Trouble TTC

Intro

Hi, I wanted to introduce myself as my husband and I have been TTC for 12 months now.  I was active for a little while on the TTGP board, but stopped being as active when it became clear that things were not going as hoped.  We are both 32, have been together for 10 years, and I came off of OCP at the end of May 2013.  Since then, I have had unexplained anovulation.  

Our OB/Gyn referred us to an RE a month early due to past-30 and anovulatory state (confirmed by temps, OPK).  Our work-up thus far as been negative (normal SA for the hubs, normal HSG for me, no evidence of PCOS).  From what I understand, I basically have a low-estrogen state. They thought it was originally due to weight, so I gained some (BMI totally normal).  They then thought it was due to exercise-induced amenorrhea, so I stopped running seriously (now about half of my previous activity level, if that).  Now they think it is otherwise unexplained post-pill amenorrhea (took OCP for 10 years prior to TTC).  

The plan is (monitored) Clomid to start in one month (due to work/vacation schedule, ability to be there for ultrasounds).  My RE thinks that as the primary problem is anovulation (normal SA), we don't need to do Clomid combined with IUI but I am a little worried about potential effects on endometrial lining/CM? Thoughts?

  • Welcome to 3T! Sounds like you have a good plan in place. Since you're being monitored, I'm sure your dr will check your lining during your u/s. I've been taking clomid for a while and haven't had any lining issues. Good luck! :)

  • Welcome! I think I might remember seeing you on TTGP.

    My only problem is anovulation too, and I decided I wanted to jump straight to IUIs with my Clomid cycles instead of TI. Like you're saying, I didn't want to have to worry about CM (though it seems like it's been normal on Clomid) and I thought there would be less pressure overall that way.

    As far as your lining, that doesn't have anything to do with IUI vs TI (not sure if that's what you meant), but Clomid doesn't always thin your lining out (mine has been fine). If you are properly monitored and your lining isn't thickening in proper relation to where you are in your cycle, then you can take extra estrogen.

    Slowly rising betas - Ectopic suspected on 8/8/14 & confirmed on 8/11/14
    Methotrexate on 8/12/14 -HCG negative on 9/2/14
    IVF #1- November- Antagonist protocol: 11/1: start stims, 200iu of Follistim; 11/12 ER 17R/14M/14F; 11/17 5 day transfer of two blasts, 2 blasts and 2 expanding morulas frozen; 11/22 BFP!! (On FRER at 5dp5dt)
    Betas: 9dp5dt 205, 11dp5dt 497, 14dp5dt 1,709
    u/s at 5w0d- 1 sac; u/s at 6w0d 1 baby with heartbeat, another sac without a heartbeat

  • Thanks guys, I've been going back in forth between trying TI + Clomid initially vs Clomid + IUI as it really does seem like the primary problem is anovulation.  My RE thinks reasonable to try TI +Clomid x 3 cycles before IUI but I just hate to waste Clomid cycles on something that may not work.  I guess after all the hype I'm just worried about taking Clomid.  @lebradford, I think I remember you from TTGP as well!

    2008 - 2010: dragging my feet out of fear and procrastination
    October 2010: first consultation with RE, dx PCOS and fibroids (DH slightly low count/motility)
    Oct. 2010 - Dec. 2012:  In DENIAL! avoided the issue because I was scared of surgery
    January 2013: returned to RE, fibroids grew significantly
    February 2013: second HSG, fibroids pushed on tubes which blocked them somewhat
    March 2013: MRI to determine what type of surgery may be necessary

    July 29, 2013: fibroids (5) removed via robotic laparascopy
    August 2013 - Nov 2013 : benched due to recent surgery

    IUI #1, Dec. 24, 2013, BFN 
    IUI #2, Jan. 25, 2014, BFN
    IUI #3, Feb. 25, 2014  BFN
    IUI #4 canceled due to lack of response to letrozole
    IUI #4.1 April 28, 2014, BFN

