Who does your IUI? UPDATE

When we completed our IUI #1 it was our nurse that completed the actual IUI. I don't know her specific credentials, I know that they have many different types of nurses and PAs that assist the RE's as its a large center. But I was just curious who does your IUI, is it your specific RE or a someone else in the office. I am prepping for IUI #2 tomorrow and wondering who might do it tomorrow.

UPDATE:
So today was a bit hellish and I still don't know what to think of it! We got to the center, my husband did his thing and we had about 2 1/2 hours to kill so we got breakfast and while we were eating we got a call from my nurse saying that his count was very low, about a million (compared to 7.5 million in Feb IUI #1) and they asked for him to come back and do another sample. Well let's just say he completely went from the world's most optimistic person to absolutely devastated in a matter of a minute. 

So we went back, he did his thing and I had an u/s. The RE wasn't 100% sure if I had just ovulated or not, I only had one follicle 17mm on the right side. Another hour later they took me in for the IUI and said that with even his second sample it didn't help. HOWEVER, I told this nurse that I needed to have an ounce of good news because my husband has been trying to jump through hoops to make his numbers better. She said that although his count was low, his motility went up to above average which was previously below average in Feb from 35% to 56% woohoo!! Well I took that good news and ran with it! I really tried to make him feel better about all the positive choices that he made in the past 5 months! 

So the nurse said that they were waiting for an RE to do the IUI, I thought that was odd, but she said that the RE that did the u/s wanted to do the IUI! Ok, fine by me! The RE stated that once they get the blood work back they will know if I ovulated or not, so I wait by the phone. If I didn't ovulate they might have us come in for another IUI tomorrow or Sunday. I just hoping and praying that this all makes sense to us one day! 

Thanks to everyone that shared their IUI experiences, I have to say I felt 100%%%% better going in today and thinking a nurse would be preforming the IUI. Kinda just wanted to make sure I wasn't the only person to have a nurse do it!

DH met with Urologist nothing is "wrong" just very low count and low mobility

RE Appointment on 1/15/14-Decided on 1st IUI

IUI Cycle #1-Clomid 50mg 2/14-BFN

We decided to take a break for 3/14-4/14

IUI Cycle #2-Clomid 50 mg 5/9-BFN

IUI Cycle #3-(RE Office called us in for a back to back IUI for another SA) 5/12-BFN

*Husband's count is still very low post wash however after 4+ months on supplements, vitamins, and a healthy lifestyle (aka no beer)  his mobility has doubled! I am very happy for him, small victories!

May 2014-RE wants us to progress to IVF as 3 failed IUI's.

IVF Seminar meeting on 6/12 & then meeting with RE on 7/9 to come up with our plan for our miracle baby!

  • At my clinic, whichever RE is on for IUIs that day does it (someone is on 365 days a year). So sometimes it's my RE or otherwise it's one of the other 3 REs at the clinic. REs also do all of the ultrasounds. The only time I meet with a nurse is after my appointment to discuss the plan and give me prescriptions for meds and labs. I really love how hands on all the REs are at my clinic.

    Me (35) no known issues DH (37) MFI. TTC 21 months (24 cycles)
    Dx MFI with low to normal count, low motility, morphology 3%
    HSG normal, ultrasound and labs on me all normal. 

    1 cycle of Clomid 50mg and TI, unmonitored by OBGYN= BFN
    1 cycle of Clomid 50mg and IUI, unmonitored by OBGYN= BFN
    Started seeing an RE!!
    2 more cycles of clomid 50mg (great response), with IUI and Pregnyl trigger (4.8-8 mil good ones after wash) = all BFN
    1 cycle of clomid 50mg (3-7) followed by Follistim 75iu (7-11) + IUI = BFN
    December 2013 DH saw urologist and is taking Anastrozole, CoQ10, and L-Carnitine
    IUI #5 natural cycle (needed a med break) = BFN
    IUI #6 Follistim 75iu (CD3-10) + Pregnyl (CD11) + IUI (final count after wash 300K) = BFN
    IUI #7 Follistim 75iu (CD3-9) + Pregnyl (CD11) + IUI on 2/20 (post wash count 12.5 million)= BFN
    IUI cancelled (DH OOT) Clomid 50mg (CD3-7) 1 follicle +(not well timed) TI = BFN
    IUI #8 and last one!! Clomid 100mg (CD3-7) +OPK before US + IUI 4/17 (post wash 8.5 mil)= BFN

    Pre-IVF testing complete! SHG great and measurements taken! Labs for infectious diseases completed, FSH (5.4), TSH (1.6), Prolactin (11), AMH (2.6), Estradiol (40).

    Started BCP 5/29 and Lupron 6/11 prep for IVF #1! Started follistim 225u/day on 6/28. Monitoring on 7/2 >15 follicles measuring 11-14, E2 758. Monitoring on 7/5 all ready to go!! Great follicle sizes and lining is at 9. Tigger 7/5, ER 7/7 16R 9M 3F. Stimmed too fast in just 7 days. 7/10 3dt of 2 8-cell grade 2 & 4. 7/14 P4 >60.

    Beta #1 (14dpo) 7/21 112 Beta #2 (16dpo) 7/23 286 a Beta #3 (18dpo) 7/25 761 Beta #4 (21dpo) 2631!!! Hold on tight little embies!!
      First Ultrasound 8/7- 1 perfect little bean with a beating heart 117bpm!! EDD 3/30.
    Second ultrasound 9/2 Little bean measuring a few days ahead with a heart rate of 161!

