Low AMH level of 6

I have been seeing an RE for a couple months now. My DH (35 years) is all clear with "excellent sperm". As for me (31 years) it seems every time I have blood work and ultrasound things are not working the way they should. Right now I am currently on Puregon only. Today I went in for another ultrasound and blood work feeling quite good. I have lost 8 pounds and was feeling very optimistic. Then the doctor came in to have a talk with me. He told me that my AMH results were in and I was reading at 6. He said ideally a woman my age should be around 15 to 20. I now have an appointment with him but not until May 9th to review our options. He said for me not to worry but that is harder said than done. Needless to say I have been crying all day and I am scared… 

Does anyone have low AMH? What options will we have? Does anyone have anyone advice?

  • What is your FSH.  If that is normal, it may not be quite as much to worry about...but it sounds like your RE uses a different metric system, b/c I'm used to seeing the average or ideal number being above 1.  

    I asked some questions about AMH on the Infertility board and some people recommend taking DHEA or D3 supplements.  They won't work miracles, since AMH is hard to change, but they have been known to help some.

    Either way, from what I gather, a very low AMH doesn't mean some dire situation for you...but you just may need higher dosages of meds to produce more follicles.  I'm no expert on it, but that's what I gather so far.  

    The good news is your RE seems to be on top of it and will work on some options for different protocols for you.  Good luck!

  • I'm pretty sure my RE said they consider a good AMH to be in the 1-2 range, so I would have thought 6 was quite good. I'd check in with your dr. again about what a 6 actually means.

  • Yeah I not too sure but he told me it is a concern as I should be around 15-20. I am not too sure what my FSH levels are. Puregon, the injection that I am is a FSH injection. My RE put me on puregon because my follicules were not as large as they would have liked.

  • I agree with PP that it depends on how your AMH is being measured, ng/ml or pmol/l. These give totally different scales. The pmol/ml the ranges are higher numbers, the ng/ml ranges are 1-3 for normal. I was confused about this too. The normal values seem to depend on how they are measuring it.

    Me (35) no known issues DH (37) MFI. TTC 21 months (24 cycles)
    Dx MFI with low to normal count, low motility, morphology 3%
    HSG normal, ultrasound and labs on me all normal. 

    1 cycle of Clomid 50mg and TI, unmonitored by OBGYN= BFN
    1 cycle of Clomid 50mg and IUI, unmonitored by OBGYN= BFN
    Started seeing an RE!!
    2 more cycles of clomid 50mg (great response), with IUI and Pregnyl trigger (4.8-8 mil good ones after wash) = all BFN
    1 cycle of clomid 50mg (3-7) followed by Follistim 75iu (7-11) + IUI = BFN
    December 2013 DH saw urologist and is taking Anastrozole, CoQ10, and L-Carnitine
    IUI #5 natural cycle (needed a med break) = BFN
    IUI #6 Follistim 75iu (CD3-10) + Pregnyl (CD11) + IUI (final count after wash 300K) = BFN
    IUI #7 Follistim 75iu (CD3-9) + Pregnyl (CD11) + IUI on 2/20 (post wash count 12.5 million)= BFN
    IUI cancelled (DH OOT) Clomid 50mg (CD3-7) 1 follicle +(not well timed) TI = BFN
    IUI #8 and last one!! Clomid 100mg (CD3-7) +OPK before US + IUI 4/17 (post wash 8.5 mil)= BFN

    Pre-IVF testing complete! SHG great and measurements taken! Labs for infectious diseases completed, FSH (5.4), TSH (1.6), Prolactin (11), AMH (2.6), Estradiol (40).

    Started BCP 5/29 and Lupron 6/11 prep for IVF #1! Started follistim 225u/day on 6/28. Monitoring on 7/2 >15 follicles measuring 11-14, E2 758. Monitoring on 7/5 all ready to go!! Great follicle sizes and lining is at 9. Tigger 7/5, ER 7/7 16R 9M 3F. Stimmed too fast in just 7 days. 7/10 3dt of 2 8-cell grade 2 & 4. 7/14 P4 >60.
    Holy crap BFP!!!
    Beta #1 (14dpo) 7/21 112 Beta #2 (16dpo) 7/23 286 a Beta #3 (18dpo) 7/25 761 Beta #4 (21dpo) 2631!!! Hold on tight little embies!!

  • I am so sorry, but I hope it all works out for the best.  I know waiting until your appointment to find out what is going on is killer! I've been there.  I don't think doctors understand the agony you go through just waiting on an appointment!  Congratulations on you weight loss and working on your health goals.  Working on my health goals through diet, exercise, and especially Yoga is what helps me out the most when I'm stressing about hearing results.

  • I think there are a couple different scales used based on the lab. I had a 0.82 which is considered low for my age of 32. First RE was all, "we will do one IUI then move to IVF!" But when I took the 150mg Clomid I produced like 4 mature follicles on day 12 (we triggered + IUI but nothing) so he calmed down a bit and said we can do 3 cycles before moving on. He said at that point we "need more control" in which case IVF is obviously total control. I saw a second opinion (I shop around buying a car so why not when 15,000 dollars is at steak?) and he was less concerned but wants me to do a Femara/Follistim cycle with trigger and IUI. Bigger chance of multiples but I'm sort of at the point where I'll take it. Besides, as long as I'm safe- I don't know any better. I'm currently in the TWW for this second cycle. Fingers and toes crossed!
  • If it makes you feel any better the refund program my clinic does requires 1.2 or higher to participate. Sounds like your number is pretty good when you compare it to that. :)

    I was wondering about this too, because there seems to be a wide range of normal values. My AMH is 10, which my doctor was happy with. He said it was a bit high but he isn't concerned. I am ovulating and have no other indications of PCOS (bloodwork/ultrasounds). Of course I have been trying to research AMH levels, which just makes me worry more. I found this website: https://www.rmanj.com/2013/09/anti-mullerian-hormone-amh-testing-of-ovarian-reserve/ Which was helpful in explaining why there seems to be such variation between normal values. It states, "There are some inconsistencies to consider when interpreting AMH hormone levels. Since the AMH test has not been in routine use for many years, the levels considered to be “normal” are not yet clarified and agreed on by the experts. Also, not all current commercial assays give equivalent results." With that said, it may be hard for us to give you adequate feedback since we might be using different reference ranges. I usually ask for a print out of my tests results and keep them in a small file. It helps me keep information available when I need to reference it. I would wait until you see your RE again. Write down some questions and bring them with you. It sounds like your RE is working on options that are individualized to your needs. Good luck!

    I go to RMA. :). My AMH came back at 12 and my RE was not concerned. I don't have PCOS either and I am 35, so I was surprised it came back at 12. OP make sure you find out how your lab measured your amh.

  • Thank you so much ladies. I had another visit for blood work and ultrasound and found out the my FSH is 7.4 which they say is normal and my AMH is 6.4. I am trying to pick myself back up again and stay positive.
