IUI Question question

Some of you know me around her, I've been through 2 IVF's,  I wanted to know if it was okay if I ask an IUI question here, because as weird and backwards as it sounds we are doing that next.. I'll explain if its okay to ask here. :) 
Me 39, DH 40
Male Factor (severe oligospermia) 
IVF#1-December 2013: Conversion Protocol @ SIRM, Canceled on day 10 of stims due to poor response
IVF#2- February 2014:  Estrogen Priming Conversion Protocol @ SIRM (ER 2/22: (9R, 7M, 4F, All arrested, nothing to trasnfer)
IUI#1-May 28, 2014- 100mg clomid, HCG Trigger, 100mg progesterone oral--BFN
IUI#2-June 24, 2014- non medicated, HCG 2,500mg x 4 doses (over a week), Endometrium- BFN
Suprise BFP- September 1, 2014- miscarriage at 5 weeks.

  • So the question first.  Is it common for a nurse to do the sperm wash and insemination in an IUI when a doctor is not in the office?

    Explanation: trying to schedule first IUI and the nurse called me today and we were talking and I said I would be out of town a weekend at the end of the month and she said it was okay because they didn't usually do IUI on weekends anyway and if they did that the nurse (RN) would come in and do it, not the doctor.  Furthermore it bothered me that they don't plan for weekend because it seems like they are more concerned about their schedule rather than maximizing my chances of success with optimal timing.

  • The nurse at my clinic did 5 of our 6 IUIs, so that is pretty standard in our clinic. None of our IUIs were successful, but I do not think the nurse doing it instead of the doctor had anything to do with it.

    Married 4/2010, TTC since 7/2011

    Dx: Officially Unexplained (I have Polycystic Ovaries diagnosed via ultrasound, but few classic PCOS symptoms, he has mild MF issues. So... not issue free, but nothing so severe as to explain IF)

    I also deal with post-surgical Hypothyroidism following Thyroid Cancer in 2009, but under control with Levothyroxine

    4 months Clomid (thinned lining) and 10 months Letrozole (every indication that I responded perfectly)

    6 failed IUIs in 2013, 3 with trigger

    IVF #1 in March 2014

    ER 3/21/14, 31R/21F, 12 frosties!

    ET 3/26/14, 1 perfect blast transferred: BFN

    FET#1 5/28/14, 2 "beautiful" early blasts transferred. BFP!!

    Beta #1 (6/11/14) 798; Beta #2 (6/18/14) 7,966.

    1st u/s (6/25/14) showed 2 sacs, 1 empty & 1 with a beautiful little bean doing what it needs to do!

    EDD 2/14/15, missed miscarriage, DX: Trisomy 21. D&C 8/1/14

    FET#2 Transferred 3 embies, 2 looking pretty good, one not so much. BFN.

    IVF#2 January 2015, tentative ER 1/23

  • The nurse practitioner did 3 of my IUI's and the doctor only did 1. Hope you have good luck with your IUI :)

  • My doctor did not do any of my 3 failed IUI's, but I don't think that's the reason they failed...best of luck you you :)




  • I think the best way to do an IUI is with a trigger and meds..I had 5 that were unmedicated..totally frustrating...good luck!!
  • My RE did all my IUIs. Of course, one particularly early morning, he couldn't seem to find my ovaries (either one!) on ultrasound, so maybe I would have been better off with the nurse! (J/k)

    IVF w/ ICSI cycle #1 
    9 days stimming with Gonal-F, Menopur, Cetrotide
    ER on 4/21 - 19 eggs retrieved, 12 mature, 8 fertilized normally
    3 embryos survived to blastocyst
    2 hatching blastocysts transferred on 4/26
    Faint BFP on 6dp5dt and getting darker each day
    Beta #1 (11dp5dt) = 292
    Beta #2 (18dp5dt) = 6,154
    Ultrasound at 6w2d reveals a single baby with a heartbeat of 125 bpm! EDD 01/12/2015
    Ultrasound at 8w0d: heart rate 164 bpm
    Ultrasound at 9w4d: heart rate 185 bpm
    Panorama tests returned in 10 calendar days. Low risk for everything!
    It's a boy!!!

  • I will jump the thread to also ask an IUI question... What made you keep on going with multiples IUI instead of just pulling the trigger to IVF then and there?

    I just did my first IUI. It failed. It was my second attempt at it. First go around I overstimmed and we converted to IVF with cryoocyte (my clinic was running a trial to see if they could succeed at a cryoocyte IVF so they froze my eggs before IVF) that ended on a c/p. after RE said I only have a 7% chance with IUI and our best bet is IVF but we can try IUI if we want to. I don't feel it's worth going OOP again for 7% chance. Am I wrong? Should I risk another IUI ?

    The first time around, we started with Clomid because at that time, we only knew about my inconsistent ovulation. In the workers, we quickly learned there was a motility issue also. However, the other counts were so good that our RE said IUI was a reasonable option to try. He cautioned us that in our case, not to get stuck in it. He estimated that 3 IUIs would be the point if diminishing returns. Statistically, after that point, our cumulative odds of success weren't going to get better, and that would be the time to move on to IVF. On our third IUI, I had Zero hope of success and just mentally had moved on. I had already set up our appointment for our IVF protocol and baseline when I got my BFP. This time around, his numbers were worse on morphology and the same on motility. Combined with the fact that I am not as young as before (duh), it seems like IUI really did t give us good odds for success.

    I can answer the "reverse" way if that helps.

    I started with IUI but I had such a HUMUNGOUS response on such little medication, my doctor canceled the trial and appealed to my insurance company to go "straight" to IVF because a person who overresponds to medication and produces too many follicles is a bad candidate for IUI because it leads to multiples. For example, I was on less than 150 of follistim a day (kept decreasing it) and I had 20+ follicles that were mature. If it wasn't canceled, I could have seriously ended up like the octomom

    TX: IUI #1 with Follistim. Canceled due to too many follicles (10 + that were large); BENCHED
    IVF #1 March 6. 14 eggs retrieved, 10 fertilized. 
    March 11, 4 eggs transferred.
    March 21 BFP  :) Beta #1 457, Beta #2 1350, Beta #3 9619.
