Size dif/gestational age for vaginal birth?

I know a lot of obs (mine included) say that if baby b is bigger, they will not allow a vaginal birth. Is it that cut and dry or is there a little wiggle room?

Also, how early do they allow vaginal births? I am currently just over 29 weeks with some concerns for ptl. I have read the literature review where they state outcomes for vaginal vs c/s, but those seem to focus on weights vs gestation.

  • My docs have been telling me they will let me go 38 weeks max. And my Baby B is bigger than A by 1.5lbs. Planning to discuss options at my OB appt this Wednesday. I have my first BPP on Tuesday so hopefully that will give us a better picture of what's going on. My OBs are willing to do breech extractions and trying to turn the baby if they are not head down, so a vaginal birth isn't necessarily out of the question. I just am not sure on what size difference they will do this up to. It seems that my OBs are more concerned with gestational age vs baby size too. I am really hoping they take mamma's size into account too because I am starting to struggle carrying all this weight around!!!!

  • It depends on the OB that is delivering you. They all seem to have varying levels of comfort w different presentations.

    That being said, you won't find anyone who will deliver a breech baby a vaginally. As for B bigger than A, I think it's around an estimated 10% size differential for there to be concerns around that issue.

    Some will do breech extractions of baby B and some will not. Doing a breech baby B who is larger than A would be tricky.

    I don't know about GA, though.

    Not sure this will help, but my mo/di boys never measured more than 10% diff on a growth scan (it was usually more like 0-3%) and baby A measured bigger on the last scan, but baby A came out 20% smaller than B (1lb 6oz smaller) and I had a vaginal delivery. It was def much harder to get Baby B out but there were no complications. They were 37 weeks. Both were head down. I'm not sure if my OB would have been hesitant if they knew baby B was bigger.

    Eta: I was in PTL at 32 w and they were going to let me attempt a vaginal delivery
  • Um I wish I had more information ahead of time.  I had a text book pregnancy up until 30w2d when my water broke.  I had no idea the risks of vaginal birth on premature babies nor about the discrepancy of weights.  That being said I don't know that it would have changed my outcome but I felt like I would have been more prepared.  After my water broke, they didn't even talk c-section, it was just assumed I'd deliver vaginally.  Nathan was also bigger by 6oz as baby B and transverse but that didn't seem to be an issue either.  So I delivered Joey at 3 lbs. 6oz head down, and Nathan was then manually turned head down and he was 3 lbs. 12 oz. at 30w3d.

  • Mine said the difference needed to be "reasonable," like 20% or something.  I delivered vaginally.  My "A" was 5 lbs. 10 oz. and my "B" was 6 lbs. 15 oz., so I just made it.  I don't know about your second question.  Mine were born at 36w5d.

    Did they do a growth scan ahead of time to estimate sizes?
