Switching from hybrid to all-injectible cycle?

Hi ladies, so IUI#2 was a BFN.  The witch came today and the RE's office called to say he wants me to skip the clomid this month and go with just the follistim. What are your thoughts on that approach? What difference would it make if any?

ME:      36, on BC since 1996,  irregular periods.   HUBS:      37, healthy

2001—meet.  2005—marry.  2006—Furbaby is born. 

On BC till end of 2009.  2010 NTNP.   

New Year’s Eve 2010—BFP?! January 13, 2011—early loss, HCG peaked at 324, no D&C needed.

Feb 2011-Nov 2012—NTNP 

Dec 2012 --present (TTC 20 months) Not even a hint of a BFP

Me:  BW, U/S, HSG, HSN normal.  AMH 2.33.  Hubs: SA normal

April 2014--IUI #1 Clomid 100 4-8 Follistim 150 9-14, 5 mature follies at trigger, Peak E2 5/5 1100, BFN 

May 2014 ---IUI#2 Clomid 100 3-7 Follistim 150 8-13, 3 mature follies at trigger, Peak E2 6/6 547, BFN

June 2014--IUI#3 Follistim 150/225 3-15, 3-5 mature follies at trigger, Peak E2 7/11 1450, BFN

July 2014--IUI#4 Follistim 225 2-12, 3 mature follies at trigger, Peak E2 8/12 841, BFN

Breaking till after Christmas, then IVF...taking 4000 mgs daily Inositol and Melatonin 3mg

  • My doc put me on straight Follistim this cycle after my hybrid cycle failed.  She said the Femara didn't do anything for me so... here I am.  It wasn't too bad, cost wise.  About the same as my hybrid cycle since I needed the same amount of Follistim anyway.  That was my biggest concern :)

    Can't say it worked for me yet, obviously, but I do know straight Follistim is a stronger form of stimulation than pills + injects.

  • Bah, TB won't let me edit but I wanted to add the differences between the cycles for me.

    As you can see in my siggy, I did 5 days of 5 mg Femara and then 100 iu Follistim from CD 8-10.  My follicles at my first scan were like, 14 and 12.  They had me stim at 125 iu from CD 11-13 with one final push on CD 14 (150 iu) and my follicles were 22 and 20 at trigger.  That's a total of 12 days of stimming.

    This cycle I did Follistim (100 iu) CD 4-9.  My first follie scan I had a 15 mm follie and a 13 mm follie.  I took another day of Follistim (CD 10) for 150 iu and went back the next morning.  My follies were 17 mm and 15 mm.  My nurse said I would do one more night of Follistim and then trigger the next day.  But then my blood work came back and doctor wanted me to trigger that night - guess my E2 and LH were signaling faster growth than expected.  On all Follistim, I only stimmed for 7 days. 

    And that, I think, is the biggest difference.  Follistim works much more rapidly - at least it did for me.  Whether or not that will make me pregnant, I don't know.  I'll find out in a couple weeks ;)

    My RE had me skip right over Clomid/Femara and go straight to injects (follistim) only with my IUIs.  It's a little more 'natural' on your body (Clomid tricks your brain, hence more side effects).  At least that's how my RE explained it.  And I've responded pretty well/consistent with that.  Good luck!

  • I honestly never tried anything but injectables so no advice about the difference. Just wishing you luck!

  • I used Femara for four cycles - only 3 IUIs though. Then I switched to Gonal-F (same as Follistim) for four more cycles. Only two of those resulted in IUIs because I over responded the other two times. Prepare yourself for over responding and decide how many mature follies you're willing to do an IUI with. For us, 4 was our max. Even that made me nervous as we are not willing to do selective reduction unless absolutely medically necessary.

    We are moving to IVF because, in short, it is more controlled.

    We've been working on baby #1 since July 2013 using Open ID donor sperm.  8 IUI attempts with 5 actual IUIs and one chemical pregnancy.  We have one fresh IVF cycle under our belts as well as a FET.  I have endometriosis and a uterine septum that was corrected via surgery in November 2013. 

    11/14/14 -  Second HSG shows that tubes are still clear and ute is looking good. 

    12/6/14 - Started BCPs in prep for IVF #2

    12/22/14 - Saline u/s and endometrial scratch (All was clear and OUCH!)

    1/2/15 - Began stimming for IVF #2
