How do you deal with this when your milk first comes in? Any and all tips invited.

  • I was seriously engorged when my milk first came in. I tried icing after feedings but that only helped while the ice was on. I ended up pumping both breasts for about 10 minutes to take the edge off and it helped tremendously!! Just don't pump too long or your boobs will think you need to keep producing a ton of a milk.

  • Ditto what the others have said! If you get too uncomfortable, use a heating pad to help with the pain or a warm shower. BUT make sure you take a hand towel in the shower with you to place on the girls, as the water can hurt when it hits your girls! 
  • My neighbor is a lactation consultant.  She suggested putting cabbage leaves directly on your girls.  I didn't do this.  Perhaps I should have as my engorgement was pretty bad for the first two weeks.  
    I did pump a little and that took the edge off.  @KariB509 mentioned this too: don't over do the pumping.  Your body will think you need milk for multiples and it can prolong engorgement

    PPs are right... this issue should resolve itself in a couple of days.  If you absolutely have to, you can pump a little if you're in pain or if your LO has a difficult time latching on.

    I had a huge (haha) engorgement issue, oversupply and overflow problems....  My little guy couldn't latch and when he did, he would just choke and cough.  It was so sad to see.  I ended up having to pump until empty first thing in the morning and then block-fed (I would feed him on one side for two feedings in a row and then switched sides for the next two feedings).  I did this for 2 days but after the first day, things were much better.  After day 2, my body regulated itself.  Good luck!

  • You can put cabbage leaves on for short periods of time. Recommendation is usually 20 minutes since extended use of cabbage leaves can decrease supplies. You crack the veins of the cabbage leaves a bit before putting them on. 

    You can also take ibuprofen and hand express to comfort. 

    If your engorgement is really bad and it's hard for baby to latch, you can use reverse pressure softening. https://kellymom.com/bf/concerns/mother/rev_pressure_soft_cotterman/

  • I did a heating pad for two minutes right before feeding and bags of frozen peas on them for 10 minutes after the feeding. It worked great!
