meds and breastfeeding

I am 34.5 weeks along, I also have crohns disease/inflammatory bowel disease. I am currently on lialda and humira. i want to try breastfeeding LO when she comes. Ideally I'd like to breastfeed for a year so she'll get the most benefit. When i became pregnant i came off 6mp (mercaptipourine) to reduce the risks of damage to LO. My specialist wants to get me back on it asap to avoid buildup of antibodies to the humira. If i fail humira and start flaring we have no other options than to surgically remove my colon.

Obviously I want the best for both my child and i. In reading online about 6mp and breastfeeding it says LO should have regular blood checks (like i get while on 6mp) however I'm not sure if the site I read was the most up to date on info. Would LO still have the best benefits if i breastfed for 6 months instead of a year? Is this a med i could pump and dump once or twice a day and reduce the amount of the med in my breastmilk? I hope someone can help me...i realize this is something no one can be certain of but the more info I have the best informed decision i can make for the both of us. Thanks

    it looks like low doses are okay.

    pumping and dumping doesn't reduce the amount of medication in your bloodstream. when the medicine is out of your bloodstream, it is also out of your milk. pumping doesn't get it out of your milk any faster. you would just be pumping if you were missing a feeding, in order to keep your supply up. in general, you can try to time the administration of a drug so that it's right after you've just nursed, so you can minimize the amount that is still in your bloodstream/milk when it's time for the next feeding.

    they do research on drugs/human milk. you can call their hotline and they might be able to give you more information.

    if you've chosen a pediatrician, you could check in with them in advance as well. sometimes they are better informed about drugs in milk vs. OBs.

    breastfeeding for ANY amount of time is great! of course there will still be benefits no matter how long you do it. 6 months would be great because after that time you'd be able to introduce solids as well. like you said, you have to take both your LO's needs as well as your own health into account.

  • Thank you all so very much for all the information! I'll definitely give infant risk a call and discuss with them the risk of all three drugs combined. I would be on a low dose 6mp just enough to supress my immune system. I just know there's so many benefits to breastfeeding and I'd really love to at least breastfeed for a couple of months. I revisit my specialist about 3 months after LO is due and that's when we'll discuss again about going back on 6mp(so if anything she'll get about 3 months of milk).
