Okay, I feel like this is going to sound dumb, but it's driving me crazy.
I started filling out preregistration paperwork for my hospital, and there's a whole consent form for a pediatrician. My sister had kids a few years ago, and she said she just used whoever was on-call when she delivered and just continued to go to them after. The form I have from my hospital makes it sound like I HAVE to have someone preselected.
I have no problem picking someone ahead of time. I found someone nearby, in network, affiliated with the hospital. Great. But do I just list them on the form? Or do I call them and let them know I've selected them?

Most pedis don't actually see them in the hospital unless they have rights there but the on call pedi does for the hospital. You go to the pedi listed the day or two after you leave..
It is the same way they won't let you leave without you physically making their first appointment or at least our hospital won't.

I can't speak for your specific forms that you fill out. I think that is just specific to how your hospital handles their preregistration process. You can always call and ask questions about it, I'm sure.
