SA Questions. (Fuun...)

Hello! 

I am new here! My husband and I have been TTC for almost a year. I was diagnosed with PCOS when I was around 18 and always knew it would be a struggle to get pregnant. We are just starting our journey working with an amazing RE and are ready to face everything head on. 

I just started my first round of Clomid on Monday after taking Provera to start my cycle. I have also been taking Metformin for almost a year.

We just got a call with the SA results, our doctor hasn't reviewed the results yet, but I was just curious about the results. Googling hasn't been much help. 

Here is what the nurse told me:
Progressive Motility Count - 72 million (normal 30 million)
Motility - 37% (normal 40%)
Morphology - 
Head - 21% (normal higher than 55%)
Tapered - 50% (normal is lower than 25%)
Tail - 60% (normal is higher than 65%)

So, from what I can see the real issue here has to do with the morphology. How concerning is Morphology?

  • These look like they are the pre wash count. They wash the sample to leave only the best sperm for IVF.

    As far as pre wash numbers go they look good. Morph is under what it should be but still very good in comparison to ours (pre wash count for us morph is at 0).

    The motility is just under too but I wouldn't fret to much over any of this.

  • I don't have any insight into the results but I wanted to share that we're in the same boat.  DH had a 2nd SA and his numbers are almost identical to yours.  (The first was normal.)  I have heard that morphology isn't the biggest concern and also that it is the biggest concern.  It's so confusing and I can't wait to talk to the urologist on Monday.  Do you have an appt. with a urologist scheduled yet?  I'll try to remember to post a summary of our appointment here on Monday for you.  Feel free to PM me if I forget!

  • I don't remember what my husband's exact numbers were, but his morphology numbers were low.  He went to a urologist who ordered 2 more SA's before his first appointment.  My husband had varicoceles on both sides (it's most common to have one on the left only).  Varicoceles can be corrected with a minor surgery.  From the research I've read, doing the surgery doesn't significantly improve pregnancy rates.  But, they can be uncomfortable and the urologist recommended the surgery, so my husband had the surgery done in December.  It takes 3 months for sperm morphology to improve after surgery though, :(.  We haven't done a follow up SA yet, my RE doesn't seem too worried about it.

    TTC since May 2012, married since 7-08-06
    Me: irregular cycles, possible endo
    DH: bilateral varicoceles (surgically corrected on 12-13-13)
    Treatment: 3 cycles of Clomid alone, 2 cycles with Clomid and IUI
    HSG Feb. 14:  all clear
    1st appointment with RE 3-20-14
    2nd appt with RE: normal postcoital test, u/s showed a large follicle the day after I got a positive OPK, most likely problem is endo :(
    2nd HSG May 14: My RE used a dye that is supposed to help calm down inflammation from endo

  • Thank you all so much for your responses! There is so much to learn in the world of fertility. 

    princesskatie22 - I know what you mean about trying to research it. In my look around today I saw things ranging from it doesn't matter to you will not be able to get pregnant without IVF. Good luck to you and your husband with your urology appt!

    Hopefully our RE will get back to us soon with her thoughts on all this. But, a urology appointment might be a good idea!

  • Hs doctor says he can not accurately say how DHs sperm really are without doing at least two SAs. This is because his morph was that bad (along with possible white cells in the sample) and there are so many factors to what can cause things. DH is on meds for a UTI as a preemptive measure because that could cause some of what they saw. His next SA is mid may.

    If you really are concerned (or not satisfied with the answers you get from your doctor) go see a urologist. It will not hurt and can only help you become more informed.
