TTC After a Loss

XP: LH surge but no mature follies-I'm stumped ladies - help?

Hi all--This feels crazier than the Crazy Train, so I wanted to open this question up to the TTCAL and IF boards if that's alright.  I'm going through a natural FET cycle (no meds) and I finally got an LH surge today (confirmed by bloodwork yesterday) on CD 19, which is a little late for me, but still within reason.  My lining looks great, estrogen is right where it's supposed to be indicating healthy follicle and ovulation and progesterone is starting to rise - indicating actual ovulation as the corpus luteum starts to secrete it.

BUT - they have yet to find a mature follicle.  I have a few smaller ones, and one at a 12, but not any that indicate they are mature enough to release an egg.  My doctor is not worried because my hormones are doing what they are supposed to in order to support a pregnancy, my lining looks great and if progesterone doesn't rise appropriately they will supplement me.  And I guess we don't really need the egg this month since we're transferring in a blast on 5DPO, but I'm still concerned.  This has never happened before, and I know it's ok to have a weird cycle once in awhile but we only have three blasts left and I'm so scared to transfer two of them if all is not perfect.

Talk me down from the ledge?  Could this have anything to do with all the meds from IVF (in March)?

August 2012 through June 2013:  6 IUIs with clomid and trigger shot, all BFNs
7/2013, Pregnant! Unmedicated--we were on a treatment break
8/20/13 no heart beat; 8/23/13 d&c
3/15/14 IVF #1:  Lupron/Follistim/Menopur; ER 3/10 resulting in four transfer grade blastocysts, transferred one pretty blast and froze the remaining three. BFN. 
Natural FET in May cancelled because the universe hates me my hormones were not cooperating.
6/24/14 FET #1: transferred two pretty embryos; BFN   
8/28/14 FET #2: BFP, ended in Chemical pregnancy

Done with medical intervention and getting used to the idea of CFNBC.

    I wish I had an answer, but I'm scratching my head. But I will say, if your lining is great and they'll give you the p4 needed - you should be fine to transfer. My doc has said even a follie as small as 14 can ovulate...so if your surge is starting I'm guessing that 12 will show as a CL in a day or two (collapsing)

  • @LizBlue I have no advice or knowledge to offer, but I'm about to start the meds for IVF next week and I'm definitely nervous about any side effects. We are doing the procedure solely for PGD (I keep having losses due to chromosomal abnormalities). If we get any healthy embryos we are hoping for an August transfer. I hope everything goes smoothly for you from here on out! Please keep us posted. :)
  • I am also stumped. You didn't do any meds at all? I am also doing a natural FET but I will have to use Lupron and Pregnyl. I am CD 10 today and my dominant follicle was 14mm. I wish I had some answers for you, but wanted to wish you luck on the upcoming transfer.

  • I have done two FET's and always been on Lupron, estrogen patches, and progesterone. But both of my FET's ended in miscarriage. The one that worked for me was the ivf fresh cycle. .... I would say if all looks good and your doctor is confident than you should be good to go. It would seem like the meds you took for ivf could have something to do with it. My only advice though is that if something doesn't feel right to you than talk to your doc about it. I completely understand how it is to be getting down to the last embryos... I have one left and the quality isn't great so I'm super scared too. Good luck!!

    Thanks for your thoughts all.  My worry is that I'm gearing up to O with an LH surge but that I won't O because I don't have a dominant follicle and they will keep growing and then I actually will O at a later time - messing up my transfer.

    I'm going to ask my IVF nurse about that specifically.  @lindscaples  thanks for the reminder to speak up if things don't feel right.

  • I don't have any advice since I'm so new to the IVF game, but wanted to tell you that I'm thinking about you and sending positive vibes your way.  I know that you are worried about messing with the transfer and that you are on CD 19 but hopefully you're just getting a surge but your body isn't really getting ready to O.  I usually get + opk very early in my cycles (CD 7-9) but I don't typically O until CD 12-13 so maybe that's what is happening?

    2nd and last IVF cycle August 2015, stimmed for 12 days, 2 egg retrieved, both mature and both fertilized.  Transferred both 8-cell embryos on Day 3, beta 9/5/15 = BFFN
    MOVING TO ADOPTION!

    I just thought of something. The reason I was switched to a freeze all instead of fresh trnsfer is my p4 was above 1 at date of ER. You say your progesterone is going up, I'd ask them if it's above one before ovulation if they want to hold off and do a medicated FET next cycle. I know it's not what you want to hear but something that popped in my head after reading other responses.

    @Frenchteachermama - yes - this is my fear.  That i'm having my surge now but won't actually O for another few days.  They are planning on doing the transfer in five days from today, saying that I O'd already. 

    @katib77 I will happily hold off for another month if they tell me to do that.  I'd almost rather do that because I'm so nervous something doesn't feel right.

    My nurse calls every day at 3pm with the results of my b/w so I will definitely have more questions for her then.

    Thanks for the hugs and support all. My estrogen plummeted and RE has no idea what's going on. They cancelled my cycle and I'm pretty sad and frustrated about it.

    He wants to do a programmed FET next cycle and I agree that's best. I know this is still a good option but today I just feel mad that nothing's ever freaking easy.

    In better news: hubby and I got a little tipsy last night and had very fun non-procreative sex. :)

  • Oh no! I'm so sorry your cycle got cancelled!! :-/ So frustrating! Glad you got to enjoy some time with the hubby though! ;)