    May 16, 2014: wtf consult, start prepping for IVF in June and add injects for one last IUI in the meantime
    IUI #5 started letrozole and bravelle but canceled after HSG led to new diagnosis

    May 21, 2014: third HSG, tubes blocked, one at the beginning, one hydrosalpinx??
    June 11, 2014: consult, approved to move on to IVF because the hydro is not completely blocked therefore allowing fluid to move through slowly rather than backwards
    IVF #1 August 8, 2014 - 3dt of 2 embryos, BFN
    September 17, 2014 - 4th HSG, the right tube is very patent (open!!) dye went straight through this time. Weird!
    October 2, 2014 - started metformin treatment
    November 14, 2014 - blood work, brought A1C down from 5.8 to 5.5

    November 26, 2014 - RE finally back from vaca and reviewed my chart, no more IVFs for rest of calendar year
    December 1, 2014 - Right after Thanksgiving, I called a new clinic and got in right away! Plan for IVF
    December 17, 2014 - ER! 29 retrieved (!!), 16 mature, all 16 fertilized (ICSI)

    IVF #2 December 20, 2014 - 3dt of 3 embryos, BFN

    We are done with treatment unsuccessfully. :(

  • Welcome, though I'm sorry you're here. :( Sounds like you've already been through a lot! I have the same Clomid Qs, so thanks for putting that out there because the replies of PPs have been helpful!! As far as TI vs. IUI, with the effort that you've put into trying to O, I would not want to "waste" cycles of Clomid as you say - why not increase the chances, if it's financially feasible? Perhaps your RE could give you a clear comparison of your per-cycle chances with Clomid + TI vs Clomid + IUI, given your DX, so you could do a clear cost/benefit comparison?

  • Welcome and Good luck. Sounds like you have a good plan. I have no advice for the clomid. I did do TI also instead of an IUI. We actually don't have MFI or any problems with me it's just egg quality. My Dr told me and IUI wouldn't give me any higher chances and at the cost of $600-700 per IUI I didn't complain. I also was on injections of Gonal not clomid and from my understand clomid can screw up your cm making IUI more advantageous. Sounds like you are going to be properly monitored so your Dr should be on top of everything if you do have an issue with your lining.

    We've been working on baby #1 since July 2013 using Open ID donor sperm.  8 IUI attempts with 5 actual IUIs and one chemical pregnancy.  We have one fresh IVF cycle under our belts as well as a FET.  I have endometriosis and a uterine septum that was corrected via surgery in November 2013. 

    11/14/14 -  Second HSG shows that tubes are still clear and ute is looking good. 

    12/6/14 - Started BCPs in prep for IVF #2

    12/22/14 - Saline u/s and endometrial scratch (All was clear and OUCH!)

    1/2/15 - Began stimming for IVF #2

    Welcome and sorry you're here! My RE also suggested Clomid (100mg) plus a trigger shot and TI for 2-3 cycles before we pay for the IUI. I have taken Clomid 4 cycles now (2 month break in there somewhere). I felt as though my lining was thin this past cycle...however I wasn't monitored by my ob/gyn on my 1st 2 cycles of clomid to compare it too so I'm not sure my lining was before the clomid. My RE on the other hand felt my lining was fine...

    I think everyone responds differently to Clomid and it's definitely worth a shot and totally up to you if you want to jump straight to iui! Good luck and hope you're not here long!

    Sounds like you're having some of the same thoughts, lol. We're also early 30s, been together for 10 years and have been trying for FOREVER. My challenges has always been working around not only MY work/travel schedule, but DH as well. This month is our first IUI and it feels like it's been moving mountains to get all these appointments squeezed in.

    I did clomid first too, but unfortunately my body was overstimulated & ended up with a slightly thinner lining. RE moved me to femara/letrozole and we're doing our first IUI this month.

    Welcome! Sorry for the diagnosis frustration- I would be pretty annoyed if I gained weight and drastically changed my lifestyle and it turned out neither was the cause. Boo.

    I have no advice on the Clomid situation, but you'e going to get some great, well-informed responses here.
