MFI - Your DH abstains for how long pre-IUI collection?

We'll hopefully do an IUI next week (depending on my u/s results)--but I thought I'd get my question out early in case anyone has some advice.

MFI ladies: we're wondering how long your RE office tells your husbands to abstain in preparation for the IUI collection.

I've heard that the ideal time may be different for guys with MFI (especially MFI of the trifecta variety: low count-low motility-and-low morph)--and possibly shorter (i.e., one day) because quality often degrades the longer you wait. But, I know the counts increase the longer you wait. For the IUI, the samples are washed--so maybe the higher count is overall better because the low quality sperm will be taken out anyway? It seems stupid to worry about a day, but unfortunately a day can make a difference with numbers like ours. Anyone thought this through?

News article from 2005 about the study that's giving me pause ---

Sperm quality increases when men go without sex ... For a day
Reuters23 June 2005National Post

For men with low sperm counts, sexual abstinence -- but only for a day -- increases semen quality. More prolonged sexual abstinence may actually reduce sperm numbers, a new study indicates. "After only two days of abstinence, sperm from patients with male factor infertility initiate a process of quality degradation," Dr. Eliahu Levitas of Soroka University Medical Center in Beer-Sheva, Israel, and colleagues report in the journal Fertility and Sterility. The findings are important for men trying to father children through in-vitro fertilization, or even through the natural method. The researchers conducted the current study to determine the effect of abstinence on sperm quality. Among the samples classified as being low sperm counts, peak sperm concentration and peak sperm motility occurred after one day of abstinence. The percentage of normal sperm also peaked at one to two days of abstinence for low-count men.Samples with normal sperm counts showed a slight decline in sperm concentration during two days of abstinence, followed by a gradual increase to a peak on days six and seven.

  • Thanks for all the responses! My office had said 1-3 days before the IUI, and it sounds like that's the range I'm seeing from you guys, too. Don't we wish things were more scientific? I guess they just aren't sure, especially with the balancing of the count and the morph!

Good luck to all you ladies with whatever you are pursuing now! :)

  • My RE told me to abstain for 2 days, definitely not more than 3. The exact words she used was that they want a "fresh" sample. My last IUI she told me to have sex on Friday night, then abstain on Sat. And Sunday, the IUI was Monday morning! Good luck!!!

The first IUI was 4 days.  We did that just because of timing and the nurse never specified.  This IUI we had sex the day of the trigger, so 2 days of abstinence.  My RE said they usually recommend having sex the day of the trigger.

With 4 days, he had 6mil total and 400,000 post-wash.  With 2 days, he had 21mil and <100,000 post-wash so I have no idea what's going on there.

They also said BD after the IUI but with our post-wash numbers this time, neither of us was in the mood.  Good luck with everything!

Edited for: I forgot a bunch of info because I'm tired.

  • From everything I've read and our own experience with repeat SAs, there's no simple rule about waiting and its effect on sperm counts/quality. I think a rule of thumb is that a few days' wait helps counts, but doing it soon after increases % motile. But I think every guy has their own threshold as to how many days helps it build up, and then when it starts losing quality.

For us, TI is pretty much not going to get us pregnant, so we just do it every 3 days to keep him fresh and healthy, then EOD after the first follicle check until IUI, so we have either a 1 or a 2 day wait (since we don't trigger, we can't time it perfectly). The one time we tried an IUI (changed to ICI, that's another story), we got lucky and it was 2 days after sex so I think that helped counts. We just followed the nurse's schedule.

One think I learned from temping and charting for 5 years - I had to stop sweating the days and even hours we had sex. As long as it's regular, and you don't do it constantly before the IUI, a day more or less of abstinence isn't going to ruin everything, I think. But that's just my opinion. :)
