They probably won't even check me this pregnancy since I'm having a rcs unless I have contractions. Why was the Dr not in there with her too? I've worked at a couple different medical offices and the students were never seeing patients alone! I would call the Dr on call and let them know how uncomfortable you are.
  • I woke up about 6 times during the last hour while I was napping with strong pains to the vagina and cervix. I think I going to call again. I don't think this is normal to be going on still since my appointment was at 3:15pm on Thursday.

  • Yikes. I would definitely call again! 

    I'm the same as most, no checks until 36 weeks, although I hear of a lot of people who opt out. I never experienced pain with my checks last time so I can't complain!

    I also am giving birth at a university's med school hospital - they never have students at the OB office but I imagine there will be students present at this birth, there were 3-4 observing last time.
