Disclaimer: this will be discussed with pedi next week at 6m appointment. And I'm afraid to Dr Google because I'm sure my kid will end up having a neurological issue if I read too deeply.
So, we've been done swaddling for a couple months now: he started rolling and breaking out of everything around the same time. He's in a Magic Sleep Suit which muffles his movements without confining him, and that has been working well until recently.
He's started in the past week doing some intense night flailing and waking himself. I've tried losing the sleep suit in hopes that he'll roll himself to belly and sleep better, but no such luck. He just freaks out more.
I've had to pretty much hold him through the flails until he calms and gets back to sleep. It happens multiple times a night. I've checked comfort: temp, diapers, hunger, pain, room temp - but those don't seem to be factors.
This is coinciding with starting to crawl and scoot, not sure if that would be considered related though?
So, back to question: concern or JBBB?

I have heard of the sleep-headbangers - I'm all for what the kid needs to get to sleep!
DS2 flails when he is uncomfortable. Like if my arm is under his head and he wants to lay flat. He also tosses his head from side to side while falling asleep. LIke @Keags5496 said, that is how he puts himself to sleep.
Have you tried NOT using the miracle suit?! Maybe that is bothering him/hindering his movements?
We went through a week of rough sleep as DS got used to being able to flip over/sleep flat after the rock and play.
@p2k2c4bp2k2 - at this age I'm okay with putting him down on his belly, but unfortunately, he'll have nothing to do with it. He won't fall asleep on his own like that, and he wakes right up if I roll him that way
I've also tried no intervention for up to an hour, but he just gets more upset and flails even more/harder. Then begins crying he can't settle himself down. All of my tricks for DS1 aren't working for this one. This past week has been terrible sleep. I'm out of ideas.
I've also noticed to that DS2 (will be 6 months next week) is working really hard on sitting unassisted and scooting. So I was chalking it up to working on new skills. I will be interested to see what your pedi says.

You want to critique me for putting '?!' Do you even realize that by pointing these things out and the "tone" I get from your posts is very hurtful? "Um, k" comes off as snobbish and rude to me.

Also, the "?!" is a "thing" I do. Not to be grammatically correct all the time. Just like people posting "Ded., LOL, JBBB, because tired..." etc.
