I don't have much on our list either! I am planning on buying a new stash of bottle nipples for when this baby starts daycare, along with a few different kinds of new pacifiers so we can see which one he likes. The other thing we are getting is another humidifier for this LO's room. It's more of a nice-to-have, but not a huge expense anyway.
Most of the stuff we needed this time around was storage oriented. Dressers, baby hangers, bins for holding toys.
We did not have a sound machine with DS1 but I'm getting one this time since I don't wont be able to have the fans on constantly when the twins get here.
I also picked up a few lovies/security blankets for the twins since DS1 refuses to part with any the ones we had for him. He keeps all 6 in the bed, but only really wants the same two.
I haven't bought anything for this LO (#3). But don't forget about the things you need for yourself PP....heavy pads, breast pads, nipple cream....ect. Also, I'm planning to get a new set of crib sheets (not necessary, but I feel better about it), binkies, and some kind of special lovie. My guess is that most things you need you already have or can get later. Also, you might want to let your DS pick out a special gift for his baby sister. My DD was so proud of the stuffed puppy she gave her brother.
We need new baby washcloths. I have the ones I use for DD, but I really liked those small baby ones for when she was itty bitty. The ones we had from when DD was tiny have been repurposed into her cloth napkins in the kitchen, so they're pretty stained and icky.
We also need a new sound machine because DD's died sometime last year and we never replaced it.
Otherwise, once Baby Brother has a room that sort of looks like it might be a baby's room, I'll start bringing up the baby clothes and bottles and supplies from the basement and figuring out what needs replacing or what else I need.
We got new pacis, new washclothes, and new bottle nipples with #2. Also needed a few clothes since they were different sex plus different seasons. With this one, we really don't need much since it's another girl, born same time of year. We'll probably get some more washcloths/ towels and new pacis, but that's it. Oh and check and see if any of your breast pump parts need replacing (if you have one).
Gripe Water! My son was colic... it started at 7 days old and he screamed in pain all night every night for 4 months. We didn't learn about gripe water until we were already 2 months in. It's all natural and helps with extreme colic or even minor gas pains. I'd get it before hand so you aren't drive get to the pharmacy in the middle of the night. I recommend the Mommy's Bliss brand because it's clear. There is one brand that is a black liquid and it will ruin clothes!

DD is getting her big girl bed this Saturday, so the crib and rocker will be moved into Baby Brother's room then! We got the antique desk out of there last weekend to make room. There is still a dining room table in there to be moved, but we're making progress!
How old is your step son? Is he going to be sharing a room with your DS?
I'm planning to get new tubing, new valves and membranes for my breast pump, diapers, wipes, and an extra baby camera to hook up to our monitor. Those are probably the common things that most STMs would need (assuming they pump). I have some other items but they are more specific to just me like a new infant car seat since our old one was in an accident.
Do you have a diaper genie? We want to stock up on diaper genie liners in addition to diapers, wipes, and diaper rash cream (we LOVE aquaphor).
We also plan to stock up on baby toiletries (like shampoo and body wash and lotion), and to revamp our car first aid kits. DH is an Eagle Scout so he's all "be prepared" all the time. :P
The only other thing I'm getting is a new diaper bag that will easily accommodate 2 kids. I had a cheap one from Walmart last time that I hated so much that I would stuff diapers and wipes into my purse so that I could leave it in the car...honestly, I need the bag more for the toddler's snacks, toys, and water, than I need it for baby stuff. But I want a bag that's as much for me as for my kids, so I'm thinking of getting a Vera Bradley convertible baby bag.

That's the one we bought! But I am a FTM so no clue if I will love it yet. But I do like that it can be a messenger bag or a back pack, not just over the shoulder. I hope it works for us!

This. I get what you mean though about feeling like you're forgetting something. I felt like that when I was pregnant DS2 and I feel like that a bit now. I just need to get stuff out of this attic and organized and I think that'll help. Plus girl stuff.
I just got a new pack n play. I didn't need it, but wanted it. I had a chicco and I sold it and just got an ingenuity. You can unzip the whole thing and wash every piece of fabric! I also want to get an ingenuity bouncer even though I have a bouncer.
As far as things I need, I will be getting new bottles depending on what LO likes, maybe pacifiers if he doesn't like the ones I have and a new baby bullet. My mom used my baby bullet one time and broke it! Grrr! And she didn't even need to use it because I had plenty of food ready.
