Reflux or Gas??

My DD is 5 1/2 weeks old. We formula feed her. She has been almost constantly fussing at the bottle for the past 3 weeks or so. She will drink for less than a minute before pulling back and crying, super red in the face, while kicking her legs or pushing back with her legs. We pull the bottle out of her mouth and try and put it back in. Sometimes she still cries, sometimes she takes the bottle back and eats. Rinse and repeat.

I try to burp her every 2 ounces, which is half way through the bottle since she takes about 4 ounces each feeding. Sometimes she doesn't burp, she's a horrible burper. Especially at night, we swaddle her and leave her swaddled since she doesn't poop in the middle of the night, and almost always never get a burp out of her.

She usually only spits up at night, but not every time. Hardly does it during the day. When I told the pedi this, she thought maybe she was swallowing the spit up. We also give her gas drops after every feeding, and sometimes in between because of the fussiness.

She makes a lot of noise at night, lots of grunting and throat clearing noises. I'm not sure if it's because of acid coming up, or because she is swaddled and is trying to get to her hands. We have her in a RNP because of the angle at night and also during the day.

She was BF the first 3 weeks while also supplementing because she was losing weight. We had her on Similac for supplementation, but she would poop that out in less than a minute of having it. So we switched her to Similac Sensitive. She did much better on that, but still super fussy while eating.

When I went to the pedi with my concerns of reflux, she put her on Prevacid twice daily. That was a week ago as nothing has changed. We also switched formula to Enfamil with Added Rice Cereal. Also no change.

Could this be gas and not reflux?? I wonder if it is more gas because she seems to do better the second half of the bottle. We just don't know what is going on, and feel like the things we are doing are not working. :(

  • DS gets mad while eating and red to the face when he needs to burp. Sometimes we need to burp him frequently when feeding. Not sure how you are burping, but over the shoulder seems to work best for us. The grunting and throat clearing sounds like DS as well. The pedi told us it is reflux. Maybe the does on your meds is too low? Also, the pedi told me that the rock and play or anything that causes baby to sit like that (car seat, etc) can make gas worse. I don't know of this is any help, just sounds a lot like my LO.

    It sounds like my little one. She has gas, reflux and colic. The way my doctor described colic is that it's in their belly (gas) and it makes them uncomfortable. The crying during the bottle makes me think it's also reflux, as well as the clearing of the throat. My girl still arches her back and straightens her legs even on her Zantac. Her doctor said it's more so to help the acid not hurt her but she may still be a little uncomfortable. Who knows. My girl gets super fussy and grunts when she has gas. I tried Enfamil and Similac formulas with no luck. I finally found a decent fit with Gerber Good Start Soothe. It's for colic and fussiness. Plus is relieves her gas and makes her poop regularly. She is a lot better on this formula. Still has her moments but don't all babies.

    My little one also sleeps in a RNP because of her spitting up.
  • @manna1212‌ sounds like the RNP is a double edged sword. We use one for spit up but it causes more gas? Oiy. Well I would rather have my little one have gas than choke on her spit up. These babes are difficult! :) ha.
  • I forget to ask, have you tried Gripe Water? Some people swear by it. It doesn't do much for us.

  • Could it be the flow of the bottle? Your DD is fussy because it's not coming fast enough?

    DS gets really fussy if it's not coming fast enough. He was also a horrible burper at that age. Their throat is still immature at that point and it took him a couple weeks to grow and get it sorted. Now he's fine except he's laying on me passed out not burping tonight.

    Just a thought.
  • When our pedi talked with us abt reflux, she said to try doing smaller bottles with greater frequency because eating large quantities at once allows the acid to rise (which made sense to us) and also had us mix a tiny bit of rice cereal in her bottles to make it a tinge thicker/heavier. Since doing those things, we have way fewer incidents.
  • She has been on Enfamil AR (with the rice cereal already added into it) for a few days now. We also had to bump up the flow of the nipple because of its thickness. So originally we had her on a premie nipple because she was chocking. Then I thought the same thing about her being frustrated that the flow was too slow, so we moved up to level 1. Now she's on level 2 with the rice cereal. No change :(

  • This sounds exactly like my lo! She does the same thing during her bottle and sometimes cries after she eats. She has been on zantac twice a day for a month. We are trying different formulas. We tried enfamil AR which helped a little but she still seemed uncomfortable. We are trying similac alimentum now but only on day 2 of that.
  • @bridetobe9132013‌ how long did you wait between switching formulas? Do you have to give it a certain amount of time to see results?

  • I think all the advice I would have has been captured by the PPs: different bottles/nipples (my LO greatly improved by giving her different nipples), slow down feedings (after every oz or two I try to burp her and then give her the paci to suck on for a few mins before giving her more food), keep her upright longer after feedings.

    My pedi said it can take a few days to a week.
