Question about labs and monitoring appt

Just curious - during IUI cycles, do you routinely get labs with your monitoring ultrasounds?  I had my CD12 u/s today and had estrogen and progesterone drawn.  Problem is, the nurse never tells me what labs I need or if I even need them so today they had to call the doctor on call to clarify the order because I wasn't sure.  Of course I forgot to ask the nurse today when she called with my results of the ultrasound.

  • I am only doing TI this cycle but I think on CD 3 they do LH, estradiol, progesterone, hcg, and fsh. For CD 12 and 14 they did LH' estradiol, and progesterone. It seems like that is routine for my office, I never even asked or anything they just automatically took my blood. I would think you would want to know if your estrogen looks like the follicles are maturing? Not sure though!

    The only blood work I've had during an IUI cycle was on 7DPO for progesterone to confirm ovulation. I know some ladies here have their E2 checked before the IUI, I believe, and I kind of wish my RE did some blood work during the cycle so I know more about what is going on inside my body.

    TTC: 2 years
    Fertility blood tests all normal
    Tilted uterus
    3 day ultrasound 17 follicles
    HSG: 11/13/13- tubes open
    DH SA: SUPER sperm (145 million, 84% motility, 22% morphology)
    All infection disease and immunity blood tests NORMAL

    FIRST IUI May 2014: 100 mg Clomid days 5-9, third ultrasound CD 13 revealed four follicles 27, 24 and 20 and 13mm. Trigger shot May 28 with IUI May 29 and 30 (fingers crossed) - BFN started spotting 11DPO :(

    IUI #2 B2B 6/23 and 6/24, three mature eggs and 130 million sperm! FX this is the month!  BFN 7/8

    3rd cycle benched due to cysts - TI with OPK tests -  BFN

    3rd mediated cycle: Clomid+Trigger+TI  (three follies left side, one on right as usual....what the hell right ovary get it together and produce some damn follicles!) 7DPO progesterone level 43 with NO suppositories YAY for a natural strong ovulation. Beta canceled started spotting 13DPO - Third time is not a CHARM! 14 day cycle WTF! Everything looks normal - RE wants to start injectables next cycle so this cycle I'm benched

    Officially benched until March....but still plan on trying the baking soda douche due to my excessive and thick CM

  • Thanks, ladies!  I have had E2 done with each u/s to help them know when I'm ready to trigger (other than measuring follicles, obviously).  I never had 7DPO labs but will ask this time.  First IUI was considered a bust from the start due to poor sperm numbers, so there was no follow-up, I just called when AF came 2 weeks later. 

    I think we will be moving on to IVF next time if numbers are poor again :(

  • it's interesting that you all don't have as much bloodwork done! I wonder if it's necessary because it would save me a lot of money if it wasn't! I kind of liked seeing my estrogen levels rising though so I could kind of confirm that my follicles were good but if they can tell just from the size I wonder if I could skip it! Interesting!

  • I get an ultrasound around around CD3.  I had bloodwork the first couple of CD3, but now I just get U/S to make sure there are no cysts, and lining is looking good.  Midcycle CD 13ish, I get U/S and they check my progesterone and estrogen.  My doctor makes more of the decision to trigger based on the ultrasound.  And then I get labs a week post trigger.

  • The only labs I've had taken were my initial bloodwork and CD3 on my first cycle when I first started to see me RE back in November. She said since all of my labs were normal I didn't have to do bloodwork with each cycle. I've done 2 medicated cycles, with trigger shots and TI, one Medicated IUI with trigger all with US to see how my follicles were growing. The only time they wanted to take bloodwork was this month to confirm that I did ovulate on my own, because my RE said it looked like it on the US when I went in on CD13 (I was planning on doing IUI#2 this month)
    Now I'm wondering after reading everyone's post if I should be having labs done every cycle. I go to a well respected fertility clinic in my area, so I am trusting that my RE knows what she's doing. But now I am having doubts!
    Me: 37 DH: 37, married since Sept. 2013. "actively" TTC for a total of 13 months (after 7 months of TTC, took a 1 1/2 year hiatus due to work scheduling and wedding planning. Started up again after our wedding in Sept.) First evaluation by an RE November 2013

    HSG-normal
    OH-tiny polyp found, but RE assured me she doesn't think it's contributing to infertility and doesn't recommend me to remove it because it's so small.
    DH's SA-normal
    Hormones all within normal levels
    Ovarian Reserve-RE says "is great" she's not concerned about my egg quantity (which I was until I saw her and results came back good!) 

    04/2012 50mg Clomid=BFN (prescribed by OBGYN, not monitored)
    06/2012 50mg Clomid=BFN (prescribed by OBGYN, not monitored)
    11/2013 100mg Clomid, with HCG trigger=BFN (prescribed by RE, monitored with US (2 dominant follicles) and baseline bloodwork done)
    01/2014 100mg Clomid, with HCG trigger-(prescribed by RE, monitored with US-1 dominant follicle)=BFN
    06/2014 7.5mg Letrozole, HCG trigger (prescribed by RE, monitored with US-2 dominant follicles), IUI #1 on 7/3/14=BFN
    07/2014 7.5mg Letrozole, benched for IUI#2 this cycle, because I ovulated on my own before my follicular US. Progesterone suppositories=BFN Diagnosed with PCOS this month too. Testosterone levels high at 76. =( 
    08/2014 7.5mg Letrozole, HCG trigger with 19mm follicle. IUI#2 on 8/31 cancelled last minute due to family emergency. 
    09/2014 Taking a 3 month break starting this month. If not pregnant in 3 months, will start up again with medicated cycles and IUIs. Had all my labs and AFC re-evaluated this month also. RE says nothing much has changed since my eval last November, okay for me to take a little break. =) 




  • I'd be surprised if I didn't get stabbed by a needle every time I go to the doctor's office. Here's my usual workup:

    cd3: u/s, estradiol, progesterone to check that my innards are back to baseline and estradiol and progesterone are low
    ~cd 9: u/s, estradiol to check follicle development after 5 days of letrozole
    every second day thereafter: u/s, estradiol to check follicle development while on Bravelle shots
    trigger until I have at least one follicle >18mm
    ~7dpiui: progesterone
    ~12-14dpiui: beta

    I am a bit confused that my estradiol level never comes into play. It's been 268 (1 follicle) and 238 (2 follicles) the last two times I triggered for IUI on the day they told me to trigger. My nurse said they like to see 150-200 per mature follicle. At what point would the estradiol level cause concern if u/s showed a follicle large enough to be mature?

    IUI#2.1: BFN (July 2014; Letrozole 2.5 mg x5 days, Bravelle 75 IU x4 days)

    Moving on to IVF. (Why we're moving on to IVF)

    IVF#1 (w/ICSI): BCP 9/9-9/23. Gonal-F, Ganirelix, Low-dose HCG (antagonist protocol). 41R/35M/32F... 2 transferred on 10/14, 14 frosties! On cabergoline to help avoid OHSS. BFN, possibly because of 90% drop in estrogen and progesterone a few days after ET.
    FET#1: Transferring 2 on January 8. BFP! beta#1 (1/17): 408, beta#2 (1/20): 1310, first u/s scheduled 2/5
