Meeting with a pediatric urologist

All along throughout out my pregnancy the doctors have been keeping an eye on DS2 left kidney for pelviectasis/pyelectasis. Which basically it's retaining fluid instead of releasing it. They would like it at 7mm or below at this point during my pregnancy but DS2 is at 10mm so we are going to meet with the urologist today to find out the plan. From all that I've read and talked to the various doctors they say it will most like resolve itself or he may need antibiotics but I'm still nervous it will be nice to hear what the urologist has to say.

  • If it makes you feel any better, we're in the same boat. You're not alone! At our 34 wk growth scan they noticed Chase's bladder was extremely full and both kidneys were dialated in the abnormal range. I think the left was at a 10 as well and the right was an 8/9. We saw a pediatric urologist two weeks later and it really helped to get more info, learn about potential causes/issues, and have a game plan for after birth. It sounds like this issue is more prevalent in boys and treatments are effective since they can diagnose the root causes so early.

    At a follow up growth scan at 38 wks his kidneys were looking better but still in the abnormal range of 8/9. The game plan for us is oral antibiotics after birth. Circumcision to cut down on other potential issues (we were planning on it already though). Provided he urinates within 24 hrs after birth, we'll set up another visit with the pediatric urologist for an ultrasound on Chase's kidneys for 2wks post birth. It's easier to see what's going on without having to do the u/s through mom. If he doesn't urinate within 24 hrs, the u/s will happen right away. Depending on the findings of the u/s, further treatment will be decided at that point.

    I know how hard it is to find out that your LO could/does have issues. I've gone through the gammate of emotions already, but decided that it does me no good to stress about it now since I can't do anything at this point. I'm sure I'll be a nervous wreck about it after birth though! The main thing is that we have all of our doctors on the same page and aware of the situation, so that does help with my peace of mind. If you don't have a pediatrician yet, I'd highly suggest finding one and scheduling a meet & greet. They'll want to have access to your OBs records, U/S, and the pediatric urologists notes. Also make sure each office has the names/addresses of all involved so that any follow up notes are sent to everyone.

    Keep your head up momma! I'll be praying that our LOs have an good/uninvasive resolution to this....and we don't go crazy with worry either! PM me if you need to chat or want to know the outcome on our end. We've got 1 wk before EDD.

    Thanks! We have a pedi since we have an older DS. I actually went through her to get the referral got the urologist. She will be out of town so she already sent out a report to the other doctors in the practice so they have a heads up. It stinks that it is a lot of wait and see right now.

  • Update: the urologist felt that there should be little to be concerned about. He said to expect an ultrasound post birth and antibiotics. The pedi will contact the urologist with the results and he will determine when we would need follow up ultrasounds. He felt that it should probably clear up on its own.

  • We had that with the first DS. We did antibiotics and a VCGU (? correct acronym is escaping me, to check to see if there was reflux) when he 6 weeks old. once it was determined he didn't have reflux, so no antibiotics and just a check by U/S of his kidney every year. He just got cleared a 5 years old. It never really resolved itself like most cases, but it never got worse. At 5, he has enough other kidney tissue that it doesn't matter any more.
  • Just following up on this post. DS' ultrasound after he was born 6 weeks ago was clear and he never was give antibiotics and has been having wet diapers. However we had a 6 week follow up ultrasound today and he still has the fluid around the kidney. So we are back to urologist next week who will review the ultrasound results. DS is a level 2 which is on the mild side which is good. I was hoping the ultrasound would be clear since it was after he was born but I guess he was dehydrated.

  • This happened to DS as well. When he was 6 weeks he had a VCGU and when that was clear, he got a renal scan at 7 weeks. Both were clear so there is no blockage or reflux, he's just built that way!
