Second Tri Sequential Screen

Hi ladies, I'm currently pregnant with di/di twins and just got the results from my 2nd trimester sequential screen back, which has confused me terribly.

Back when we had our 1st trimester NT scan and sequential screen, we had an elevated risk of Down Syndrome because my HCG was 1.98 MoM, everything else looked good including both NT measurements (2.3mm for A and 2.5mm for B). After discussions with our doctor, we decided to have cell free DNA testing done via the Harmony test. Results came back as <1/10,000 so that brushed any concern we had away.

Fast forward 5 weeks to when I just had bloodwork drawn for the 2nd trimester sequential screen and that is now indicating my HCG is still high (2.73 MoM) and that we are still at an increased risk of having DS.

I had asked the nurse practitioner at the visit where they drew the blood last week why they were doing this and she said something about well they re-calculate the algorithm using the 2nd tri results and things almost always come back fine at that point. She couldn't answer me about the value of the 2nd tri screen over the Harmony test results.

The scientist in me is saying ignore the sequential screen as the DNA testing would have picked up any abnormalities but the new mom in me is freaking out while waiting for my doctor to call me back. Did anyone go through this before? Believe the Harmony results or sequential screen results?

  • When my sequential quad screen at 16 weeks came back with elevated risk of DS.  I opted for the Maternity21 test (very similar to the Harmony, or maybe even the same thing).  Once those resulted came back negative we were done testing.  The Maternity21 results over-ride the blood work results from my understandings.  The sequential screening is not specific to multiples so the results are difficult to interpurt.

  • I'm sorry I have introduced myself yet but here is my two cents. 

    -I was told that the blood test portions of the NT and the second tri screen are not accurate with multiples so they just go by the US measurements. This makes sense to me.

    -I had Mat21 (similar to Harmony) and got conflicting answers about whether the test is accurate for multiples. Sequenom website which makes the test says it is accurate for multiples. I spoke with a friend of mine who uses the test in his job and he says the test is accurate for multiples. Based on my understanding of how the test works (I have a medical background) it seems like it should be accurate for multiples

    So for me, I would be VERY reassured by a negative Harmony or Mat21, especially since your NT ultrasound measurements were good.
