Pregnant after IF

Possible SCH

So since my heavy bleeding and clots yesterday I have had a lot I time to play Dr. Google so if I'm way off base just talk me down from RE google ledge :)

The RE I saw yesterday isn't my RE (mine is on vacation) and he stopped short of calling this an SCH. He explained that sometimes the placenta detaches some and bleeding and clots can form. From what I'm reading this is exactly what an SCH is.... So why wouldn't this RE just label it? I realize on my u/s from last week (I started spotting) and then from yesterday doesn't show an SCH according to both RE's. I am stumped as to any other explanation for this bleeding and desperately want answers.

Someone speak SCH to me please.

Re: Possible SCH

  • SCHs can sound very unsettling. I was completely caught off guard when my RE found 3 of them at my 6 week ultrasound. Due to their location, in my situation, I was told that if I started bleeding, it would not be good, but most SCHs need to bleed in order to heal. 
    The way my RE explained it to me is that when the embryo attaches to the uterus, it burrows deep into the tissue and can cause separation between the layers. The separation can fill with a little bit of blood, so on an ultrasound, it appears (to my novice eyes) as another sac in some ways.She said to think of it like a uterine bruise. If the SCH is close to the cervix, it will drain. Mine were behind the sac, so it had a bit of a journey to get out. I didn't end up bleeding until about 11 or 12 weeks. I was put on 2 weeks of bed rest until the bleeds shrunk and the baby got bigger. I was a nervous wreck, but here I am at 37 weeks!
    I know they are scary, especially since you've bled quite a bit..I'm so sorry you've had to deal with this. But they seem to be pretty common and hopefully your RE can give you some peace of mind!

  • Did they give you a pic of the ultrasound? It was very easy to see the SCH in mine. It freaked me out because it was much larger than the ones I compared to on Google, but cleared up quickly. If you have a pic, perhaps it can give you some info.

  • I have the pic which doesn't show an SCH according to the RE. I was reading though that if there isn't pooling around the sac then the blood would just mirror the rest of the uterine lining.

    I also read that often when there is a small bleed (I've been spotting for a week) it can become a BIG BLEED triggered by a violent episode of vomiting. I woke Sunday morning around 3 and violently vomited for 4 hours before I bled.

    I didn't mention this to the RE because I didn't learn about this until this morning. Maybe I'm grasping at states for answers but I won't get to see my RE until Wednesday.,... So needless to say I'm just speculating.

  • I was released from my RE on a Thursday (about 10 weeks). That Monday, I had my first OB app't (ultrasound). That night I started bleeding. I went in the next day and was d/x with a SCH (visible on the U/S). They gave me modified bedrest. I ended up taking the week off as precautionary. That OB said one of three things will happen- I will lose the baby, I will have a bleed out, or the SCH will absorb. For me, it was the latter of the three. The bleeding was gone by 12.5 weeks. It is very scary.

  • When I was pregnant first time out I had a SCH (after IVF w/ISCI). Very terrifying. It started with I passed a lot of blood, (went through my underwear & trousers before I knew I was bleeding) passed clots too. When it happened I called the RE's office & the nurse said unless I was passing so much blood that it was soaking more than a pad an hour that I wasn't in any danger & that if it was a miscarriage there would be nothing that the emergency room could do for me so go home & go on bed rest & try to stay as still as possible.  They weren't able to get me in for an ultrasound the following day due to being backed up with ET or ER, can't remember which, so I had to wait until the day after that to get it. Longest almost 48 hours of my life. When we went to the ultrasound we showed twins but one of which he didn't think would make it & it eventually didn't. But that was completely unrelated to the SCH. But at that point seeing that there was still the one going strong was such a relief as we were so sure it was over, even had the clots/tissue with us thinking they would be able to preform tests on it thinking it was a miscarriage. So in my experience the SCH was very very traumatizing but we now have my little boy. I was put on pelvic rest until 12 weeks, told no lifting above 10lbs & no being on my feet/walking for more than about 10/15 minutes at a time.

    I know how scary it is and what you are going through but hang in there. IF does weird things to the brain and the heartache and emotional toll of struggling with getting pregnant means we are stating out our pregnancies from a different place than a simple sex baby.  try as much as possible not to freak out to much, there is nothing you can do that will change the outcome so try not to stress. I've known a few people IRL that have had SCH and been fine too. Not trying to diminish your fears at all but try to stay as positive as possible. Hugs, hang in there!!!!

    I took my huge clots into my RE's office too! I don't understand with that much blood and passing of large clots how on earth I was still or am still pregnant.

  • I have bleed 4 times during my pregnancy so far and only one of their was diagnosed as a sch. The others were over fast enough that by the time they god the ultra sound there was nothing. My ob/re explained that it was probably a sch but they couldn't be sure without seeing it. Perhaps your re didn't label it for hat reason.

    Vomiting, straining from pooping, sneezing or coughing can all cause a sch from what they told me, as can over doing it physically.

    Try to relax and stay away from dr google :)
  • FWIW, my RE had a very good ultrasound machine and my OB eh not so much. You can't see as much detail. When I was at the RE and MFM he could see and measure the SCH, but the OBs offices could barely see it. My OBs were always hesitant to label it an SCH, they just knew I had one from my RE.

    Also, if it were an SCH passing the clots would make it smaller and probably harder to see. I always thought of an SCH like a blister, if the fluid inside is released the 'pocket' is going to be smaller since there's nothing in there to fill it up!

    12-2 BFN :(

    3-1-14 moving on to the next step. Switched RE. going to plan 3 IUI cycles while saving for IVF#2

    April 2014- IUI#1  BFN
    May 2014- IUI#2 BFN
    June 2014 IUI#3 BFN

    Taking a break for a while, focusing on our wedding.

    Sept 24th missed period! Surprise natural BFP
    10-15-14 first ultrasound
    06-05-15 EDD

    I'm doing ok. The bleeding has reduced to spotting again and my MS is in effect big time so I'm hoping both are good signs! I'm very nervous about my U/S tomorrow..... Actually I'm panicking over it.
