Confused by SA

So, we had IUI #4 this morning. We have had this weird decreasing SA problem so we were prepared for low numbers again today. The RE said they are "better, but still low" - at 3 mil. Where I'm confused, though, is that he said "but, because of the volume, we are able to wash it down to 20 mil 40% motile." Now, 20 mil with 40% sounds much better than 3 mil, but I'm confused. I know it has something to do with per mL, but the math doesn't add up to me?! Clearly, though, it was still low and we have our WTF appt scheduled for Monday "just in case."

    20 million with 40% motility sounds more like a pre wash count, 3 million maybe post wash? We have never had a post wash count with such low motility because the bad swimmers are usually washed out. The post wash motility should be higher. I think the lowest we have ever had was like 75% after wash. I would definitely ask for clarification, if the 3 million is post wash you might want to think about having your husband see a urologist. Especially if his numbers are getting worse.

    Dx MFI with low to normal count, low motility, morphology 3%
    HSG normal, ultrasound and labs on me all normal. 

    1 cycle of Clomid 50mg and TI, unmonitored by OBGYN= BFN
    1 cycle of Clomid 50mg and IUI, unmonitored by OBGYN= BFN
    Started seeing an RE!!
    2 more cycles of clomid 50mg (great response), with IUI and Pregnyl trigger (4.8-8 mil good ones after wash) = all BFN
    1 cycle of clomid 50mg (3-7) followed by Follistim 75iu (7-11) + IUI = BFN
    December 2013 DH saw urologist and is taking Anastrozole, CoQ10, and L-Carnitine
    IUI #5 natural cycle (needed a med break) = BFN
    IUI #6 Follistim 75iu (CD3-10) + Pregnyl (CD11) + IUI (final count after wash 300K) = BFN
    IUI #7 Follistim 75iu (CD3-9) + Pregnyl (CD11) + IUI on 2/20 (post wash count 12.5 million)= BFN
    IUI cancelled (DH OOT) Clomid 50mg (CD3-7) 1 follicle +(not well timed) TI = BFN
    IUI #8 and last one!! Clomid 100mg (CD3-7) +OPK before US + IUI 4/17 (post wash 8.5 mil)= BFN

    Pre-IVF testing complete! SHG great and measurements taken! Labs for infectious diseases completed, FSH (5.4), TSH (1.6), Prolactin (11), AMH (2.6), Estradiol (40).

    Started BCP 5/29 and Lupron 6/11 prep for IVF #1! Started follistim 225u/day on 6/28. Monitoring on 7/2 >15 follicles measuring 11-14, E2 758. Monitoring on 7/5 all ready to go!! Great follicle sizes and lining is at 9. Tigger 7/5, ER 7/7 16R 9M 3F. Stimmed too fast in just 7 days. 7/10 3dt of 2 8-cell grade 2 & 4. 7/14 P4 >60.
    Holy crap BFP!!!
    Beta #1 (14dpo) 7/21 112 Beta #2 (16dpo) 7/23 286 a Beta #3 (18dpo) 7/25 761 Beta #4 (21dpo) 2631!!! Hold on tight little embies!!      First Ultrasound 8/7- 1 perfect little bean with a beating heart 117bpm!! EDD 3/30.
    Second ultrasound 9/2 Little bean measuring a few days ahead with a heart rate of 161!

    Yeah, you're right, the 40% motility wouldn't be the post wash number. I'll call tomorrow, but now I'm stewing about it. Does it make sense that he would give us the per mL number? So, if we had 3 mil per mL and we had, say, 6.5mL we would have 19.5 mil?

    I have only ever been told the actual count per ml (ex total 22mil per ml, 3.4ml with 35% motility) and then just the total post wash count and motility (ex then we ended up with 6mil post wash 80% motility).
    the post wash count is always much lower than what the total amount of sperm are because they wash out the bad sperm. Like when we had 22mil/ml and 3.4ml we still only ended up with 6 million post wash but the motility was higher and they were the best ones. I have never had one come back where the post wash number was higher than the count per ml. But I suppose it's possible.

    I, too, thought the 3 million was per mL, or maybe even motile per mL if motility is the concern? If so, it might explain why only 40% motility would be present post-wash to get 20 million sperm, although it's strange because I always heard they would use swim-up test to get better  motility.

    Good look at your appointment, I hope you get clear answers and a helpful plan if needed!

    It sounds to me like there was quite a bit to the sample volume wise so even though they weren't alot per  mil you still had a good amount of actual swimmers because there were so much to work with. At least that's how it sounds to me.

    Me: 27 Dh: 35 Testing Begins 3/5/13
    Six SA's show DH has low numbers across the board = severe MFI
    Genetic testing for me = MTHFR+, also carrier for blood clotting disorder Otherwise all else normal
    Dh's karotype= Normal!!
