High-Risk Pregnancy

Intro *loss mentioned* (long)

Hi Ladies!

I have technically been high risk since I became pregnant due to a loss at 22w3d from PPROM. I haven't intro-ed because honestly everything has been smooth sailing until recently. I never had MS or any ailments other than the occasional heart burn occurrence. At 16 weeks I started Makena injections weekly (17P) as well as bi-weekly cervical length checks with the MFM.

At 21 weeks I started to feel contractions, and everyone told me they were BH so nothing to worry about. By the end of the night I was having painful contractions so I went into the hospital and they gave me terbutaline to stop them. The injection and some IV fluids helped stop my contractions, and they sent me home after an ultrasound confirmed my cervix shortened by 1 cm (previous 4.2 and went to 3.2). I followed up with the MFM about 2 weeks later and no change. I went in on Thursday and my cervix measured 2.1cm. The MFM handed me some paperwork and told me I needed to go straight to the hospital and that I would be there for the weekend.

When I arrived at the hospital and was hooked up to the monitor they found I was having contractions. I thought it was just the baby balling up/rolling around in there because they felt totally different then they did 4 weeks ago. I was given an IV of magnesium sulfate (which was TERRIBLE!) and then steroid injections to speed up baby's lung development. After 12 hours my contractions really dropped off. Then I went 24 hours without a single one. They did another ultrasound and my cervix is at 1.8 (MFM's orders stated that if I was above 1.5 I could go home on strict bed rest).

So here I am, on bed rest "until further notice" according to my doctors. I have a follow up in 5 days and then every week thereafter. Hopefully nothing changes, and I am actually really hoping that I am able to have modified bed rest where I can return to work. I am a very active person so this is driving me nuts! I wanted to introduce myself to you lovely ladies, and join the bed rest check-in board as well.

Thanks for reading if you made it through all that!! :)

Re: Intro *loss mentioned* (long)

  • Have they not mentioned doing a cerclage? I would think that it would be essential since you're so early and your cervix has shortened so much. I also had a loss due to PPROM at 22w5d in my previous pregnancy and I had a preventative cerclage placed at 14w in this pregnancy. I have had no problems with PTL and my cervix was long as of the last measurement at 28w. I'm now almost 35w and have had no bed rest or anything except pelvic rest and some lifting restrictions. I would suggest advocating for a cerclage.

  • A cerclage at this point has a lot of risks. My MFM does not perform them after 24 weeks. They didn't place one earlier because there was never a reason cited for why my water broke last time. They think it was an infection, but it took a lot to get me dilated to deliver my son so they felt the evidence did not support an IC, and they wanted to watch me. For future pregnancies I will definitely be getting a cerclage placed early.

I am really happy that your cerclage is working for you. That's fantastic that you are at 35w and you haven't had to go on bed rest. I must admit I am a little jealous about that!

  • I have no advice I just wanted to say I'm sorry for your loss and hope you keep this little one in there as long as possible.  I also PPROMed but earlier then you at 18w and will be getting p17 shots next pregnancy and am trying to decide whether to push for a preventative cerclage early on or not.

    Good Luck and FX.

  • I started bedrest at 25 weeks this pregnancy. No cervical shortening, but tons of contractions that haven't responded to much (I was on Nifedipine). I'm also on 17P due to PTL with DS1 at 29 weeks. So sorry you're facing all of this. Every single day is a HUGE victory right now so try to focus on that. I'm 37 weeks tomorrow and just went on my first walk!! Hoping a few months from now you're in my position wondering whether baby will ever come out ;-)... join us on the bedrest checkins!

I am so glad you made it to 37 weeks, that is such an accomplishment! I cannot wait to be in your shoes. I keep trying to focus on the fact that the longer my little guy stays in there, the healthier and stronger he will be. I will definitely be joining the bed rest checkins!

I didn't realize you were past 24w. Hopefully, things will calm down and you'll be able to get back to some normalcy. T&Ps being sent that your guy keeps cooking!

  • Remember, no matter how nuts bed rest makes you- every day inside is at least one extra day that baby won't need to be in the NICU. It's the only thing that's kept me going through 11 weeks of bed rest so far (with possibly another 6 to go). I

wouldn't call myself a terribly active person, but this kind of confinement isn't easy on even the most laid back person. I just keep reminding myself that I will have plenty of time to walk when baby is in the stroller after she comes home. Until then I just have to keep her baking until she's ready to come out.

  • We are due around the same time bluebird! We can do this in small steps. I also was put on modified bed rest at 21 weeks and taken out of work due to my cervix shortening. After a week of rest and starting crinone my cervix has went back and stayed neared 2.8. So, no cerclage for me either just the worry and monitoring of PTL. I am a very active person and had wanted to run through my pregnancy so this is definitely killing me slowly. But as the others said each day/week is a victory. my MFM wants me to get to 28 weeks now where the baby has almost 100% chance he said! Let's keep each other sane!

  • There are plenty of us struggling with bedrest. You are doing an awesome job taking it easy and being on bedrest for the health of your baby! My OB always said for each day your LO is inside, it saves them 2 days in the NICU.

I dealt with PTL with my son 4 years ago. Meds and bedrest for 8 weeks kept him safely inside.

Now with baby #2 I started with PTL at 25 weeks, but am now approaching 30 weeks. Each day is a victory and even though I am going crazy most days, I know I'm doing what's best for my little baby.

Keep your head up mama! Netflix, ordering baby stuff online and crochet kill time. Reading is also a good way to kill time. Definitely feel free to check in whenever with the rest of us.

  • Thanks ladies! I'm glad I found this board so I can talk to other women who can relate. Keeping our baby safe is the most important thing to me.

@Mrs_D_in_KY  Thank you for that little fact about each day saving the baby 2 days in the NICU. I didn't know that, and that will help me with each and every day that I am on bed rest :)

  • It's definitely something I remind myself each day and when I start to feel down because I feel like I'm not contributing much to the household. DH quickly reminds me I'm doing the most important job in the house, keeping the baby safe.

My doctors always said the highest level NICU doesn't compare to the mothers body. Like my 4 year old says: baby you better stay in there where it's nice warm and cozy just for you until you're big and strong.

And just think it will all be over before we know it and then well be walking zombies due to lack of sleep. Lol
