Visiting from 3T... questions re: laparascopy and hydrosalpinx

Hello! I am stopping over from 3T to ask about success after dealing with hydrosalpinx (as well as with multiple surgeries). My long history is in my signature. But basically I had surgery last July to remove fibroids. I've had four failed IUIs since then and was preparing for IVF. But then today I had a third HSG and both my tubes are now blocked and the RE is recommending laparoscopic surgery in order to move forward. I need to schedule a new consult to discuss details, but someone else at 3T said it sounded like what he was discussing is a hydrosalpinx (I looked it up and that sounds right). Basically, I was really upset today at the possibility that I have to have surgery AGAIN and I'm wondering if there is anyone else who has had a similar situation and later became successful. Same question regarding hydrosalpinx.  Thanks!!

2008: HSG (normal), couple rounds of clomid through gyno
2008 - 2010: dragging my feet out of fear and procrastination
October 2010: first consultation with RE, dx PCOS and fibroids (DH slightly low count/motility)
Oct. 2010 - Dec. 2012:  In DENIAL! avoided the issue because I was scared of surgery
January 2013: returned to RE, fibroids grew significantly
February 2013: second HSG, fibroids pushed on tubes which blocked them somewhat
March 2013: MRI to determine what type of surgery may be necessary

July 29, 2013: fibroids (5) removed via robotic laparascopy
August 2013 - Nov 2013 : benched due to recent surgery

IUI #1, Dec. 24, 2013, BFN 
IUI #2, Jan. 25, 2014, BFN
IUI #3, Feb. 25, 2014  BFN
IUI #4 canceled due to lack of response to letrozole
IUI #4.1 April 28, 2014, BFN

May 16, 2014: wtf consult, start prepping for IVF in June and add injects for one last IUI in the meantime
IUI #5 started letrozole and bravelle but canceled after HSG led to new diagnosis

May 21, 2014: third HSG, tubes blocked, one at the beginning, one hydrosalpinx??
June 11, 2014: consult, approved to move on to IVF because the hydro is not completely blocked therefore allowing fluid to move through slowly rather than backwards
IVF #1 August 8, 2014 - 3dt of 2 embryos, BFN
September 17, 2014 - 4th HSG, the right tube is very patent (open!!) dye went straight through this time. Weird!
October 2, 2014 - started metformin treatment
November 14, 2014 - blood work, brought A1C down from 5.8 to 5.5

November 26, 2014 - RE finally back from vaca and reviewed my chart, no more IVFs for rest of calendar year
December 1, 2014 - Right after Thanksgiving, I called a new clinic and got in right away! Plan for IVF
December 17, 2014 - ER! 29 retrieved (!!), 16 mature, all 16 fertilized (ICSI)

IVF #2 December 20, 2014 - 3dt of 3 embryos, BFN

We are done with treatment unsuccessfully. :(

    I did have a hydro removed and moved on to (finally!) be successful with IVF.
    If what you have are truly hydros, the fluid that leaks into the ute from them can prevent or hinder implantation of an embryo(this is what may have been happening in my case. My full history is in my siggy but we did have a couple "unexplained" chemical pregnancies with great looking embryos). I was given 2 different options when my hydro was finally discovered-- a lap to disconnect the tube or there is a way to shore up the connection of the tube to the ute. The second method involved a hysteroscopy(less invasive than a lap) but it did require a 3 month wait to allow for healing. In our case we moved straight to a lap, disconnected the tube, and were successful with our next transfer.

    I know the idea of another surgery is daunting, but with hydros it really is for the best to take care of them before you cycle. Let me know if you have any more questions!

    Thank you so much! I may definitely have some questions for you in the future. And I was wondering that with a lap... afterward you can go straight into a cycle? When I did the surgery last year I had to wait three months. Did you have to do another HSG to confirm everything was okay with the tubes or no?

    Since the surgery was just to disconnect the tube and we were doing a frozen transfer, my RE let us move right into a cycle without waiting at all. I'm not sure that would have been the case if we had to do a fresh cycle, but you could always ask. No HSG was needed after my surgery.

    Keep in mind that since you would be gearing up for your first fresh IVF cycle, there is some testing/prep involved so your timeline will depend on that.

    Ok, thank you! We had started to prep for IVF but good to know that a lap would not further delay it! Yeah we still have to attend their seminar and do BC and all that.

    I was! I was! I had both of my tubes removed due to hydrosalpinx in 8/2013. As u may know, this is recommended when going forward with ivf because the fluid in the blockage is toxic to a fetus...fast forward to 2/2014- first cycle of ivf received my big fat positive. Now 15 weeks pregnant... By the way I also had a myomectomy in 2012...it does suck to have surgery again... The removal of the tubes had me sidelined for approximately three days... Please feel free to ask other questions. If u need to personally message me :-)

    I've had 2 laparoscopies. After my HSG showed blocked tubes, my RE said we needed a laparoscopy to confirm. Before the surgery they asked if I consented to having them removed if they were going to hinder my chances at conceiving and I said NO WAY! lol 2 failed cycles later, I decided to remove them. They were so full of fluid that they could actually be seen on a ultrasound screen (very rare). I had them removed and literally was up and walking the next day. I waited 1 month and did a FET and it didn't work. (We later found that I had some other issues!) but eentually it did work. I just wish I did it earlier. In my opinion, if you're going to spend 10k on IVF you want the BEST environment possible for those little embryos! Good luck to you :)

    My tubes were destroyed from endometroisis.  I did have surgery to have them cleaned up.  I did get pregnant 3 mo later on our own but it was ectopic.  When we were referred to the RE he discovered that I had a hydro on one side, and my other tube had been damaged even more from the ectopic (and the ob removed some of it, but hat is a different story).  So my RE would not do a transfer until I had the hydro removed because of what everyone else said, the fluid is poisonous to the embryo and it is not able to implant.  My RE ended up removing both tubes. This was in November 13 and they let me start up for an FET in Jan.  
    Good luck! You definitely want the best environment to give those embryos the best chance!

    No first hand experience but I had an ectopic and my RE and I talked about my tubes.  He told me that he always removed hydros before he will do IVF, so it sounds like standard protocol, especially given what others have said above.  Good luck!
