Fortify with oil?

Does anyone know anything about fortifying breastmilk with oil? @babycaps @TallAsh‌? We are still struggling with this weigh gain thing . . .

I have been googling and can't find much. The cardiologists are pretty loose about it and just say to use whatever. They have me the measurements. Just curious is anyone knows anything. Thanks!

If this doesn't work we are going to consider hind milk pumping but I don't think I can pump frequently enough for that. Then we are going to go to Neosure. I've got 2 weeks to fatten him up.

Re: Fortify with oil?

  • We are trying to avoid neosure bc of his past tummy troubles. The oil is supposed to be a lot milder on his stomach.

  • Our pedi recommended this with DS1 as he measured in the 0% for the first several months and was an IUGR baby. I think we only did it once though.

FWIW it didn't seem to bother him/upset his tummy, but then again nothing seemed to really upset his tummy.

We didnt' continue it b/c we couldn't really find any info about it or anyone else that had done it and we didnt' think he was that little.

  • Thanks Yankeepeach.

Will is doing weight checks at both the pediatrician and at the cardiologists. The pedis are ok with his weight gain but his cardiologists aren't. I'm a bit frustrated but trying to remember it's all in the name of better surgery outcomes.

  • The two week limit is just to show some weight gain or we will switch to a fortifier. He had such trouble last time that we are trying to avoid it.

As for type of oil she just said it didn't matter. She wrote olive oil or corn oil on his papers. I'm weird about corn so I won't do that. I asked on my heart board too and the majority of them used coconut oil.

Honestly I'm a bit lost on this. I'm going to go in to weigh him at the pedis tomorrow on his normal scale. I'm also going to start weighing him regularly at home. I feel like this cardiologist kind of jumped to weight issues.

What are his weights so far? Normal is 1/2 to 1 oz a day weight gain but if baby has any kind of health issues drs seem to push hard to get that 1 oz a day which just may not be normal weight gain for your baby.

It sounds like you are doing some pumping? If so pump twice and take the fat off the top of one bottle and add it to the other bottle. That gives you one low cal bottle to freeze and one higher cal bottle to give baby.

Before nursing or pumping also use breast compressions and massage to speed the flow of milk and the massage helps to loosen the fats stuck to the walls of the ducts. This helps baby get to the hindmilk faster.

Have you seen an LC for hints to help with weight gain? Good luck.

AmyG you probably don't know our history which makes things more complicated. He was born at 5 lbs 2 oz and weighed 5 lbs 10 oz yesterday at 5 weeks. He has some major health issues that impede his growth so I don't disagree with the doctors that he is a slow gainer. He has a big heart defect which makes him burn more calories than he can take in and he needs to gain substantial weight to be big enough for open heart surgery. I more disagree because he is being switched between scales. I'm going to my pedi this morning to use their scale to see where we really are.

We EP and work with LCs frequently bc he won't latch. He doesn't like my nipples after getting used to the bottle in the hospital. The LCs won't give me opinions on anything the doctors say bc of his issues. I am trying to hind milk pump in addition to the oil (at the advice of the doctors) but right now I'm not able to pump enough. One thing we have going for us is my doctors are very pro-breastmilk. It is very beneficial for his problems.

MCT oil is partially comprised of coconut oil, so that would make sense why other heart moms have used that.

I can't help but think it would be more palatable as well...

Is Will only taking oral feeds? Is he getting tired?

I'm just asking bc some of our babies do go home with an NG tube to help with feeds. If he's not able to take a full bottle feed, could that be an option before switching him to a formula/fortifier that he doesn't tolerate well?

@Tallash - he only takes oral feeds. He's actually really good at it now so no problems there. The doctor told me yesterday they are happy with his intake and he takes the amount they would expect. It's just that his large VSD is making him burn more than he can take in. They want him 24 cal/oz but not necessarily more ounces. I'm sure you see that a lot. He's still very sleepy but it doesn't seem to be impeding his eating.