    We've been working on baby #1 since July 2013 using Open ID donor sperm.  8 IUI attempts with 5 actual IUIs and one chemical pregnancy.  We have one fresh IVF cycle under our belts as well as a FET.  I have endometriosis and a uterine septum that was corrected via surgery in November 2013. 

    11/14/14 -  Second HSG shows that tubes are still clear and ute is looking good. 

    12/6/14 - Started BCPs in prep for IVF #2

    12/22/14 - Saline u/s and endometrial scratch (All was clear and OUCH!)

    1/2/15 - Began stimming for IVF #2

    I've had a nurse do one of my IUIs, and the RE did the other. At my clinic I think it must just depend on who's available. I haven't had many ultrasounds yet, but it's always been the RE that's done those.
    2.5 years TTC with MFI, 3 failed IUIs 
    IVF w/ICSI October 2014: 17R, 13M, 12F 4 Frosties

  • The RE's do all the IUIs where I'm at and it just depends which doctor is in that office for the day and assigned for that day. The nurses do all the monitoring appts.

    Me: 32  DH: 33  Married: March 2004

    July 2006: started TTC
    2008: HSG (normal), couple rounds of clomid through gyno
    2008 - 2010: dragging my feet out of fear and procrastination
    October 2010: first consultation with RE, dx PCOS and fibroids (DH slightly low count/motility)
    Oct. 2010 - Dec. 2012:  In DENIAL! avoided the issue because I was scared of surgery
    January 2013: returned to RE, fibroids grew significantly
    February 2013: second HSG, fibroids pushed on tubes which blocked them somewhat
    March 2013: MRI to determine what type of surgery may be necessary

    July 29, 2013: fibroids (5) removed via robotic laparascopy
    August 2013 - Nov 2013 : benched due to recent surgery

    IUI #1, Dec. 24, 2013, BFN 
    IUI #2, Jan. 25, 2014, BFN
    IUI #3, Feb. 25, 2014  BFN
    IUI #4 canceled due to lack of response to letrozole
    IUI #4.1 April 28, 2014, BFN

    May 16, 2014: wtf consult, start prepping for IVF in June and add injects for one last IUI in the meantime
    IUI #5 started letrozole and bravelle but canceled after HSG led to new diagnosis

    May 21, 2014: third HSG, tubes blocked, one at the beginning, one hydrosalpinx??
    June 11, 2014: consult, approved to move on to IVF because the hydro is not completely blocked therefore allowing fluid to move through slowly rather than backwards
    IVF #1 August 8, 2014 - 3dt of 2 embryos, BFN
    September 17, 2014 - 4th HSG, the right tube is very patent (open!!) dye went straight through this time. Weird!
    October 2, 2014 - started metformin treatment
    November 14, 2014 - blood work, brought A1C down from 5.8 to 5.5

    November 26, 2014 - RE finally back from vaca and reviewed my chart, no more IVFs for rest of calendar year
    December 1, 2014 - Right after Thanksgiving, I called a new clinic and got in right away! Plan for IVF
    December 17, 2014 - ER! 29 retrieved (!!), 16 mature, all 16 fertilized (ICSI)

    IVF #2 December 20, 2014 - 3dt of 3 embryos, BFN

    We are done with treatment unsuccessfully. :(

    All five of my IUIs were done by nurses, only my shg and IVF have been done by RE. All ultrasounds have been done by a tech and honestly she's one I trust just as much as RE as she's been there for 20 years and has so much knowledge.

    TTC since July 2009. Dx MFI & LPD.

    IUI#4 1/23/13 on 75iu x9 Follistim = BFP then chem preg m/c (Feb 2013)

  • Thanks everyone!! It is good to hear that it is a healthy mix between RE & nurses doing the IUI's. Up until our first IUI we were always with our RE for the HSG & SIS. When it comes to monitoring & u/s prior to the IUI its whatever RE is on that rotation I think, there are 5 RE's within this center. After my monitoring I typically meet with our nurse who is assigned to our RE for the next step and then any follow up calls she typically makes to us re: bloodwork etc. I know that our own RE will preform the IVF's with a separate whole team of IVF nurses. 

    I just never thought to ask who would be doing the IUI. I guess I assumed it would be a RE since it was a procedure so when the nurse did it I was a bit surprised but didn't want to say anything and be insensitive or insult anyone. 

    Thanks for sharing, I feel a bit more relieved I should get some sleep we have to leave the house in 6 hours to make our way to the hospital for the IUI!

  • I've only had one, but our nurse did it. I was shocked at first but really like her and felt comfortable with it

    4 Losses (2003, 2008, Apr 2012, & Oct 2012)
    All RPL and IF testing with multiple REs = normal

    5 IUIs = BFN

  • Thanks for asking this question - I am now anxious to find out what they will do at our office. The HSG was painful and long, and the RE wasn't even able to get the self-guided catheter in (he shot dye from outside the cervix) - and he's an innovative and award-winning laparascopic surgeon, and I was already nervous about the IUIs, and couldn't even imagine a nurse trying to do that!!

    Did it go smoothly with the nurse at IUI #1? If you weren't comfortable with that, do you think the office would honor your request?

  • All of my ultrasounds and all 4 of my IUIs have been done by my RE. The office I go to only has 1 RE, so I see the same doctor every time.

    All my iuis were done by one of the REs in the office (mine isn't there everyday as they rotate between offices).

    ETA the REs also do all the ultrasounds.

  • Mine has always been done by one of the RE's.  If it's in the normal office I go to during the week, it's MY RE, otherwise I have to go to their main office and it's one of the other RE's
