Did you have to schedule a csection?

I am 35 w 2 d with di/di twins.  Baby A (who has always been and still is bigger) is head down, face towards the back - perfect for vaginal delivery.  I've been told all along that it doesn't matter if Baby B is head down or not since she is smaller so could be delivered breech (she is currently transverse).  I will obviously do what is best for the babies but I really want to try for a vaginal birth if at all possible which it currently seems like it is.  Most of the doctors in my practice have been supportive of this plan.

My doctor's office called today to schedule my c-section for 38 weeks (Baby B has a 2 vessel cord so they don't want me going any longer) if I make it to that day.  I told the nurse I would rather schedule an induction than a c-section as I am trying to avoid the c-section unless it's medically necessary.  She wanted to schedule one so it's on the books with the dr I want in case I need it and they don't schedule inductions this far out.  I pretty much refused without more information.  We agreed I could wait and talk to the dr at my appt on Friday about it and decide then.  Is this standard procedure for twins though?  Did everyone have a c-section scheduled in case they made it that far?  I will obviously deal with having a c-section if necessary but I don't want to be forced to have one just b/c it's convenient for the dr if it isn't medically necessary for the babies.  I'd love any input from those of you who have BTDT.  Thanks!

Re: Did you have to schedule a csection?

  • I scheduled my c-sec with the knowledge it would be cancelled if I decided to try for a vaginal delivery. I was the same with twin placement. Baby A head down, B transverse. But I also wanted my tubes tied... And I was, personally, scared to try vaginal with my baby b being transverse. But, the added tubal was really my deciding factor. Sorry, not too much help.
Hope your ob gives you more info. The only thing I can think of is that a lot of hospitals require you give birth in the OR. So, maybe the scheduler is just calling it a c sec just to reserve the room if you'll really be induced.

  • I wouldn't say it's standard procedure for twins, but in my experience it was standard procedure for a possible C section. I wound up having one because both my boys were breech, but my OB told me we would schedule one just to have it "on the books" by 34 weeks, just in case. If Baby A had flipped to vertex we would've changed that C section appointment to an induction, or she would've let me go a little longer and then scheduled an induction when necessary. It feels scary to schedule a section when you don't want one and are in a good position to deliver vaginally, but remember that no one can force you to have surgery. So, go ahead and schedule it, and then as the date gets closer it should hopefully get changed to an induction (as long as Baby A stays put). It sounds like the operating room at most hospitals is a busy place, so they need to schedule potential sections several weeks out, at least.

  • I had a scheduled c/s for the latest date they were comfortable with me going...all was done with the understanding that if things were favorable for a vaginal delivery I'd do that instead. I ended up with the c/s because of double transverse.

  • My babies were is a different position everyday. My doctors had me schedule a c/s so that we would have the OR booked if we needed it. The plan was for me to show up for my c/s time and have an ultrasound. If A was head down I could go ahead with a vaginal delivery. Baby A cooperated and I was induced. Baby B was born breach. It was the perfect delivery with no complications.
  • I was told at my second OB appt, at only 9 weeks that we will be scheduling a c section even though I would prefer a vaginal delivery. She said it all depends on their position later on but just to make sure it's on the books and to avoid the craziness of an emergency c section we will definitely book it. But I hope to not need to do it. Either way they are having me deliver in the OR rather than a labor and delivery suite in case there is a complication. Often baby A will come out vaginally and then baby will flip or the placenta can get in the way, etc and then they do a c section for baby b.

  • I will be scheduling a c-section just to get it on the books.  I'm hoping to go into labor on my own to deliver them vaginally before then.

  • I would have done the exact same thing as you. I really wanted a vaginal delivery but baby a flipped breech and stayed that way. We didn't talk about scheduling until 28-30w and then I never did it. It felt so odd to pick a birthday, honestly.

Turns out I had an emergency c/s after my water broke due to a prolapsed cord, so it was moot.

  • I was able to schedule an induction for 38 weeks. Both babies were head down but the induction failed and after 4 days we did an unplanned c section. Baby a's heart rate started to drop and my cervix just was not cooperating so we went ahead with the section.

My hospital was pretty big and there were plenty of doctors and nurses there to do an unplanned section but perhaps some hospitals just like to have it on the books so that they have the appropriate staff available. There will be a TON of people in your delivery room so not every hospital has the staff to do this easily without some planning.

  • No I never had to schedule a c-section.  My water broke at 35 weeks and I went on to have a vaginal delivery.

  • I scheduled a c/s when baby A was breech by she flipped and my dr suggested that we leave the c/s on the books so they have a room, but that we would do an induction. Unfortunately, I didn't make it that far and the dr on call insisted on a c/s.

  • I am leaning toward a c-section for my di-di twins, regardless of position, and was called with the date today.  My OB said they typically do twin c-sections between 37 and 38 weeks.  Mine was schedule for 37w1d.  I told this to a friend who recently had twins in the same state, and she said legally doctors weren't allowed to electively deliver twins before 38 weeks. I am trying to look up a law or something stating this, but am having no luck.  Anyone have any insight?  I figured it was a bit earlier than anticipated for di/di, but I trusted the OB...

The standard in my practice is 38w, they will not let you go to 38+1. That being said, I have HBP and diabetes so my dr wants to schedule between 36-37 weeks due to concern about pre-e and possible still birth. She was going to consult with the other drs in the practice and we will schedule my c/s (baby A is breech and B is transverse) on Monday when I'm 35w. I'm hoping to push it to at least 37w just to avoid NICU time if possible.

Im in CA and my ob wasn't allowed to do an elective c-sec before 38. She wanted to do it at 37 but was going to have to "work it" as to not get in trouble. At 37, I was showing signs of labor but not fully in labor. We were worried for an emergancy situation. I had my ds very quickly and was worried I'd jump into labor and it would go super fast. And luckily, I showed up at hospital at 5 cms. An hour later, when they took me to OR, I was a 9.
Anyways- we didn't have to fake it.
  • I was never asked to schedule a c-section and they didn't even schedule my induction until I hit 39w.  I was induced at 39w4d and my girls were born vaginally at 40w0d.

I haven't scheduled mine yet (waiting to see if Baby A will flip) but my OB won't schedule anything before 39 weeks. I know it's best to keep them cooking for as long as possible, but I'm not gonna lie...39+ weeks with twins really freaks me out.
