Trouble TTC

Hello- Intro

Hi everyone… I'm not sure if this is the right place for me. I've been reading the infertility board and made an intro there (now that I have figured out how to post) but it seems to be more of an IVF only board… So I thought maybe I would try over here. I hope I'm in the right place ;)

My husband and I have been trying to conceive for over a year now. I went off birth control and nothing happened. My OBGYN put me on Provera and I got a withdrawal bleed after that. She thought magically I would start getting my period again after the bleed - needless to say, that didn't happen. So after six months of no period, I got sent to an RE. My RE put me through the ringer for testing. At my routine HSG he found a septate uterus and strongly suspected endometriosis. I had a laprascopy/hysterscopy in June and they found and ablated level 3 endometriosis as well as removing the septum. Now I'm ending another rousing round of Premarin followed by Provera (to build up my uterine lining after surgery). My doc wants me to re-do an HSG in September and then he'll begin to address the fact that I do not ovulate. I'm grateful for any stories or advice that can help  Has anyone had a history of endo and/or septum along with ovulation issues? I feel like I've achieved a very non-desired trifecta here.

Re: Hello- Intro

  • No advice about endo but just wanted to say welcome!  Sorry you have to be here.  Good luck moving forward and hope you can come up with a good plan with your RE.  From what I've hard a septate uterus can be very easily corrected.  Hope that is the case for you!

Me - Left salpingo-oophorectomy at 19 due to large cyst/torsion  and 2 large uterine fibroids found at age 30
Him - Borderline low sperm count (correcting through Fertility Blend supplement)

    TTC #1 since Jan 2013
    First appointment with RE 10/2013.
    April 2014 - Clomid 100mg CD3-7.  HCG shot CD10 IUI #1 done on 4/12/14 (CD12) ....BFN
May 2014 - Break cycle to repeat saline sonogram and re-group.  Travel to Kauai 5/7-5/12 (Yay!!)
    June 2014 - Clomid 150mg CD4-8.  HCG shot CD14.  IUI#2 done on 6/9 and 6/10 (CD15-16) w/ Prometrium suppositories during TWW .... BFN
    July 2014 - Clomid 150mg CD3-7.  Ultrasound CD12 - 4 mature follies! HCG trigger shot CD12 AM.  IUI#3 on CD13 w/ Prometrium suppositories during TWW ....BFN
July 2014 - Repeat ultrasound done to measure size of fibroid - 8x9cm (the size of a grapefruit!) Surgery referral made.
August 2014 - MRI done which revealed 2 fibroids: a 13x15cm fibroid as well as an 8x7cm one.  Super freak out mode over surgery plan.  OB-GYN/surgeon said surgery would most likely have to be a laparotomy (open abdominal surgery)
August 2014 - After a 2nd opinion, plan to take Lupron x 2 months to shrink fibroids then laparoscopic/robotic myomectomy.  Surgery date is set for Nov 21st!
November 2014 - Robotic myomectomy done (2 large fibroids removed, mild endo found).  Benched x 3 months

    PAIF/SAIF welcome

  • @iPutKetchupOnKetchup‌ - is that the same as a bicornuate uterus? My re thought I had that until he went in and realized it was a septum.

I am very frustrated at waiting to try clomid (or something of the like). He said he needs to do the HSG next month to be sure my tubes are clear since the septum blocked that in her last HSG. Also he says I need time to heal internally before said HSG. He's a strange guy but he said he's not worried about clomid at all. His exact words were "I can make you ovulate, that's a home run". His mouth to Gods ears! And I happen to have quite a potty mouth myself so I welcome it from everyone else

  • Welcome, sorry you find yourself to be here. I think this is a perfect place for you, you will find many women with anatomical and ovulation problems, unfortunately. :(

I'm glad they were able to ablate the endo and treat the septum. I agree with Ketchup, I would be a little impatient but perhaps your RE is hoping to create excellent conditions for you when you do O? I would personally want to know the chances and timing for the endo returning since that creates such havoc with fertilization and implantation, and then put things on more of a rush if there is a risk there.

Have you had blood work to give an idea of what's stopping your ovulation? Some causes could possibly require months of treatment to rectify, such as hyperprolactinemia, thyroid or insulin issues. I would want to know what the issue was at least before putting it on a back burner?

You asked about women with a similar "trifecta" of fun...I just had endo found/treated this week with a laparoscopy, and although I O I also have PCO so there's weirdness there. No uterine abnormalities, but I did have a cervix that was almost closed, which also was just fixed this week (yay!). Basically, although I have been shocked by the number of issues uncovered (DH also has MFI), it also gives me great relief to know that there are really strong reasons that kept us from ever getting pregnant in 5+ years, and so now that each thing is being dealt with, it gives me lots of hope for a BFP. Hopefully you will have a similar experience of hope as everything is identified and treated. :)

  • Wow great illustration! I didn't know two separate uteruses were possible. I had a very large partial septum (not quite full). I think the septum blocked the dye from getting to the tubes?? I could be wrong though. I don't have another appt until Sept unfortunately

  • Welcome. I'm new here too. I don't share the same situation (our's is MFI), but I do have a friend who had the same condition. Her septum was removed successfully with surgery. Also, sounds like we may have the same RE! He said, "I bet my lunch money your test results will be fine!" :) Best of luck to you! Hope you have a short stay here.

  • Hi and welcome! I'm sorry you have to be here, and needed surgery, but I'm glad you're on the mend now! I have also been anovulatory, and I know that can be frustrating since it feels like there's not even a small chance of conceiving.

I also would ask why you have to wait so long for the next HSG and starting ovulation induction? What did your H's SA look like? Just curious if they could get your next cycle scheduled soon! How early in Sept. is your next appointment? I would maybe call and ask if they could move things, especially if you are worried about the endo. In my experience it helps to tell the RE office what I want to do and not really ask (but I still do it in a nice way). Like I called and said, "I'm taking prometrium to start a period and I was reading that the HSG could be done with a medicated cycle, I'd like to try that if that's okay!" They let me do it! I didn't want to wait another month either.

Anyway, welcome again! I hope everything works out soon for you!
