Pregnant after a Loss

Talk to me about PIO?

My progesterone levels are dropping quickly. I've been 30 for the past 2 weeks. Then on Monday it was 25 and today (Wednesday) is 17. I go in tomorrow for an injection. Please share your experiences with me.

Re: Talk to me about PIO?

  • I'm sorry. I'm sure that is nerve wracking. I hope that it does the trick quickly.

I don't have any experience myself, but have heard that they can hurt a bit, but that massage and heat can help afterwards to prevent knots from forming and that warming it a bit before injection can lessen the pain. On the upside, with PIO you get to skip the vag irritation and possible spotting that can come with supps.

If you end up getting a prescription to do them at home, there are some recent threads that might help (you may have already seen them).

I hope you don't mind me responding even though I'm not PgAL.  I've been on PIO shots for two FET cycles and they did seem scary at first but they aren't too bad.  I usually ice first and then DH runs warm water over the PIO in the syringe.  This helps thin the PIO out so it's not as thick going in.  I usually stand on the opposite leg from the shot side to take all the weight off the muscle the shot is going into.   Afterwards, DH massages the area so the PIO doesn't cause a lump in the muscle.  I usually sit on a heating pad after that for about 10 mins.  We alternate sides to give each side a rest for a day.  I do find my left side is always more sore than my right for some reason.

    I hope this helps!

  • I have had decreasing progesterone in all 4 of my pregnancies. They were usually between 35-50 at 17dpo and would continue to fall for 3-4 weeks. I usually bottomed out around 13-16. I did injections with my first pregnancy, but ended up having an injection site reaction to one of the preservatives. Since then I have stuck with the suppositories (sometimes twice daily depending on the value). After the 3-4 weeks my progesterone would slowly climb.

I know it is do stressful, but I'm sure that the injections will help boost you numbers for you. Try to hang in there. ((HUGS))

  • I'm on the scary progesterone journey this time too but I'm on oral progesterone. OB says as long as level >15 things should be cool.

With DD my levels fluctuated between 15 to 17 and 18 to 22 depending on the week and the dose and the time of day I had the labs.

It is scary to have the levels fall! Be good to yourself. Hugs.

No personal advice.  However a friend of mine had the suppositories until 13 weeks or 16 weeks or something like that and switched to injections.

This may not be helpful (sorry if it's not).  She has a nurse that comes once a week to inject, which occurs on her behind.  They switch cheeks each week.  She prefers to have it done on Friday while working from home as it takes up to a minute to actually inject since it's oil based.  She also said she's very sore afterwards for the next day.  She tries massage and a hot pad on the area to help afterwards.

    Good luck!
Oh how I would love to have a nurse come to my house. That would be extremely helpful. But since I have to do the inject once a day that's not really an appropriate expectation. Thanks for the tips!

  • Thank you everyone for your support and advice. I did the first inject in the doctor's office today. I'll be doing them at home once a day from now on - or I should say my husband will be doing them. It's hard to reach around your own ass with a syringe. Next appt and blood draw is Wednesday. Nothing to do but stay on mother humping expensive meds and hang on. (((hugs)))) back to all.

Just saw this. No advise on the PIO.With my last loss, I was put on HCG IM injections and I just could not stand to let my H do them. I was able to do them myself, but felt I had to contort and I didn't like having to do injections with my left hand when I did the left side.

My progesterone levels never took off-started low (13), climbed up (highest was 27), then back down. Before being released to o/b, level was 16.9 at 7 weeks, then was 23.7 at 9 weeks. I always had blood draws around 7:30-8am and did not eat before, so at least that stayed consistent. I haven't had a draw for p4 since 9 weeks. My RE had said they look for the number to be above 12.

    Good luck to you!
