Pregnant after IF

Regular Ob/Gyn with twins or someone specialized?

We've moved recently and I need to find an Ob/Gyn that is closer to where we live. For those of you expecting twins, are you seeing your regular Ob/Gyn or did your RE refer you to someone else/specialized? Is there something I should be looking for in an Ob/gyn when having twins & AMA (since this AMA always seems to be brought up)? Where there any other considerations that you considered if you did end up switching Ob/gyn because of the twins? Thanks!

  • We've only had our first obgyn appt, but she did refer us to a MFM for all of our genetic testing, since they don't do testing on twins. I have no idea what to expect from the MFM, but we have an appointment at 13 weeks, and then back with my obgyn at 14 weeks. We plan to stick with the obgyn and see the MFM as necessary. And congrats!!!!

    Me: 32, DH: 34  / TTC since February 2011 / SA: all normal, HSG: all clear! / on Lovenox for anticardiolipid antibodies
    4 IUIs with Clomid, Letrozole, and Menopur. All BFN.
    9/12: lap / hysteroscopy: found and removed mild endometriosis, cervical polyp, and 2 para-tubular cysts

    First u/s on 2/17/14: TWINS!!!!! both w/HBs of 114 at 6w3d, HBs 150 and 152 at 7w5d

    5/27/2014: Team purple!!!!  EDD 10/10/2014 / 
    Delivered by c-section at 32w0d 8/15/2014 due to preeclampsia/HELLP syndrome
    Baby Boy 4lbs 1oz, 17 inches
    Baby Girl 3lbs 5oz, 16 inches

  • I think most of the women on here with twins see both an obgyn and a MFM.
  • @Aurora1181 and @MishaBelle521 thanks for your responses! @Aurora1181 looking forward to hearing how your first appt with the MFM specialist goes & how often they suggest that you go for a visit.

  • I would've been my OBs patient with an occasional MFM appt if my cervix and babies would've cooperated :)

    Me: MTHFR, Low AMH (1.1), High NK cells and Antiphospholipid Antibodies.
    DH:MFI-Low morph (1%), DNA fragmentation: Excellent!
    5 rounds of clomid, 3 IUIs, and multiple non-medicated cycles- BFFN                                                      
    IVF with ICSI in August 2013 brought us our babies.  ER-9R, 7M, 4F w/ICSI  ET of 2-Grade 2 blasts. 
    + HPT at 6dp5dt. #1 (8dp5dt)- 105    #2 (13dp5dt)- 510  #3 (15dp5dt)- 960  #4 (17dp5dt)- 1889
    Dx shortened cervix, PTL, and preeclampsia during pregnancy.  Lots of medications and 13 weeks of bedrest, babies were born healthy at 34w4d!

  • We are in Ontario so we have a midwife for our primary care with consults with the hospital MFM and OB teams as needed.

    We had the option to meet exclusively with the multiples clinic at the local hospital but elected to stay with the midwife because continuity of care and post-partum support was important to us.

    Partner IVF#1a- December 2013 - H's eggs, my Ute - CANCELLED due to low response
    Partner IVF #1b - February 2014 - H's eggs, my Ute - ER February 4 (10 retrieved, 3 fertilized), Transfer Feb 7 of one Grade 1 and one Grade 2 day 3 embryos.  1 - Day 3, Grade 1 frosty saved.   BFP - 6dp3dt via FRER, Beta #1 - 110, Beta #2 175, Beta #3 - 348, Beta #4 - 2222!, Beta #5 - 4255.  Ultrasound (6w1d) - 2 heartbearts!  

    We lost our beautiful Twin baby girls on June 18, 2014.  Tavin Sara and Casey Elizabeth were born at 21 weeks gestation and were absolutely beautiful, precious, amazing babies.  We miss our daughters every day and love them with all our hearts.

  • I had my NT test/genetic counseling and anatomy scans with MFM, as well as a consult. At the consult the MFM laid out their recommendations for the duration of my pregnancy. One of recs was for growth scans every 4 weeks after 22 weeks, and those ultrasounds are always read by one of the MFMs. Otherwise, everything is with my OB.

  • I alternate between seeing a high risk doctor and my regular OB.  I see my MFM during my cervical length and he was the one to refer me to have an Echo done on baby B.

  • Thank you so much everyone! I just scheduled my Ob appt and saw that they work closely with a MFM practice. So I'll wait until that first appt. & see what they say. But good to know a head of time that I may have some extra monitoring or have to see another specialist.

  • We saw an OB and MFM and it was great seeing my babies weekly on ultrasound !
  • I'm only seeing an OB and a little apprehensive about it. When I ask, she said the mfm doesn't do anything different.

    I just had my first OB appt yesterday and he said I will stay with him as far as overall care goes, but the MFM will do my NT, my AS and will likely see me 2-3 more times later in pregnancy for u/s cervical checks and NSTs. I am AMA and IVF, but not high risk enough (right now) to be fully transferred.

    ETA: my growth scans are every 4 weeks until 20 weeks then every 2 weeks to 30 weeks then every week from 30+ and every other day from 36+ if we make it that far. Induction/cs/delivery to be set for 37 weeks if they aren't here before depending on how they are holding up. 37 just seems early to me...

    IF Dx: Endo, hetero MTHFR mutation, poor morphology
    #1 IUI: 1/18/12 = BFN
    #1 IVF/ICSI 4/2/12 = 2 x 7-cell and 1 x 5-cell transferred (3dt) = BFP!!
    H was born at 41w2d on 12/29/12 - be still my heart!
    #2 IVF/ICSI 1/19/14 = 2 x 8 cells transferred (3dt) = BFP!! EDD 10/09/14
