Endometriosis and laparoscopy

First off, my apologies if this has been asked before, but I've searched all over and can't really seem to find the answer to my questions. I'm truly hoping someone can give me some help.

My sister recently had some tests done regarding other health issues, and one of them was a lumbar MRI, everything came out fine, except for the tiny fact that a cyst was found in her pelvic area. So I went with her to see her OBGYN and he did an ultrasound and said that everything seemed to be just fine, that she did have a cyst but that at her age (35) it was not uncommon. She told me that she did complain a bit during the ultrasound, she said that she would get this small amount of discomfort when that stick (that's what she calls it LOL!) was moved in a certain direction. He showed some concern and said that he wants to do a laparoscopy (sp?) to check for endometriosis (sp?).

We both got home and did our usual google search and she is not very comfortable having that procedure, she says that she might seem to have some of the symptoms of endo, like lower back pain and discomfort during intercourse, but aside from that she claims to not have any other symptoms, no horrible cramps or excessive bleeding during her period, or any other one.  She does freak out easily, and the complications of the laparascopy, even though not common, still scare her and also myself. Her husband and her were talking about wanting another baby, they have already have two children, the youngest is 9 and they've been wanting another one for a very long time, and now that they are able to do so, this happens. Needless to say this came as a big blow to her, and has been super down, doesn't want to do anything, and well I'm here trying to find some answers for her, I hate seeing her so sad, it breaks my heart. Assuming she were to actually have endo, would it be a bad idea to try to conceive now, before that procedure is done? Would the endo harm her or the baby in any way? I see her so confused and so scared, this was not something that her or anyone was expecting, and I know the diagnosis isn't there, but I'm fearful it might be.

  • If she can get pregnant, the endo will have minimal to no effect on her pregnancy. She may have increased pain as the pregnancy progresses, but nothing unbearable. The vast majority of women actually get a relief from their endo pain during pregnancy because the higher levels of progesterone during pregnancy cause a bit of remission

    The biggest concern is with trying to get pregnant. At her age, I would recommend that she schedule a consult with a reproductive endocrinologist at the 4 month mark since she has symptoms consistent with endo. 
    Thank you so much for the advice! I'll be sure to give it to her, and make sure she schedules an appointment with a specialist. I need her back to her cheery self. I know that she already has kids, so many might think that she should be fine, but it's something that she has wanted for years, and is nos financially stable to bring another child into their life, and she was so excited.

  • I think it is INSANE to do a lap under the circumstances you've described. Endo is the leading cause of infertility; however, not every woman with endo is infertile. There's absolutely no reason to jump into surgery before attempting to get pregnant. If she finds they're unable to get pregnant, then sure, revisit the idea; but until then, honestly, the lap could cause more problems than it solves.

    I was diagnosed with endo 13 years ago, and had lap followed by Lupron. If she is not experiencing severe pain, terrible periods or infertility, then I can't understand why a doctor would recommend a lap. It is the only way to definitively diagnose endo, but not necessary if she doesn't need treatment.

    If she is otherwise healthy, then endo itself is not a reason to stop TTC. Not everyone that has endo has infertility issues.
