A has an irritation between his scrotum and inner thigh. Heat rash or diaper rash?

He doesn't have irritation anywhere else.

I am trying to decide if I should let him go naked for an extended period of time or apply diaper cream to the area. I think the answer is go naked, but then I get to deal with accidents.

Or will diaper cream help heat rash if it is that?

LO has extra sweaty days.
If possible, I let him go diaper-free for a while each day. (Not always possible)
He seems to poop every other day lately, so I try on the 'no-poop' days.
He hasn't peed all over ever when I've had him naked.

DS1 pooped multiple times a day until about 8-10 months, so I wasn't chancing it with him. Lol.

If we can't do no diaper then I use cream for sure.
He hasn't actually had a rash, but quite sweaty and sometimes red.

Ask your pediatrician if you can apply non-talc powder or corn starch on the area to dry it out. T's pedi told me to do that on his heat rash on his neck.

We use cornstarch baby powder to keep AJ dry.

My mom and grandma always swore by cornstarch on bum rashes and it worked on my daughter dried it right up

I let Colton go diaper free everyday and it keeps heat rashes away. We cloth diaper so I just lay one under him and leave it open.

It sounds like something I've had that's similar - it goes away if I just clean it thoroughly, make sure it's dry, then put aquaphor on it. ETA by "I" I mean LO.
