D&C "Complication"

Hi Ladies, 

I'm wondering if I can get some opinions from you all because I'm not sure if I am over reacting or not.(Sorry in advance, this may be a little long) I had a D&C on Friday after finding out that our baby did not have a heartbeat at 9 weeks. I did not have much bleeding over the weekend and by Sunday I started to feel "not right" (fever/chills etc). So I called my Dr. on Monday morning and they said just to be safe, to come in. So I went in yesterday and the Doctor said she was not surprised to see me. She said that she didn't get nearly as much tissue during the procedure that she expected for doing the procedure on a 9 week fetus, but the surgery room that the procedure room was in did not have an ultrasound machine, so she couldn't check. She did an ultrasound yesterday and she was not surprised to see that there was quite a bit of tissue left in me. She suggested that I take Misoprostol (4 doses every 6 hours starting at 5:00 PM yesterday) I took 2 doses last night and based on my conversation with her, I was expecting to wake up to a lot of blood. When I woke up this morning, there was NOTHING at all!! There was some confusion on how I should take the pill (the pharmacist said that the pills can be taken orally or vaginally, but the dr didn't specify. Of course the office was closed by this time and I couldn't get confirmation) so I just took them orally. When I woke up this morning to no blood, I called the answering service to find out if I should take the 3rd dose vaginally. That was at 6:30 this morning. It is now 9:20 and I JUST got a call back from them that they want to do ANOTHER D&C tomorrow morning! Am I overreacting because I think that there is NO REASON that there should be tissue left!! Why wouldnt she have ordered for an US machine if she KNEW she didnt get as much tissue as she thought she should get. Not to mention, that my insurance is an HDHP to where I have a $2,000 deductible, then it is an 80/20 after that up to a $4500 out of pocket max. So basically I am looking at hitting my $4500 out of pocket max, just from the first surgery alone and she wants to do the procedure AGAIN. Does anyone have any thoughts on this? I dont want to do the procedure again but I dont see any other options.

  • Oh my god! I am so sorry to hear about this. I absolutely agree that she should have ordered an u/s machine. That's just crazy. I would be very angry, as you are.

    I cannot offer any advice but I wanted to give hugs!! I hope you can figure something out that won't break the bank. Isn't it shitty how a lot of stuff just comes back to money. Ugh. Life. (((Hugs)))

  • I'm so sorry about the need for a repeat D&C since they didn't get all the tissue the first time and that the miso didn't work. I've never used that drug so I'm no help there. I will say though that as much as the money issue sucks you have to get that tissue out. There are few ladies on this board who will tell you about all the not fun things that happen if they don't. Can you ask to make sure they use a ultrasound machine tomorrow?

  • I am so sorry you are going through this. Unfortunately I do not have any advice, but just want to send lots of hugs your way. I would explain your frustration with the situation to the doctor. Hopefully she is sympathetic and can explain her actions.  Good luck hun!

  • I'm so sorry you're having to go through this. I would demand a patient advocate to argue the repeat surgery should be on them since the doctor didn't order the appropriate equipment.

    It was her job to complete your D&C the first time, and when she knowingly didn't do that she should have informed you immediately. Yes, sometimes a small amount of tissue is left behind, there are lots of those stories here, but she knowingly half assed your surgery and didn't tell you until you had symptoms.

    Call the hospital your surgery was at and ask for the patient advocate, explain the situation and ask for advice. ((Hugs)) this is ridiculous, and I'm so sorry.
  • Holy shit I would be so pissed!  I don't have any experience with the meds you were given but I did have some retained tissue after the D&C following my first loss.  I passed it naturally.  It sucked, but my body dealt with it.  I know sometimes that isn't the case though and bad things can happen with retained tissue.

  • I would be pissed too!  I agree with @mlal78 that another round of miso, taken vaginally, might be a good idea before jumping into another D&C.  Also, a complaint to the hospital about the doctor is certainly in order here.  If another D&C is necessary, you should not have to pay for it!  Good luck and I hope this can be cleared up soon!

  • I agree with a few of the PP that you should talk to a patient advocate and put in a formal complaint as well. Unfortunately, other than that I have no other advice as I had a different experience. So very sorry you're going through this. Lots of **hugs** and sending you positive energy!

  • BIG HUGS!! Definitely call your insurance as I have read many blogs where insurance has tried/faught to deny the second D&C and it has caused massive lengthy messes. Give them the heads up and talk to them about your deductible, etc. They should be able to lay everything out for you on how the fee's worked for your first D&C and what will happen if you have a second one. I know of at least a couple people who chose to do cytotec and for one person I know everything passed before she had the second D&C (she chose to give it three weeks before scheduling number two) and another had the second D&C ten days after the first but she said she had already met her deductible so her fee's for D&C number two weren't bad.

  • I'm so sorry you are going through this.  When I took miso, it took awhile to work, much longer than my dr originally told me.  I did start bleeding within 12 hours but it took 3 days to pass the majority of the tissue.  Additional tissue passed 2 more days later.  The whole experience was realyl traumatic and I would definitely not choose miso again.

  • Oh dear! I'm do sorry to hear you have to go through all this. I'd be just as furious that she half asses you D&C from the get go let alone not telling you she couldn't get it all. I myself retained some tissue which can be normal. Caused chills, extreme discomfort, and mass amounts of clotting, but most passed within twelve hours and the bleeding has slowed down immensely. I really hope all goes well with your second D&C. It's unfortunate that you have to go through this. Lots of hugs and T&Ps

  • I am so sorry to hear that you are going through this. I would definitely question why an US wasn't ordered if the dr didn't think she got everything (?!?!) I hope they make extra sure that they get everything the 2nd time around. Big hugs for you and best of luck with the procedure.

  • I'm so sorry for your loss and for the complications you're going through. 

    Before my D&C, I had to take two doses of miso... totally took them orally because no one told me (nor was it on the script) that they were to be taken vaginally.  When my dr asked me how much bleeding I had before surgery and I said none, it was the first thing she asked- "Did you take them vaginally?"  If it were me, I would ask to try a second round taking them correctly before jumping the gun on another surgery, as your doctor seems so quick to do. 

    I also have some retained tissue... not sure if it's as much as yours (probably not), nor did I have any signs of infection.  First of all, shame on the doctor for not giving you antibiotics knowing that there was likely tissue remaining.  It's the first thing my RE did when he realized I had tissue remaining after a hysteroscopy.  He's letting me get a period before considering another D&C to see if I'll pass it on my own.  If you do have to get a D&C for your safety (an infection could be serious), I would insist on having one done with a scope/in combination with a hysteroscopy.

  • Holy crap. I cannot believe that this happened to you, and that your dr was so nonchalant about it!! 
    #1 - I have no clue why she wouldn't just have you take the miso all vaginally this time (since she neglected to do her job and tell you what to do with it), since that usually does work.  I have to take one dose the night before my D&C and I had a good amount of bleeding. 

    #2 - You need to get a patient advocate. Someone needs to know about all of this.

    #3 - Are you considering switching doctors? Because if you weren't before…

    #4 - Lots of ((hugs)) to you. So sorry that you have to go through this, again!!
