I don't know what to do for her. She screamed for about 30 minutes tonight when she was trying to eat -- nothing we did could calm her down. We were just at the pediatrician on Monday (um, yeah... third week in a row...), and they had us start administering her zantac dose differently because she was spitting it all out. Now she is getting the full dose, but still in awful pain. We go back to the pedi on Monday for her 2-month shots, and our pedi said if by then we aren't seeing any relief then she will send us to a pediatric GI doctor. Does anyone have any experience with this? What will this doctor do? It hurts me so much when she cries from pain. And the worst part is she is HUNGRY, but she can't eat. Ugh, I am so frustrated.

My pedi suggested that we do mylicon every feeding (we did it three times a day), tummy soother twice a day and .9mls of Zantac along with soy milk and keeping them at a 45 degree angle. It is awful but if it gives you hope, my girls are finally growing out of it. Katie is off zantac and Karly still spits up a little but is off zantac.

Zantac is in a class of drugs called acid blockers. Often they do not work well and did nothing for DS's reflux. He was switched to prevacid, which is a proton pump inhibitor (PPI) which is a stronger class of drugs. While it's often a challenge getting the prevacid in him, it works wonders. I would ask your pedi to switch to prevacid and try that. Also, if she is in alot of pain, you can give infant tylenol for the pain. We had to do that for DS's reflux when his throat hurt waiting for the prevacid to kick in. It can take a couple of weeks before the prevacid starts working. TheShellSeeker did a pretty good job explaining it. Zantac shuts down only 1 of 3 acid producing elements in her body. Prevacid shuts down all 3. Our doc had us switch to Prevacid too and it's been awesome. I also use gripe water during the day too for the gas and that has also been helping.
If you are nursing, have you also considered making some changes to your diet? I went pretty extreme and did a lot of things at the same time, so I'm not sure what combination made the difference, but between the Gripe Water/Prevacid/Dietary changes (on my end), he is a much happier kid.

a ped GI is the best thing for her.
they will likely either increase her dose or put her on Prevacid (i'm a prevacid rep- so if that happens page me if you have questions about it).
they can also offer eating/feeding/positioning techniques, etc. They may do an endoscopy to see how things are looking on the inside - if so- don't worry- it's a really easy procedure.... just seems really scary.

Prevacid got it's pediatric approval about 6 years ago... it's for age 1 y/o and up. Pediatricians in general take a LONG time to adopt a new med into their routine.... they are afraid, want to see longer term safety, want to hear about other peds they know using it, etc.
someone else explained it pretty well - that Prevacid (a PPI) turns off the entire pump that creates acid, while Zantac/Pepcid only turns off one of the 3 receptors for the pump - it's an H2blocker (histamine)... so the other 2 recpetors on the pump are still active and promote acid production.
PPIs are the best there is for acid suppression- in adults and kids... until recent years even adult docs were hesitant to use them as first line treatment b/c they were considered "big guns".... Now, since H2blockers are all OTC and even Prilosec is OTC (it was the first PPI)... adult doctors have no fear of going straight to a PPI (like prevacid, nexium, etc).
Peds still often consider PPIs "big guns" for babies... b/c the approval is more recent, and it's not approved in infants.... though, tons of docs do use it in infants.
sorry- that's a really long answer for why they don't go straight to Prevacid
Hope it sheds some light on why- so people don't think their peds aren't doing the best they can for their kids... they are- they are just being cautious.

you know im here to agree about the prevacid! it def helps soooo much with the pain..the boys still spit, but without pain, after prevacid they spit with a smile, haha
tina is right too...she WILL grow out of it..it seems like forever, but it does get much better. I have heard the peak is about 3-4 months usually..so you are almost there!?

at about that stage with Ethan we added prevacid. for about 2 months he took both zantac and prevacid... Good luck, Jill!
Oh, and I meant to ask, are you still doing cereal, or did you try the enfamil AR? We made that switch at about the same time and it was a worthwhile difference. Is she having trouble digesting the cereal? or having constipation issues with it? Poor baby...
I hope she (and Mommy and Daddy) get some relief...
I also wanted to mention gripe water and also if your Dr is willing to go to prevacid, we get it compounded rather than giving E solutabs, because I felt like we were wasting so much and it was so hard to get the dose into him.
