XP: Measurable Follicles at Baseline

Hi ladies. I kind of posted this on 3T since I am doing an IUI but I am really hoping someone might have more information.

Yesterday at my baseline appointment (CD3) I had 8 AFC on the right ovary, but on the left there were 3 little ones and one around 14.4mm and another 13mm. After looking at the bloodwork the RE confirmed that they are follicles and producing estrogen. Because it is an IUI cycle he might push me to do a natural instead of the medicated cycle I wanted.. But my question is- Does anyone know why this happens? This is the 3rd time I've had measurable follicles at baseline.

My AMH recently went up to 2.7 and the last time my FSH was checked it was 4.8 - That all seems really good...

The questions I am coming up with have to do with implantation. Because I normally trigger so early, my lining sucks. And with my new list of autoimmune diagnoses, I am not sure how the timing works for the RI protocol (start lovenox CD6 and get an ultrasound 5 days later- but before the IUI). So even if the eggs fertilize, I'd never know if it was just that they could not implant.

Every part of me says maybe if I did one more IVF with the lupron and BCPs it would work out better. But it seems like such a risk.

Last IVF was Estrogen Priming.. and we found out that I all my eggs were dysmorphic because of my high estrogen levels. I've read every article published about it. And most say it doesn't always happen, that it is extremely rare, and never happens to all of someone's eggs. But that's me.

I'm just super curious as to how I would respond with the other protocol. Maybe I need lupron suppression for an IUI?

Hmm, I typically had follies too early but we assumed it was bc if my high FSH and that I was recruiting the follies in the luteal phase of the cycle just before. I was going to suggest estrogen priming so if that doesn't work for you, maybe a week of BCPs?

Before I finished reading your post I was wondering about doing lupron with your IUI. It would keep your ovaries quiet until you were ready to stim and I wouldn't think you would have to worry about over suppressing since you are doing an IUI. The one thing you might want to talk about is whether lupron would hurt your lining. I have heard of women having trouble building a lining after being suppressed by lupron. Normally you would add estrogen to help build the lining, but I imagine that would be risky given your issues with your egg quality declining due to high estrogen levels. I hope your doc has some recommendations.

Thanks for the replies.. I am not sure about BCP since they contain estrogen.. And I completely forgot about lupron lining. I haven't done a lupron IVF since 2011, so I know I am forgetting some things.

I just read a study that claimed taking cabergoline would help people who might trigger early... but I'm already on it.. and my prolactin levels are under 5.

I have no advice, but wanted to send some hugs and good thoughts your way. Have you discussed this theory with your doctor? I'd be curious what he has to say!

@RunnCC37 I just sent him an e-mail. I am going to get there at exactly 6am tomorrow so hopefully he will have 3 minutes to touch base with me about the 5 questions and scenarios I sent him.

I just looked up DHEA and read that in at least 4 studies it had a significant E2 spike in all patients..

Why am I on this then??? Dr. KK backed it down to 25 mg and my nurse emailed me saying I needed to take 75 mg... Looks like I have another question

I'm wondering if what they are calling measurable follicles are actually estrogen producing a cysts?

Either way, I think using BCP,and possibly lupron will be beneficial for you.

Thanks ladies.. They grew even more and I will be ovulating within the next day.

Unfortunately my Dr. said this is a one time thing- but I feel like we've been seeing this in the last 2 cycles.. just each time more extreme. In August I triggered on CD8 and in December I had a measurable follicle at baseline and triggered on CD9. He said to wait until next month and see what happens. Then if it does happen again, maybe he will entertain these ideas.

I'm just nervous because he said I will ovulate 2 of the follicles but there is a 3rd that might not respond to the LH surge... so maybe I will be in the same position next month.

I feel like the only person who could have easy, breezy IVF cycles and my IUIs are way more complicated.

He just wrote me back recommending estrogen priming. I'm so confused.
